Essential Duties:
Responsible for being aware of where all assigned residents are and what they are doing at all times. When assigned to a one-to-one with a client, the requirement is to have eyes on the client at all times during the assigned period. If observing a client leaving the premises, the expectation is to follow them until further assistance can be provided.
Facilitate residential daily schedule including participating in, and providing support to residents, to include but not limited to, activities such as chores, deep clean of designated space, meal service, laundry service, groups, activities, and events
Provide transportation to residents for community outings and meetings in company vehicle in a safe and legal manner.
Practice compliance with all policies, record keeping requirements, checklist assignments, assigned documentation, and any other assigned duties.
Remain current and up to date on all trainings and certifications required to maintain employment with Newport Academy.
Qualifications:
H.S. Degree or GED required; bachelor's level degree in psychology, counseling or sociology preferred. Prior work experience in other behavioral health treatment settings with adolescents a plus. Must have a valid driver's license and be eligible for insurance coverage for driving the company's vehicles.

Job Summary:
The Patient Services Care Coordinator is responsible for organizing and managing all aspects of a patient's care at TCPC. They will support the surgery center and provider care teams by delivering administrative and patient care services. Their goal is to take initiative to improve the patient's care and experience. Essential functions of the Patient Services Care Coordinator position include, but are not limited to:
Direct contact with patients, both in-person and via telephone.
Schedules patient appointments relating to surgery and procedure orders. Acts as direct contact for surgical procedure and injection scheduling.
Communicates with the healthcare team about a patient's treatment plan.
Helps patients complete paperwork.
Makes phone calls.
Coordinates with other departments and outside facilities.
Completes pre-procedure screening and education.
Assists with prescription prior authorization and completion of forms.
Coordinates patient appointments, including diagnostic tests, physical therapy, and pain psychology, in accordance with the patient's treatment plan.
Assists clinical team with patient tracking.
Updates patient information in EHR including medications, allergies, primary care physician, pharmacy, etc.
Enrolls patients in the patient portal.
Coordinates the transmission and acquisition of necessary patient records.
Manages all anticoagulant hold requests. Facilitates timely completion of the request. Contacts patient, prescriber, and pharmacy as needed.
Cross trains to other departments as needed.
Prepares and sends out information packets to patients prior to surgery.
Communicates with Medical Device Representatives to coordinate in-person device support.
Communicates with Providers and Physicians as needed regarding appointment scheduling and educational materials.
Complies with organizational and HIPAA policies and regulations to ensure patient care is kept confidential.
Performs miscellaneous job-related duties and tasks as assigned.

This position has no supervisory responsibilities.
Qualifications:
High school diploma/GED plus one year of medical clinic experience.

Associate's or Bachelor's degree (preferred).
Minimum of one year of strong customer service with an exceptional ability to listen and communicate with others required.

Position Description:
A Care Coordinator provides person-centered assessment, planning, facilitation and advocacy for options and services to meet the needs of elderly or adults with disabilities who are 65 or older and enrolled with Medica Health Plan (MSHO/MSC+). Service area includes Hennepin and Ramsey County.
This position monitors the health and safety needs of individuals and intervene as necessary, ensure effectiveness of services provided and collaborate with other professionals working with those you serve.
This position maintains a caseload of 60 - 75 clients but can be subject to change per Medica contract requirements or at the discretion of the Program Director.
This position offers a Hybrid work model where the selected candidate may work in the Office or Remote from home.
Essential Functions:
95% Case Management
Provide person-centered case management services that assist individuals enrolled on Medica Health Plan in locating and gaining access to needed waiver and other community services while meeting the person's goals related to medical, social, housing, and various other preferred goal areas
Complete assessments in the community, at facilities or via telephone per Medica contract to determine level of care and individual needs; gather information relevant to each person's social and medical history by interviewing person and key family members
With the individual's involvement, develop a person-centered care plan and monitor needs
Evaluate, facilitate and monitor services and individual progress in relationship to established goals and objectives. Works with the person to adjust service plans as necessary
Provide education surrounding benefits including Medicare and Medicaid
Act as a liaison with community and referral services
Maintain necessary records for continuity of service and reports to funding agencies
Maintain accurate case notes. Document all actions related to case management coordination for the individual. This includes all face-to-face and indirect interactions or written correspondence with the individual, individual's
support system, service providers, and county/health plan staff. Documentation should reflect that active and consistent activity is taking place in an effort to support the person's plan
Complete all required paperwork, reports, and charting simultaneously and in an organized manner

Qualification Requirements
Bachelor's degree in Social Work or related field.
Two years experience working with Minnesota Home and Community Based Wavier Programs (AC, EW, CADI, DD, BI, CAC) or Health Plan Care Coordination Experience with MSC+/MSHO.
Experience working with seniors or disabled adults in a community based setting, including experience with assessing strengths and needs, and providing information, referral and follow-up to other community resources.
Working knowledge of case management method of social work practice. Culturally competent. Excellent written and verbal communication skills. To perform this job successfully, candidate chosen must be competent in computer skills including database and internet experience as well as ability to type proficiently.
Strong organizational skills, attention to detail, flexibility, teamwork and the ability to manage multiple demands each day. Ability to work independently while using supervision appropriately.
A valid Minnesota driver's license and current insurance as travel within Ramsey County and surrounding areas for required home visits.
Must be able to pass a background and motor vehicle check.
Qualification Preferences
Minnesota Social Work Licensure is preferred.
Three or more years of social work experience working with Medicaid Waiver programs in Minnesota (Alternative Care, Elderly Waiver, CADI, DD, and BI Waivers) or working with a Managed Care Organization providing Minnesota Senior Health Options (MSHO) or Minnesota Care Options Plus (MSC+)
Physical/Mental Requirements

JOB SUMMARY:
Hiawatha Valley Mental Health Center is seeking a Care Coordinator responsible for coordinating integrated medical and behavioral health care for consumers. The care coordinator arranges regular meetings of the multidisciplinary treatment team, enlists consumers' participation on the team, coordinates the completion of an integrated treatment plan for each consumer, monitors symptoms and response to medical and behavioral health treatments using structured instruments and consumer interviews, works closely with the primary care and behavioral health care providers to monitor consumer outcomes and revise the treatment plan as necessary to ensure the appropriate level of care is being provided through a person-centered system of care. The Care Coordinator organizes consumer care activities to meet the consumer's medical and behavioral health care as well as social services, housing, educational systems and employment opportunities as necessary to facilitate wellness and recovery of the whole person. The Care Coordinator also shares information among all of the participants concerned with a consumer's care to achieve safer and more effective care.
JOB RESPONSIBILITIES AND ESSENTIAL FUNCTIONS:
Monitors consumer compliance with Certified Community Behavioral Health Center (CCBHC) data outcome measures.
Consults with consumer and multi-disciplinary treatment team about treatment options and preferences;

Primary Function of the Position
Hmong American Partnership is seeking to hire COVID Care Coordinators that want to support the safety, health, and wellbeing of the Hmong community. This position is primarily responsible for the delivery of the HAP COVID Community Coordinator Project within the Health and Wellness Department. The candidates must demonstrate a strong work ethic, be customer-service driven, be accountable, and demonstrate excellence in quality work in previous work experiences. Primary responsibilities include: working closely with clients to help them access community resources, enter information into enrollment tools and resources, making appropriate referrals, responding to questions about COVID testing, outreach activities, and staffing MNIT Amazon hotline. This position will provide in-person assistance and also work remotely to support the Hmong Community.
Position Responsibilities
Staff HAP's MNIT Amazon hotline daily during work hours and weekends and evenings as needed.
Answer calls and follow up on voice messages left on the MNIT Amazon hotline.
Attend orientation training for MNIT Amazon hotline.
Attend updated COVID training as needed by MDH and HAP.
Pre-register members for testing on a regular basis.
Respond and follow-up to community questions about test sites, registration questions/issues
Attend and be on-site to support, provide coordination, and translation support to local public health testing sites, as needed.
Connect community members and clients to COVID resources and navigation services.
Provide information on COVID-19 case investigation, contact tracing, and other information to clients and community members.
Navigate the COVID-19 health system on behalf of the client and community members.
Support project coordinator in areas of outreach and community engagement e.g. drop off flyers and materials at distribution or testing sites
Enter client data into tracking sheet; ensure timely and accurate entries.
Participate and support coordinator in program logistics as needed e.g. gather materials or call partners to participate in testing sites.
Provide and fill out COVID-19 application, intake, and referral process for MNsure.
Perform other duties as assigned.
