OASIS and Home Care Home Base experience required. * $118,000-$120,000 Salary Responsible for promoting the development of critical thinking skills and clinical competence by executing the new hire orientation process for field clinicians. Also responsible for providing a blended experience of job shadowing and hands-on field visit training, as well as guidance on industry regulatory requirements, documentation compliance and patient quality. Responsibilities * Coordinates and directs the provision of homecare according to individualized patient plan of care and company policies and procedures. * Demonstrates knowledge of home care and competency in discipline specific patient care skills required by the care center for the provision of patient care. * Orients new clinicians (mentees) to the care center(s), including clinical home care, policies and procedures, documentation, technology, compliance, and regulatory requirements. * Facilitates the learning experience to allow mentees to perform competently in their required responsibilities by mentoring, coaching, and role modeling correct behaviors. * Ensures new hires follow and complete the Welcome Home orientation program in order and in the appropriate time frame * Plans specific learning experiences to promote the acquisition of new knowledge, skills and abilities required of the job. * Provides timely feedback to the mentee regarding all aspects of the orientation experience. * Provides clinical skills practice opportunities for mentees and evaluates and documents correct performance. * Evaluates and makes recommendations as to when the mentee is capable of working independently. * Advocates for additional individual training needs for the mentee, as identified. * Utilizes a combination of care center resources and recognized standards of practice to achieve positive clinical and training outcomes. * Initiates, plans, and provides clinician staff education as needed. * Utilizes accepted company teaching materials such as clinician and patient/family education. * Trains and/or provides reinforcement of training to clinical staff on new patient EMR software applications as needed. * Completes documentation timely, accurately, and at the point of care and according to industry standards, conventions, and guidelines, such as OASIS and HIS, SOC/Admissions, Recertifications, ABNs, subsequent visits, etc. * Facilitates delivery of clinician training and education related to organizational initiatives and/or changes in processes impacting patient care delivery. * Adheres to organizational guidelines for case load management and mentee training. * Performs other duties as assigned. Qualifications Required- * Current unencumbered state license and applicable credentials to practice as a Registered Nurse, * Minimum six (6) months of professional field clinician experience with Amedisys. * Proficient in Amedisys EMR documentation system and policies and procedures. * Demonstrated completion of peer mentor training. * Strong communication skills both verbal and written * Demonstrated ability to guide others through practical experiences, offering direct instruction and providing immediate specific feedback * Ability to provide constructive feedback and create an environment of open dialogue and collaboration Amedisys is an equal opportunity employer.
