Essential Job Functions:
Ensures the timely and adequate delivery of hospice services to the terminally ill patient and their family, operating within the plan of care as established by the hospice team and attending physician.
Assesses the total needs of the patient/family during regularly scheduled and after hour home visits; Documents: assessment, identified problems, nursing interventions, goals, and outcomes of interventions.
Coordinates total patient/family hospice care under the supervision of the Director and with the interdisciplinary team, the attending physician, and other providers; documents such coordination.
Collaborates with the interdisciplinary team in the development, review and revision of the clinical component of patient/family plan of care, including exchange of information, review of problems, assessing effectiveness of interventions and documenting outcomes.
Ensures continuity of care between patient/family, team members, ancillary providers, long term and inpatient care facilities, and the attending physician.
Communicates patient/family/caregiver needs, ongoing nursing assessment, interventions, goals and outcomes through the interdisciplinary process.
Provides and documents continuing education of the hospice concept of care to patients and their families/caregivers.
Supervises the delivery of patient care provided by Hospice Aides, Licensed Practical Nurses; ensuring compliance with the established plan of care and completing required documentation of supervision.
Facilitates the delivery of hospice services to patients residing in long term care facilities, documenting joint coordination of care with facility professional staff, and attending facility care plan meetings as appropriate.
Participates in on-call rotation for delivery of care after office hours, on weekends, and holidays.
Provides ongoing monitoring of patient appropriateness for hospice services and completes required documentation for certification and recertification.

Position Description:
A CareCoordinator provides person-centered assessment, planning, facilitation and advocacy for options and services to meet the needs of elderly or adults with disabilities who are 65 or older and enrolled with Medica Health Plan (MSHO/MSC+). Service area includes Hennepin and Ramsey County.
This position monitors the health and safety needs of individuals and intervene as necessary, ensure effectiveness of services provided and collaborate with other professionals working with those you serve.

This position offers a Hybrid work model where the selected candidate may work in the Office or Remote from home.
Essential Functions:
95% Case Management
Provide person-centered case management services that assist individuals enrolled on Medica Health Plan in locating and gaining access to needed waiver and other community services while meeting the person's goals related to medical, social, housing, and various other preferred goal area.
Complete assessments in the community, at facilities or via telephone per Medica contract to determine level of care and individual needs; gather information relevant to each person's social and medical history by interviewing person and key family member.
With the individual's involvement, develop a person-centered care plan and monitor needs.
Evaluate, facilitate and monitor services and individual progress in relationship to established goals and objectives. Works with the person to adjust service plans as necessary
Provide education surrounding benefits including Medicare and Medicaid.
Act as a liaison with community and referral services
Maintain necessary records for continuity of service and reports to funding agencies
Maintain accurate case notes. Document all actions related to case management coordination for the individual. This includes all face-to-face and indirect interactions or written correspondence with the individual, individual's support system, service providers, and county/health plan staff. Documentation should reflect that active and consistent activity is taking place in an effort to support the person's plan.
Complete all required paperwork, reports, and charting simultaneously and in an organized manner

Assist with training new case managers as needed.
Offer support to members of the team in area of expertise.
Participate in team meetings, agency all staff meetings, health plan meetings and trainings.
Actively participate in on-going professional growth and development; maintain appropriate professional behavior and participate in appropriate supervision.

Qualification Requirements
Bachelor's degree in Social Work or related field.
Two years experience working with Minnesota Home and Community Based Wavier Programs (AC, EW, CADI, DD, BI, CAC) or Health Plan CareCoordination Experience with MSC+/MSHO.
Experience working with seniors or disabled adults in a community based setting, including experience with assessing strengths and needs, and providing information, referral and follow-up to other community resources.
Working knowledge of case management method of social work practice. Culturally competent. Excellent written and verbal communication skills. To perform this job successfully, candidate chosen must be competent in computer skills including database and internet experience as well as ability to type proficiently.
Strong organizational skills, attention to detail, flexibility, teamwork and the ability to manage multiple demands each day. Ability to work independently while using supervision appropriately.
A valid Minnesota driver's license and current insurance as travel within Ramsey County and surrounding areas for required home visits.
Must be able to pass a background and motor vehicle check.
Qualification Preferences
Minnesota Social Work Licensure is preferred.
Three or more years of social work experience working with Medicaid Waiver programs in Minnesota (Alternative Care, Elderly Waiver, CADI, DD, and BI Waivers) or working with a Managed Care Organization providing Minnesota Senior Health Options (MSHO) or MinnesotaCare Options Plus (MSC+)
Physical/Mental Requirements
Physical working conditions include frequent computer work and telephone calls and moving around the office. In addition, this position requires the employee to drive to meet with clients in their homes on a daily basis.
Mental requirements include verbal and written communication abilities to conduct client assessments and develop care plans. Must possess good people and listening skills and be able to maintain client confidentiality. SUMMARY OF PRIMARY FUNCTION Under the direction of the Dispatch Supervisor-RTC Program Manager or Transportation Programs Manager and in line with Tri-Valley's philosophy, mission, and goals, the Transit Route Scheduler's primary responsibilities include daily scheduling and dispatching of the Heartland Express Transportation services as well as related word processing, and general program support duties. Route scheduling responsibilities include, but are not limited to coordinating rides, scheduling special trips, ticket management and sales, monthly reports, and public relations. Consistently displays a positive, respectful demeanor toward callers, clients, vendors and drivers.
ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Primary Duties and Responsibilities
Answers incoming calls; assists or transfers to others as appropriate.
Operates a variety of communication equipment including radio, telephone, computer and scheduling system.
Schedules daily calls and reservations in Marshal, Kittson, Pennington, Polk, Red Lake, Clearwater, Norman and Mahnomen Counties; including the in-town commuters, rural and inner-city buses using a dispatch software program.
Monitors radio, vehicle location via software for information regarding scheduling, issues and field situations; notifies supervisor of situations requiring attention.
Provide a variety of public transportation information to passengers, the general public and other public agencies; provide quality customer service and respond to public inquiries and requests for service in a courteous manner.
Respond to emergency situations as requested including hazardous and sever weather conditions. Coordinate with supervisors, police and other dispatchers as required.
Communicate with passengers as needed.
Answers questions within the position responsibilities and transfers on to others as appropriate.
Tracks and monitors allocation of bus tickets to drivers.
Prepares the rural route and special trip listings for the drivers.
Prepares and enters a monthly calendar of all routes into dispatch software program.
Helps to notify the public of closings.
Tracks riders' no-shows.
Schedules daily bus routes in response to passenger service requests (same day service). Review, organize, and maintain trip manifests to maximize route efficiency.
Maintains riders' profiles; including home addresses and phone listings on all new and existing riders.
Secondary Duties and Responsibilities
Creates and maintains various documents, reports, spreadsheets, and data as directed by supervisor.
Assists in preparing monthly Status and Annual Reports.
Schedules special trips as requested.
Has general knowledge of Jefferson routes.
Sells Transit bus tickets including Summer Kid passes.
Stays up to date on the FTA part 40 requirements to make reasonable suspicion referrals.
Serves as a back-up Route Scheduler for the RTC program.
Helps maintain the cleanliness of the office location.
Performs other duties and tasks as assigned by supervisor.
MINIMUM REQUIREMENTS
Associates Degree or two years equivalent of experience working in a similar position such as office support, dispatch, communications, or reception/switchboard utilizing phone, organizational and communication skills with the public.
Two years of experience working with computers, including Microsoft Word and Excel, email and internet.
Ability to understand city street structure and to effectively communicate city and rural directions to others.
Excellent interpersonal, written and verbal communication skills.
Pleasant and professional phone skills.
Ability to work as a part of a team. Ability to establish and maintain effective working relationships with others.

In this role, the Case Management Coordinator will own the customer experience for Pella customers from warranty through the life of their product. This includes processing requests as needed through review, ordering, scheduling, and post appointment follow up until the service request is fully complete. This process will include the following:
Research and troubleshoot product issues in an accurate and timely manner.
Update all systems as needed throughout the journey of a case. Each touch point with the customer requires an update.
Quote. order parts and collect payment at the appropriate time for cases as needed.
Schedule and confirm service appointments with customers with the correct number of technicians.
Schedule delivery for non-labor related service needs.
Request compensation from Pella Corporation as appropriate and in a timely and accurate manner.

Work well cross functionally to maintain smooth customer communication across departments as needed with PC Team, sales, warehouse/shipping, technical support, and management.
When team members are on vacation, out sick or we have an high level of volume, this role will also be expected to support the phone queue in answering in-bound calls with a positive attitude and confidence that tells our customer that we will be able to help them with their Pella service issue.
