Responsibilities * Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care. * Completes OASIS assessments timely and accurately. * Provides skilled nursing care to patients in accordance with the plan of care and agency policies. * Helps to achieve and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources. * Initiates appropriate referrals when indicated. * Promotes and maintains patient health through teaching and appropriate rehabilitative measures. * Assists patients in learning appropriate self-care techniques. * Other duties as assigned. Qualifications * Current RN license, specific to the state(s) you are assigned to work. * One year of clinical experience as an RN. If less than one year clinical experience, you must be approved by VP of clinical. * Current CPR certification. * Valid driver's license, reliable transportation and liability insurance. WHAT YOU'LL DO As a home care registered nurse (RN), you will ensure hospice patients are comfortable and forge compassionate, meaningful connections with the people in their lives. WHERE YOU'LL WORK Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day. Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.
