Day (United States of America) Job Summary: Core Responsibilities and Essential Functions: Program Administration * In collaboration with the trauma Medical Director, is responsible for all aspects of preparation of trauma program for (re)verification by the American College of Surgeons and/or (re)designation by the State of Georgia Department of Public Health: Office of EMS and Trauma * Manages the Trauma Service Staff including : Trauma Outreach Coordinator, Trauma Nurse Coordinator, Trauma Registrar/Data Abstractor(s), Performance Improvement Coordinator * Works cooperatively with the Hospital Administration and the WMG to ensure a smooth functioning and financially viable service * Plans and facilitates Trauma Committee meetings and activities * In collaboration with the Trauma Medical Director and Trauma Committee, sets goals for improving quality metrics * Responsible for creation, development and monitoring of budget * Coordinates the development and deployment of trauma specific protocols and guidelines and monitors adherence and effectiveness of the protocols. * Responsible for the creation of policies, procedures and guidelines for care of the injured patient * Promotes and contributes to the positive working environment within Trauma Services * Completes requisite reports to the State Office of EMS and Trauma and The Georgia Trauma Commission per contractual obligation. * Serves as liaison to Regional, State and National trauma organizations to improve access to and delivery of trauma care * Serves as liaison to all clinical and ancillary departments within the hospital to ensure world class trauma care delivery * Serves as subject matter expert and liaison with the WellStar Health System regarding trauma care delivery * Interfaces with Quality Department regarding quality initiatives and Joint Commission requirements Clinical Activities: * Attends Trauma Activations to serve as a resource person to enhance trauma team performance * Works collaboratively with the Trauma Team to assure coverage for time sensitive responsibilities/duties * Serves as a resource to patients and families as needed to ensure optimal trauma care provision to our patients * Attends Multidisciplinary Rounds with Trauma Team * Rounds on trauma units to ensure resources are available to provide world class trauma care Trauma Registry: * Manages Trauma Registrar/Data Abstractor(s) * Oversees the timely completion, accuracy, and reporting of the Trauma Registry * Works on behalf of WellStar Kennestone Hospital with state organizations toward the development of a Sate Trauma Registry * Maintains the highest level of trauma registry validity to assure accurate program profiling, TQIP, State and internal reports using the established data validation process for the Trauma Registry Performance Improvement: * Manages Trauma Nurse Coordinator(s) and Perfromance Improvement Coordinator(s) * Coordinates and leads Trauma Committee meetings and prepares monthly statistical reports for committee * Evaluates quality assurance filters as defined by the American College of Surgeons, State designated standards and internal filters, for process and care improvement opportunities * Ensures improved pre-hospital care of injured patients through coordination of efforts with local EMS, EMS Coordinator and state Trauma Centers * Monitors TQIP reports for opportunities for improving trauma care within the Trauma Center, Region and State * Works in conjunction with the Trauma Nurse Coordinator to review, monitor and identify PI opportunities at the system and provider levels. Escalates cases as appropriate for peer review. Job Summary:
The Clinical Nurse Professional Development Educator (NPD) works collaboratively to assess, identify, plan, implement, coordinate, facilitate, and evaluate educational activities through a systems model. This includes the tools, theories, skills, and knowledge of the specialty to improve healthcare practice of learners based on the Association of Nurse Professional Development (ANPD) Practice Model. Using expertise in education and professional development practices, the Clinical NPD collaborates with staff, leadership, faculty and researchers to implement and evaluate professional development programs that influence professional role competence, continuous learning and growth, promotes synergistic practices and patient outcomes throughout the organization, and advances the NPD practice based on Wellstar's Nursing Professional Practice Model and in accordance with Association Nursing Professional Development Scope and Standards of Professional Nursing Practice. Roles include learning facilitator, change agent, mentor, leader, champion for scientific inquiry, advocate for Nurse Professional Development specialty and partner for practice transitions.
The Clinical Nursing Professional Development Educator supports the Clinical Nursing Professional Development Educator Specialist, clinical manager, and interprofessional team to implement, document, and modify evidence-based, quality, and educational programs and activities that promote professional competency and role performance for nursing and non-licensed staff. The NPD Practitioner also supports patient care by being a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. They practice in a clinical environment in a functional role.

Job Summary:The Director of Pharmacy shall provide oversight, direction, strategic planning and coordination for the operational, clinical, financial and related services provided by the assigned WellStar hospital. Job Summary:
The Director of Care Coordination will be based at Wellstar Columbia County Medical Center in Grovetown, Georgia and will report to the Chief Medical Officer. The Director of Case Management is responsible for the day-to-day operations for all case management activities across the hospital, including utilization management, safe transitions planning, financial performance related to payer authorization processes, avoidable day tracking and reporting, medical necessity, ongoing quality, productivity and overall efficiency for case management processes. This director will ensure that the system standardize care coordination processes are implemented and followed always. The RN Clinical Nurse is a proactive member of an interdisciplinary team of licensed and unlicensed care givers who ensure that patients, families and significant others receive individualized high quality, safe patient care. They practice in a clinical environment that is administered by Nurse Managers and other leaders and is supported through the Wellstar Shared Governance Model. The framework for practice is steered by the Wellstar Professional Practice Model and evidence-based practice and research. RN competencies are derived from these models and supported by the Wellstar Values.
It is expected that all RN Clinical Nurses are licensed, knowledgeable and uphold the practice of nursing as outlined by the Georgia Professional Nurse Practice Act and implements the Scope of Practice and Code of Ethics Standards put forth by the American Nurses Association. A. C. This leader will oversee the Emergency Department at Wellstar Columbia County Medical Center.
The Director of Nursing is a registered professional nurse who provides leadership and management of a specific service line through the application of advanced competencies in communication; job knowledge, professionalism and business skills. This leader is a member of the chief nursing officer's administrative team and may serve as her/his designee for assigned responsibilities. He/she reports to the nursing operations leader and demonstrates own leadership to effectively partner with nursing leadership, hospital vice presidents, medical staff and other departments to create a center of excellence for their specialty service and to meet or exceed the hospital's mission, vision and goals. The Director of Nursing provides direct leadership for a group of nurse managers and others who comprise their specialty service line. The role
The RN Case Manager provides intermittent skilled nursing services; communicates the patient's progress with other disciplines and directs, supervises and instructs hospice aide staff in the provision of personal care to the patient. As a RN Case Manager you will:
Key Responsibilities:
Under the physician's order, admit patients eligible for hospice services
Assess and evaluates patient needs/problems, identifies mutually agreed upon goals with patients
Report patient status and need for other disciplines to clinical leadership, attending physician and hospice physician
Update care plans on an ongoing basis; revise and resolve patient problems and goals as changes occur and/or recertification
Complete informational visit and obtain patient consents for hospice admission per office procedure
Be responsible to ensure the use of the 4Ms (What Matters to the patient, Medications, Mentation, and Mobility) and provide Age-Friendly Care
Skills and Experience Required:
Current unencumbered registered nurse in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
Must maintain a valid driver's license and good driving record
Ability to work in a field setting and exhibited ability to make sound nursing judgments
