The Senior Front Office Supervisor is a leader who oversees Front Office Operations across a defined region of partner groups, while acting as a liaison between the Front Office Coordinators (FOC) and the Director of Front Office Operations Leadership team. The SFOS works alongside Clinic Operations including the Regional and Group Directors. The objective of this role is to provide the highest level of service to patients, employees, and referral sources through the coordination and administration of front office activities. The SFOS will lead FOCs of partner groups across the Alliance footprint. They will train and implement strategy and processes utilizing technology and manage human capital to ensure Alliance PT Partners is successful in this mission.
The role will oversee, manage and be responsible for the front office operations of multiple clinics within their partner group and its subsidiary entities. The Senior Front Office Supervisor (SFOS) will travel between clinics within their partner group supporting current Front Office Coordinators (FOC) with Alliance Physical Therapy Partners (APTP) front office policies and procedures to enhance administrative efficiencies. Utilizing technology will be central to ensure Alliance PTP is successful in providing exceptional patient experience that is aligned with the company's mission, vision and values.
The SFOS will partner with the Director of Front Office Operations (DFOO) and Alliance's clinical leaders to successfully manage the growth and profitability of the business. The SFOS will support an effective commercial company culture rooted in Alliance's core values of People First, Integrity, Compassion and Relationship
Essential Duties and Responsibilities:
Work effectively and collaboratively with DFOO, RD/GDs, and clinicians to execute comprehensive strategic plans for operating in a growth environment.
Provides administrative support in accordance with established practice standards and departmental policies.
Train new FOC hires and schedule front office coverage when the need arises.
Assess and evaluate front office efficiency and workflows, indicating areas of improvement recommendations.
Conducts on-site interviews for FOCs in partnership with the local Clinic Director.
Conducts routine assessments and audits.
Serves as the lead trainer who will onboard and train all new FOC and is responsible for setting them up for success.
Implements standardization and ensures adherence to front office policies and procedures by all front office team members.
Acts as a liaison between the FOCs, DFOO and CD providing support to all FOCs.
Observes, supports, assists and manages current FOCs with front office policies and procedures including but not limited to implementation of coaching, counseling, corrective action and annual performance reviews.
Works collaboratively with Font Office Leadership and Clinic Director to provide FOC performance feedback, coaching opportunities, and corrective action planning.
Serves as the liaison between the clinic staff and patients and is responsible for effectively and professionally communicating company policies, procedures, and insurance information.
Follows all Compliance, Medicare, and HIPAA policies.
Answers phones, directs calls to appropriate individuals, and prepares messages.
Copies, sorts, and files records related to office activities, business transactions, and other matters.
Maintains filing systems either manually or electronically.
Employee must be in good standing and not currently or within the past 24 months been on a formal disciplinary action.
Qualifications/Skills:
High school diploma or equivalent required.
At least three years of administrative and clerical experience preferred.
Ability to communicate effectively and professionally.
Ability to handle multiple tasks in a very busy environment.
Demonstrates continued interest in self-development and the development of the front office teams.
Excellent interpersonal and customer service skills.
Excellent organizational skills and attention to detail.

Position Summary:
The Therapist I provides individual, group and group therapy sessions for clients in an Intensive Outpatient and Partial Hospitalization mental health facility. Therapists assess the therapeutic needs of clients, develop appropriate treatment plans, and facilitate group, family and individual therapy sessions. Facilitates specialized groups as assigned based on education, training, and experience; supervises and/or co-leads groups with clinical interns, when appropriate.
Selects appropriate interventions to meet the clinical needs of individual clients.
Maintains therapeutic relationships with clients using praise and other reinforcers to encourage appropriate behaviors and attitudes.
Sets limits and holds boundaries in a kind and firm manner.
Conducts effective verbal crisis intervention and de-escalation involving other team members, as needed.
Collaborates with an interdisciplinary treatment team to coordinate client care from admission through discharge.
Attends clinical, consultation, and operations meetings; serves as a clinical advocate for assigned cases.
Communicates client progress with the referral sources, families, parents and/or guardians, as appropriate.
Addresses clients, visitors, physicians, and co-workers in a welcoming and respectful manner.

What We Are Seeking:
Position requires a master's degree in social work, marriage and family therapy, counseling or psychology. Must hold a (LPC, LMFT, LCSW, LP, LLPC, or LLP) , in good standing, within the state of Michigan. Requires minimum of 2 years' of experience working with clients with a primary mental health diagnosis.
Other Requirements:
Requires clearance of TB test, criminal background check clearance, and any other mandatory state/federal requirements.
Current CPR and First Aid certification or willingness to complete within 30 days of start date.

Ensure the timely completion of quality control activities and daily poundage reports.
Assist with tours of the food bank to volunteers and to other visitors.
Follow all setup and clean up procedures correctly with special attention to putting away items in their proper place.
Coordinate with other reclamation staff so there is one person in the department at all times.
Renew the Safe Serve regulations receiving a score of 90% or higher every two years.
Comply with all personal policies and procedures.
Perform all other duties as assigned by the Reclamation Coordinator.

TheField Care Coordinator- HIDE SNPis an essential element of an Integrated Care Model and is responsible for establishing a set of person-centered goal-oriented, culturally relevant, and logical steps to ensure that the person receiving LTSS receives services in a supportive, effective, efficient, timely and cost-effective manner. This position is Field Based with a Home-Based office. The expected travel time for member home visits is typically 75% within a 50-mile radius and/or 50-minute drive from your home pending business needs.

Primary Responsibilities:
Develop and implement care plan interventions throughout the continuum of care as a single point of contact
Communicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all members
Advocate for persons and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team
Assess, plan, and implement care strategies that are individualized by the individual and directed toward the most appropriate, least restrictive level of care
Identifies problems/barriers to care and provide appropriate care management interventions
Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services
Provides resource support to members for local resources for services (e.g., Children with Special Health Care Services (CSHCS), employment, housing, independent living, foster care) based on service assessment and plans, as appropriate
Manage the person-centered service/support plan throughout the continuum of care
Conduct home visits in coordination with the person and care team
Conduct in-person visits, which may include nursing homes, assisted living, hospital or home

Conducts patient visits with clinicians to monitor for compliance with the Five Star Visit model in addition to compliance with clinical standards of practice, compliance to policies and procedures, and compliance to regulatory requirements.
Conducts patient chart audits to assess clinician's ability to document timely, accurately and thoroughly, focusing and documenting patient decline related to terminal diagnosis.
Provides immediate performance feedback to the clinician following a clinical visit. Communicating results of the audit. Identifying both positive practices as well as areas of opportunity.
Collaborates with nursing leaders, providing results of auditing activities and coordinating a plan for performance improvement.
Collaborates with Institute staff to meet the educational needs of clinical staff when deficiencies are identified during chart audits and patient visits.
Acts as a teacher and mentor to newly hired staff as well as current staff to poise the staff member for success.
Reviews various sources of quality data with quality and compliance leadership to identify areas of opportunity to improve clinical practice in the organization.
Documents all audits into the NSCC quality monitoring platform.
Collaborates with clinical leadership to continuously improve clinical practice and achieve high quality consistent patient outcomes.
Proactively participates in quality assurance processes/programs.
Attends IDT meetings to ensure discussion is focused on updating plans of care, documentation is complete and includes objective and evidence of ongoing decline.
Participates in survey preparedness activities as needed.
Maintains a positive working relationship with internal and external customers.
Periodically attends scheduled team RNCM meetings to foster education, priorities, provide support.
Utilizes allocated supplies and resources appropriately to ensure business outcomes are met.
Adjusts to changes in workload and schedules based on changing team priorities.
Actively participates in internal and external education, training, in-services, and other activities to promote personal and professional growth.
Adheres to organizational code of conduct, policies and procedures and all regulatory and legal requirements.

Qualifications:
Bachelor of Science in Nursing preferred. Registered Nurse (RN) licensure in the State of Michigan required.
Three (3) to five (5) years of demonstrated experience in Hospice RN case management required.
Advanced knowledge and clinical expertise in hospice and palliative care required.
Demonstrated strong understanding of regulatory/compliance requirements and performance improvement processes.
Certification in hospice and palliative nursing (CHPN) preferred.
Demonstrates characteristics, skills and enthusiasm for teaching and mentoring staff.
Demonstrated subject matter expert for electronic medical record.
Demonstrated ability to organize, prioritize and manage teaching, learning, training, and human performance technology projects required.
Demonstrated willingness to add to own professional growth.
Must be able to read, write and speak English fluently and be able to effectively communicate orally and in writing in internal and external relationships for all essential job functions.
Must have demonstrated ability to work cooperatively with and collaborate with others to achieve project outcomes.
Ability to effectively use technology in support of management and clinical operations.
Must possess sound judgment; effective organizational, prioritization and follow-through skills;attention to detail; accuracy; dependability; tact; and ability to maintain confidential information.
The physical demands of the position include vision, effective speech, and hearing for extensive telephone contact; repetitive motion; traveling; driving or riding in motor vehicle; standing, sitting, walking, bending, reaching, and stretching; lifting up to twenty-five (25) pounds unassisted.
Must have reliable transportation and willingness to travel and maintain the rigors of a busy schedule.
Frequently works variable hours/days; activities and workload may require extended days.
