About the Job:
The MDS Coordinator is responsible for planning and organizing the MDS/RAI process in coordination with the RAI/PPS Coordinator. This position assures compliance with all State and Federal MDS transmission requirements. The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.

Works within coordination of RAI/PPS Coordinator for scheduling and proper coding of all new admission and PPS assessments.

Consults with IDT members for pertinent clinical information as needed. Utilize direct interview/observation skills, chart audits, resident/family interviews and communication with direct care staff when completing assessments.
Works with Interdisciplinary team to determine resident significant change in status and OMRA assessments.
Completes Nursing Sections of MDS per facility policy and ensures completion of assigned sections by other disciplines.
Follows and updates facility MDS schedule and tracking forms per policy and procedure.
Follow facility policy and procedure on Resident Care Planning.
Ensure that care plans accurately reflect the cares and clinical monitoring provided.
Audits completion and review of completed MDS assessments randomly on each floor monthly.

Job Responsibilities:
CARE COORDINATOR WRAPAROUND PROGRAM Wisconsin Community Services Working under the Wraparound Practice Model of care, which includes a unique set of values that emphasizes individualized, strength-based, trauma-informed, community-based and culturally intelligent care, provide intensive and individualized care coordination for children and adolescents.
Essential Functions:
1. Maintains a caseload of approximately 8 children and families.
2. Contact enrolled children and families within 48 hours of enrollment, and set up initial visit to occur within 5 business days of enrollment to introduce self and supports available.
3. Schedule and attend weekly visits with assigned children and families, facilitate monthly Child and Family Team Meetings to discuss treatment planning, and maintain regular phone contact with assigned children and families and service providers as needed.
4. Assemble a Child and Family Team within two (2) weeks of enrollment by interviewing the family, identifying family members/natural supports/service providers and other significant persons.
5. Within the first 30 days, coordinates the meeting of the Child and Family Team and develops the initial individualized Plan of Care based on the identified strengths and needs, including a comprehensive 24-hour Crisis Plan.
6. Monitor the provision and quality of services provided to the family through the Child and Family Team and is the liaison when new services/resources need to be sought or developed.
7. Advocate for Children and Families across a variety of settings, including home, educational, court, and community settings, as needed.
8. Attend all scheduled court hearings as a representative of Wraparound Milwaukee to support children and families throughout the court process and explain supports available/in place.
9. Seek community resources with the assistance of the Team and modify the SAR whenever services or resources need to be added and/or deleted.
10. Provide or arrange for transportation for youth/young adults to appointments, crisis/respite services, including assisting a child and family with identifying natural supports and sustainable transportation plans, etc., if needed.
11. Provide or secure support and crisis/emergency services for youths enrolled and/or their families. This may be done through face-to-face contact, phone contact/ availability or an on-call system.
12. Completes all necessary paperwork in a strength-based manner per Wraparound Milwaukee/Agency requirements,(i.e., court letters, Plans of Care, change of placement forms, SARs, Referrals, Progress Notes, evaluation instruments, consent forms, etc.).
13. Maintain accurate information online, ensuring that family demographic information and Child and Family Team Member contact information is up to date.
14. Collaborate with other necessary individuals the youth and family may have contact with.
15. Keep Wraparound Milwaukee informed of all emergencies or critical incidents (refer to Wraparound Policy #14 - Critical Incident Reporting).
Other Duties and Responsibilities:
1. Attend in-services and participate in staffings, weekly/monthly meetings, and consultations, as needed.
2. Assist with child and family coverage for co-workers as needed or directed.

• Must possess a bachelor's degree in a relevant area of education or human services and a minimum of one year of continued experience providing mental health services.

• Working knowledge of computer applications preferred (Windows and Microsoft Word).

Working Conditions:
Work is performed primarily in a busy office environment managing caseloads of children, young adults and families. Some of the work is done sitting at desk using a computer; requires outreach in the community and families' homes; much of the outreach is done in urban neighborhoods and several hours per day may be spent driving; hours average 40 per week; flexible work schedule include some hours outside the normal work schedule on evenings and weekends.

JOB QUALIFICATIONS
EDUCATION
For positions requiring education beyond a high school diploma or equivalent, educational qualifications must be from an institution whose accreditation is recognized by the Council for Higher Education and Accreditation.
Minimum Required: High school diploma or equivalent. Successful completion of a medical/dental terminology and an anatomy & physiology course, per departmental procedures, within one year of hire.
EXPERIENCE
Minimum Required: Two years' experience in a medical/dental business office or health care setting. Prior experience should include working with and obtaining insurance eligibility and benefit information.
CERTIFICATIONS/LICENSES
The following licensure(s), certification(s), registration(s), etc., are required for this position. Licenses with restrictions are subject to review to determine if restrictions are substantially related to the position.
Minimum Required: Basic Life Support (BLS) certification awarded within 90 days of hire.

About the Role
Lutheran Social Services (LSS) is seeking a compassionate and detail-oriented Forensic Care Coordinator to provide comprehensive, person-centered case management for individuals involved in Wisconsin's forensic programs. This role requires ongoing professional development in Motivational Interviewing and other evidence-based practices, with monthly coaching and quarterly fidelity reviews.
You will work with clients in one or more of the following programs:
Conditional Release (CR): Supporting individuals found not guilty by reason of mental disease or defect.
Outpatient Competency Restoration Program (OCRP) & Jail-Based Competency Program (JBCR): Assisting adults adjudicated not competent to stand trial but likely to regain competency.
Opening Avenues to Reentry Success (OARS): Voluntary program supporting individuals transitioning from correctional institutions back into the community.
Supervised Release (SR): Coordinating services for individuals committed under WI Statute 980 (sexually violent persons) as they transition from secure treatment centers to community living.
Forensic Care Coordinators collaborate with treatment teams, community providers, and natural supports to promote independence, recovery, and successful reintegration.
Key Responsibilities
Conduct assessments of client needs using required tools (e.g., functional screen).
Develop and implement individualized service plans with clients, families, and providers.
Coordinate and authorize services according to service plans.
Provide trauma-informed, evidence-based direct services.
Maintain accurate documentation and electronic client records.
Testify at legal proceedings and prepare court documents as required.
Facilitate person- and family-centered team meetings.
Collaborate with community resources, treatment teams, and supervisors to determine interventions.
Participate in staff development, supervision, and professional seminars.
Meet productivity expectations and agency standards.
Travel daily to community locations; occasional overnight travel may be required.
Qualifications
Education: Bachelor's degree in a human services field (examples include Social Work, Psychology, Criminal Justice, Counseling, Community Mental Health, Rehabilitation Counseling, Vocational Counseling, etc.).
Experience: Prior experience in human services preferred.
Licenses/Certifications:
Valid driver's license and reliable transportation.
Satisfactory driving record per LSS Driver Safety Procedure.
Ability to meet LSS auto insurance requirements.
Skills & Competencies
Strong communication and documentation skills.
Knowledge of casework principles, human behavior, and social problems.
Ability to work collaboratively with diverse teams and clients.
Proficiency with electronic health records and computer applications.
