Oversees scheduling of the Patient Services Care Coordinators, including approving time off requests. Adjusts staff scheduling to ensure adequate coverage.
Reviews, verifies accuracy, approves, and processes timecards of assigned Patient Services Care Coordinators staff.
Ensures Patient Services Care Coordinators are trained, knowledgeable in, and following all current downtime tasks/procedures.
Supports the training of staff in the latest policies and procedures. Ensures the implementation of updates provided by the management team.
Aids in planning department meetings and events.
Fosters a culture of collaboration and shared purpose by promoting leadership values throughout the team.
Partners with senior leadership, ensuring alignment of departmental goals with organizational vision and priorities.
Performance Evaluation:
Utilizes the phone system to screen and monitor calls, providing the Director with feedback.
Monitors the performance of Patient Services Care Coordinators and provides feedback and updates to the Director as needed, no less than monthly. Includes individual performance summaries, challenges each team member is facing, and any struggles they have encountered.
Pulls performance reports and visits satellite locations rotating on a weekly basis.
Attends/is present during coaching conversations, written warnings, and performance reviews.
Acknowledges complaints, investigates issues, determines resolution, communicates outcomes, and follows up to ensure problems are resolved.
Monitors and evaluates the performance of Patient Services Care Coordinators to ensure that individual employee goals are aligned with overall organizational objectives, fostering a sense of purpose and direction.
Supervisory Responsibility:
In collaboration with the Director of Surgical Services, this position participates in the supervisory tasks delineated in the Job Summary section above.

Native American Community Clinic's Public Health Dosing Program is seeking a Chemical Health Care Coordinator to join our team. This position is responsible for coordinating key activities including patient outreach and care coordination using the Screening, Brief Intervention, Referral to Treatment Model (SBIRT Model) and supporting clients in accessing key resources to enable recovery.
The public health dosing program uses an integrated, harm reduction approach to providing care to patients with Opioid Use Disorder. The program is culturally specific but does welcome all patients.
The ideal candidate will be committed to providing high quality, culturally responsive services in a fast-paced environment. This position is an excellent opportunity to promote recovery within the Native American community and beyond. We are seeking an LADC, UMICAD, or Bachelor level social worker for this position.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
* Meet face-to-face with clients to complete Screening, Brief Intervention, and Referrals to Treatment (SBIRT; SAMHSA evidence-based practice)
* Work as a member of a multidisciplinary team and collaborate to improve service delivery and streamline referrals
* Provide care coordination as recommended by the provider using harm reduction, trauma informed, and strengths-based approaches to empower clients in their recovery and meet their health care goals
* Provide education and awareness materials on MAT
* Promote MAT and peer recovery services
* Provide outreach to those who are hospitalized or using the emergency department for substance use related conditions
* Assist clients in crisis with access to timely assessment, intervention, and stabilization services
* Collaborate with Patient Advocates and Patient Navigators to support clients in the process of obtaining housing, employment, and education
* Assist with grant deliverables including GPRA interviews and follow up
* Other duties as assigned
SUPERVISORY RESPONSIBILITIES:
This job has no supervisory responsibilities.
QUALIFICATIONS:
* Certificates, licenses and registrations required or equivalent experience:
* Bachelor level social work, or
* LADC, or
* UMICAD I, II or III
* Experience working with American Indian Community Strongly preferred
* At least some experience working in substance use field required
