Angels of Care Pediatric Home Health is a clinician owned and operated home health agency with experienced and knowledgeable staff serving the special needs community. Pay Range: $57,200 - $114,000
Job Description: A certified OccupationalTherapist (OT) will implement treatment programs to assist pediatric patients with physical, cognitive, sensory and social/emotional disabilities or delays by planning and administering occupational therapy in the home and community. Requirements:
Texas State OT license
Current CPR certification
A minimum of 1 yr. of experience preferred
Responsibilities:
Provides high quality care and meets the needs of the patient and family by performing evaluations and interpreting assessment results; developing goals appropriate for child; creating and implementing occupational therapy treatment plans in conjunction with the physician.
Assists pediatric and young adult patients in the areas of physical, neurological, cognitive, sensory, feeding, ADL's, and social/emotional development and improves their level of independence by implementing skilled interventions to maximize the potential of each individual child.
Participates in educating, coaching, and empowering caregivers and families to develop skills to carry over therapeutic activities into the child's daily routine.
Coordinates with referral partners to provide services for children in accordance with the physician order.
Assures continuation of therapeutic plan following discharge by designing and instructing patients, families, and caregivers in home exercise programs.
Documents patient care services and care coordination in an intuitive electronic medical record system.
Maintains patient confidence by keeping information confidential.

Position Specific Responsibilities
Participates in the development of occupational therapy plans of care for patients.

Provides education and instruction to team members, patients, and families.
Evaluates outcomes of care.
Supervises paraprofessionals as needed.

* Participates in agency quality improvement programs when requested.
* Ensures that all care is provided with respect for patient rights.
* Reports all grievances and complaints made by patients or families to the appropriate persons.
* Reports all allegations of patient abuse and/or misappropriation of patient property.
* Follows standard precautions and infection control procedure.

How you'll make an impact as a Home Health Physical Therapist
Work one-on-one with patients when and where they need the most help - in their home!
As a Home Health Physical Therapist, you have the unique opportunity to make a huge difference in your patients' lives and see firsthand how your work impacts their ability to live in their home while gaining the strength and independence they need to participate in their desired daily living activities, such as going to church or working in the garden.
Minimum Qualifications:
Bachelor of Science in Physical Therapy
Current State License as a Physical Therapist
Valid Driver's License
Current CPR, negative TB screen and Hepatitis consent/declination
Two years' experience as a Physical Therapist in an acute care or rehabilitation setting
Knowledge and Skills:
Therapy skills as defined as generally accepted standards of practice
Good interpersonal skills
Knowledge of durable medical equipment
