Documents services provided in electronic filing system. Assists with referrals for economic assistance, social services, housing resources and educational services. Supports life skills advocacy, treatment follow up, disease management and educational services, including referral and linkages to long-term services and supports. Qualifications

Job Summary

Assists with referrals for economic assistance, social services, housing resources and educational services. Qualifications

Job Summary and Responsibilities
Works under the supervision of the Director of Nursing. The MDS Coordinator/RN is responsible for the coordination of the MDS/Resident Assessment Instrument and assist in the ongoing education of those preparing the MDS. Responsible for the electronic transmission of the MDS/Resident Assessment data to the State Department of Health. Demonstrates the knowledge to complete the MDS/Resident Assessment Instrument. Facilitates communication between the interdisciplinary team. Assist in the assessment of the resident, in the planning of care, and in the evaluation of the plan of care, according to the policies and procedures while maintaining standards of the State Nurse Practice Act. Assists in screening for Skilled Nursing Facility admits.
All the criteria-based duties and standards within this document will be performed according to policies, procedures and guidelines of the Medical Center.
What you'll be doing:
Schedule and complete the MDS and RAP's according to the State and Federal guidelines.
Input data into the computer system.
Assist and train other members of the interdisciplinary team to complete their portion of the MDS.
Complete resident interviews as needed for completion of the MDS.
Transmit MDS data to the State Department of Health.
Aims testing
Does Dr rounds, orders, consults,
Participate on Fall committee and Psychosocial committee,
Participate in the interdisciplinary care conferences.
Does the initial nursing care plan and facilitates its implementation.
Screen for Nursing Facility admits. .
Utilize the nursing process to develop and implement a comprehensive plan of care for the patient/family which promotes optimal patient outcomes.
Meet the educational needs of patient/family/co-workers utilizing principals of teaching/learning.
Delegate nursing care based on knowledge of staff qualifications/competency and patient care.
