Provides personal care and related services in the home/ALF/Nursing facility to Hospice patients. Functions under direct instruction and supervision of an RN.
Position Specific Responsibilities
• Under the direction and ongoing supervision of the registered nurse, follows written instructions for patient care as prepared by an RN.
Assists patients with personal care (which includes lifting and transferring patients without assistance) to preserve good personal hygiene and provides range of motion while maintaining a healthful, safe environment, while maintaining patients' privacy and dignity.
Reports changes in patient's condition and needs to Supervisor or RN.
Maintains record of services performed and of apparent condition of patient.
Establishes and maintains a good working relationship with the patient/family and co-workers of the Agency.
Participates as an interdisciplinary team member to provide input requiring continuum of patient care.
Establishes and maintains a good working relationship with the patient/family and co-workers of the network.
Performs other duties as assigned.
Additional Responsibilities:
Possesses adequate knowledge of the Medicare COPs and state regulations to ensure quality of patient care and regulatory compliance.
Participates in on-call coverage to ensure patient care.
Conforms to all applicable Agency policies and procedures.
Participates actively in continuing education and in-services.
Maintains confidentiality of patient information and business trade practices.
Assumes accountability for reporting incidents and complaints according to Agency policy.
Maintains a committed and cooperative attitude with staffing, promoting teamwork, effectiveness and efficiency.

Education and/or Experience
WA/AK: Required - Completion of State-approved CNA certification training program
TX: Preferred - High school diploma or GED. A combination of education and equivalent experience will be considered.
WA/AK: Preferred - 1 year home health/hospice experience as a nurse's aide, hospice aide, orderly, or related experience in an acute, rehabilitative, or long-term care facility, or hospice organization or private home care aide.
TX: Preferred - One to three months of related experience and/or training.
Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.

Hospice Aide Requirements
* CertifiedNursingAssistant highly preferred.
* In states where licensure is not required (AL, AZ, CT, GA, IA, IL, KS, LA, MA, ME, MI, MS, MT, NM, OH, OK, PA, SC, TN, TX, WV), applicants must meet one of the following:
* They are already a licensed/certified aide; or
* They have completed a state-approved nurse aide training program AND have been continuously employed in that capacity for the previous 24 months
* Current CPR certification required.

* State certification as a home health aide.
Colorado
* Completed state training program AND certified via DORA.
Indiana
* Aides required to be registered on the Indiana Nurse Aide Registry, licensure is not required.
Missouri
* Licensed/certified as a Nurses' Aide; or
* Have completed a nurses' aide training program AND employed as a nurses' aide for the past 24 months if not licensed/certified; or
* Enrolled in a professional or practical nursing program for at least four (4) months, that included a clinical rotation within the past five (5) years and has successfully passed a Fundamentals of Nursing course (must provide transcript and letter from the program director on school letterhead verifying BOTH clinical rotation and successfully completion of Fundamentals of Nursing course).

ASSIGNED JOB RESPONSIBILITIES
Performs only those
personal care
activities contained in a written assignment. Home Health Aide (HHA) assignments may include:
Home Health Aide (HHA) helps the patient to maintain good personal hygiene.
Home Health Aide (HHA) plans and prepare meals. Feeds or assist with feeding the patient when needed.
Home Health Aide (HHA) assists the patient ambulation.
Home Health Aide (HHA) assists with certain treatments as ordered by the physician, and taught and supervised by the nurse for a specific patient including: assist with the change of the colostomy bag; assist with the use of a walker or wheelchair; reinforcement of a dressing; assist with prescribed range of motion exercises; assist with prescribed ice cap or collar; measure intake and output of fluids; and assist with medications that are ordinarily self-administered.
Home Health Aide (HHA) reads and records temperature, pulse and respiration.
OTHER RESPONSIBILITIES FOR THE HOME HEALTH AIDE (HHA) NOT INCLUDED IN A WRITTEN ASSIGNMENT MAY INCLUDE:
Home Health Aide (HHA) assists in maintaining a safe and healthy environment.
Home Health Aide (HHA) promotes the patient's mental alertness through involvement in activities of interest.
Home Health Aide (HHA) provides simple emotional and psychological support to the patient, family, and other caregivers.
Home Health Aide (HHA) reports any changes in the patient's mental or physical condition or home situation to the VITAS Nurse and members of the interdisciplinary team.
Home Health Aide (HHA) teaches basic personal care to the primary caregiver.
Home Health Aide (HHA) practices basic infection control measures.
Home Health Aide (HHA) observes, reports, and documents patient status and the care and activities provided.
Home Health Aide (HHA) completes at least 12 hours of in-service training per calendar year.
Home Health Aide (HHA) handles other tasks as assigned.
THE HOME HEALTH AIDE (HHA) MAY NOT:
Be assigned by VITAS to any institutional setting.
Change sterile dressings.
Irrigate body cavities (such as an enema).
Irrigate a colostomy or wound.
Perform a gastric lavage or gavage.
Catheterize a patient.
Administer medications.
Apply heat by any method.
Care for a tracheotomy tube.
Suction a patient.
Provide any personal care that has not been assigned by the VITAS nurse or included in the plan of care.
QUALIFICATIONS
High School graduate or equivalent preferred.
Meets personnel qualifications of a Home Health Aide as specified in 42 C.F.R. Section 484.4 and Section 484.36.
Documented evidence of 75 hours of training as specified in 42 C.F.R. Sections 484.36 (a)(1).
Documented evidence that Home Health Aide has successfully completed a Competency Evaluation program as described in 42 C.F.R. Section 484.36(b).
