    The MAT Clinical Care Coordinator is responsible for the coordination and care of patients receiving Medication Assisted Therapy (MAT) for opioid addiction. Teaming with a Registered Nurse, the position provides health home services to a panel of over 100 MAT patients in coordination with prescribing buprenorphine providers and Behavioral Health clinicians at CHCB. This position will also coordinate with other treatment providers from whom patients are receiving services. This position will oversee and support the housing advocates, and maintain confidentiality in all survivor matters, direct interaction, written information, discussion etc. They will share in the responsibility of backup staffing of our 24 hour hotline and services during assigned times. They will facilitate and coordinate support groups and healing activities for survivors as well as a weekly meal at Laurie's house. The Shelter Coordinator will provide services that are trauma informed, holistic, and equitable to survivors and children impacted by domestic/sexual violence. Please view our job description: Shelter Coordinator Requirements Bachelor's Degree in social or human services or combination of education and experience from which comparable skills and knowledge are acquired; Supervisory experience preferred.
