Make a Difference The Patient Care Coordinator will provide daily, shift-to-shift leadership to support the operations of the Ortho Med/Surg Unit. As the Patient Care Coordinator, you will exhibit excellence in clinical patient care, customer service, critical thinking, schedule management, relationship competencies, and organizational skills. You will have the authority and autonomy to ensure the appropriate clinical and customer satisfaction outcomes are met, while effectively leading your team. The Ortho Med/Surg unit at Community Hospital East is an 18-bed unit that cares for orthopedic surgical patients including hip/knee replacements and fractures. In addition to caring for orthopedic patients, you will also gain experience working with surgical patients including, but not limited to: colorectal, urology, OBGYN, laminectomy, and complex medical patients. Physical Therapy and Occupational Therapy staff are housed on this unit with a state-of-the-art PT/OT therapy room. The ortho team promotes relationship-based care and emphasize the importance of self-care for our team. Exceptional Skills and Qualifications Applicants for this position should be able to collaborate with others in a team setting, have excellent communication skills, and a positive attitude toward problem-solving. Graduate of National League for Nursing (NLN), Commission on Collegiate Nursing Education (CCNE), Accreditation Commission for Education in Nursing (ACEN), or National League for Nursing Commission for Nursing Education Accreditation (NLN CNEA) accredited school of nursing, or three years of related professional nursing experience. Licensed as a Registered Nurse (RN) with a valid license to practice in the state of Indiana as listed in the Nurse Licensure Compact (NLC). Bachelor of Science in Nursing preferred. 2+ years experience as an RN within the specialty area or related specialty area preferred Computer skills for documentation and work-related purposes are required. Demonstrates accountability for own actions with an openness to change and learning. Demonstrates customer service skills to provide exceptional patient and family experience. Takes initiative to help others. CPR certified.

Reviews prospective admissions against approved admission criteria, policies, and procedures. * Generates, guides, and leads independent discussions with referral sources for client support, conflict resolution and business development with other treatment and healthcare professionals, * Responds and reacts to the needs of referred clients. * Assesses family needs for mediation, sober escort and intervention and determines which professional resources are most appropriate to contact based on information gathered. * Decides which Interventionist and facility to recommend, the dates, and the travel. * Works closely with other department as well as external parties to ensure the timely admission of clients. * Documents daily all activities, contacts, appointments, and events in the designated Customer Relationship Management system. Maintains all the documentation involved with the admissions process. * Completes insurance verifications for all clients per CBO guidelines on all private insurance admissions. * Screens calls from referrals and sets up transportation for admission. * Maintains a waiting list for admissions and stays in contact with potential candidates to facilitate admissions. * Greets and assists clients, visitors and staff at the front office. * Oversees the traffic flow and incidents in the lobby area. EDUCATION/EXPERIENCE/SKILL REQUIREMENTS: * High School Diploma or equivalent required. Bachelor's Degree in social work, psychology, counseling, or other related field of study preferred. * One or more years' experience in healthcare admissions preferred. Preferably in the mental health or substance use disorder field. * Experience in client assessment, referral, treatment planning and communication with external review organizations or comparable entities. * Knowledge of community resources. * Outstanding interpersonal and interviewing and assessment skills. Skill in telephone etiquette and paging procedures. LICENSES/DESIGNATIONS/CERTIFICATIONS: * CPR certification and de-escalation training required (training available upon hire and offered by facility). * First aid may be required based on state or facility requirements. The Intake Coordinator will 1) determine the eligibility of those veterans who seek assistance by completing the screening forms during the initial interview; 2) process and prepare preliminary assessments of all very low income veteran families referred to the SSVF Program; 3) make appropriate referrals to all community partners and resource providers for crisis intervention and short term housing stabilization; 4) complete a screening over the phone and an assign eligible participants to the applicable case manager 5) enter enrolled clients into the Homeless Management Information System (HMIS); and 6) update and maintain all Excel spreadsheets or client trackers. WHAT YOU SHOULD HAVE FOR THIS ROLE: Bachelor's degree and less than five (5) years of experience in the field; a person with five (5) years of experience in the field and no degree; a veteran with three (3) years related work experience. Must have knowledge of customer service skills; previous telephone system experience and proficient computer skills; well organized, self-starter, and able to work as a member of a team. Excellent verbal and written communication skills, high degree of organization, and creative problem-solving skills.
