We're looking for an Insurance & Intake Coordinator -- a "Infusion Guide" -- who is passionate about raising the bar in infusion therapy care.
The Infusion Guide will contribute to the company's growth through detailed referral and intake coordination along with excellent customer and physician customer service. The Infusion Guide will make sure our patients receive the highest level of care possible in infusion therapy.
This is a Full-Time position that will be located onsite whenever patients are scheduled. This role will be based in Roanoke, VA. The ideal candidate will have prior experience reading medical charts and working on insurance pre-authorizations. The Infusion Guide role is perfect for those wishing to remain at the forefront of healthcare innovation, while having predictable hours, autonomy, and a manageable workload.
In this role, you will:
Handle all aspects of the infusion referral process; obtaining authorizations, financial counseling, benefit investigation
Liaison with referring providers to obtain documentation necessary for treatment, and communicate status of their patients
Communicate referral process and status to patients in a customer-centric way
Openly communicate with referring/prescribing providers
Assist in maintaining medical inventory and office supplies

Willing and available to work modified schedules (weekends, holidays, etc.) as required. Ability to drive and must possess a valid driver's license. Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.

Hours: full-time (37.5 hours per week), Monday through Friday
$5000 Hiring Bonus for QMHP, active as of 08/06/2025: $5000 Total, paid in three (3) installments; $2000 upon receipt of the following: Background clearances, including fingerprint background check, Acceptable drug screen results, Satisfactory references; $1500 upon satisfactory completion of the six (6) month probationary period; $1500 upon completion of twelve (12) months of continuous employment, with a satisfactory performance evaluation. This hiring bonus is applicable to new hires only, or otherwise, individuals not having been employed for a minimum of 1 year from the date the application is received.
Position Summary
The CareCoordinator, QMHP (Qualified Mental Health Professional) will serve as carecoordinator in the school system, engaging all students, to meet the mental health needs within the school system. The CareCoordinator will engage, conduct initial screening, provide supportive level counseling and/or education, and refer students to a higher level of care. This position will be assigned to Bedford County public schools.
Required Education and Experience
Bachelor's degree from an accredited college/university in an applicable human services field (including social work, counseling, behavioral science, psychology, or other DMAS-approved human services and related fields degrees).
One year of relevant clinical experience.

Related Knowledge, Skills and Abilities
Knowledge of psychotherapeutic interventions with education and mental health or substance abuse treatment. Demonstrate screening and assessment skills to determine need for higher level of care. Ability to engage with youth, school administration officials, and teachers in professional manner. Ability to articulate screening and assessment outcomes to parents, students, and administrators, and articulate next plan for client care.
Willing and available to work modified schedules (weekends, holidays, etc.) as required. Ability to drive and must possess a valid driver's license. May be called upon to travel to any Horizon worksite; transport clients, and provide services to clients in other locations. Ability to participate in and successfully pass behavioral intervention training; includes both verbal de-escalation skills and physical skills techniques to be utilized with consumers as needed.
