includes a $5,000 sign-on bonus. Anticipated Pay Range: $75,000 to $85,000
This role includes field days (for staff supervision and oversight of patient care) and office days. Administrative tasks on field days can generally be completed off-site unless office presence is needed for operational reasons.
As a Clinical Care Manager, you will lead a team of dedicated clinicians in delivering exceptional client care. You will manage a case load of clients, partner with an interdisciplinary team, train and onboard new hires, and manage client referrals. Supervise and support direct care staff to ensure compliance with policies and regulations.
Manage a caseload of clients, develop personalized treatment plans, and coordinate care with external providers.

Lead orientation, training, and competency verification for new hires.
Act as a liaison among staff, community, and healthcare providers to ensure seamless communication.
Provide in field supervision and real-time guidance to clinicians.
Participate in quality assurance, performance improvement, and staff development initiatives.
Support on-call operations, ensuring timely follow-up and accurate documentation.

Routine travel within the service area is required for both patient visits and staff supervision. There may also be occasional regional travel for training, etc.

The Bereavement Coordinator/Volunteer Coordinator is a hospice-trained interdisciplinary team member who oversees an organized program to ensure grief counseling for the bereaved. S/he is responsible for developing and delivering a program of bereavement support and counseling to patients during their hospice care and to the bereaved family members of patients for at least one (1) year after the death of the patient. Additionally, s/he directly supervises volunteers. S/he adequately supports volunteer operations and ensures compliance with state and federal regulations. S/he coordinates patient and family needs for volunteer services and oversees the development and implementation of the plan of care to meet those needs.
Position Specific Responsibilities
Bereavement Coordinator Responsibilities:
Participates as a member of the IDT, including development and implementation of the plan of care.
Works with the hospice Social Worker in the completion and analysis of the pre-bereavement risk assessment and care plan development for the patient and the family.
Promotes and supports assessment of bereavement risk factors at admission and throughout the hospice patient's care.
Develops and implements the bereavement plan of care after the death of the patient.
Identifies the needs of non-family caregivers, including staff and residents at nursing facilities where the patient may have resided, and ensures bereavement support for them.
Assists in identifying the need for intervention of other IDT members.
Effectively communicates patient and family needs to IDT.
Provides education about bereavement services to members of the IDT.
Recognizes issues or concerns and escalates appropriately.
Prepares reports, in-services, and other documents to demonstrate the value and effectiveness of the bereavement program as requested.
Delivers high quality bereavement support, either with internal resources or through outreach to community resources when needed.
Develops, organizes and delivers grief support groups either independently or in association with other grief support resources in the community.
Maintains a resource library of materials appropriate to various bereaved individuals, including those with high-risk or disenfranchised grief - provides materials as part of routine bereavement contacts and upon request.
Documents care and services provided according to state and/or federal requirements.
Attends and participates in community events, promoting Compassus and the universal need for grief and bereavement support.

Education and/or Experience
High school diploma or GED required.
Bachelor's degree or higher in Counseling, Social Work, Psychology, Theology, Divinity, or equivalent field strongly preferred.
Minimum of two (2) years of experience in the provision of grief, loss and/or bereavement support for individuals and families strongly preferred.
Experience in Social Services, Human Services, Resource Management or related field is helpful.
Prior experience managing volunteer services or non-profit organization is a plus.
Skills

Must be listed on the state aide registry with a clear record and eligible to provide care in homes, nursing facilities, or hospitals.
Requires a reliable vehicle, valid driver's license, and state-required auto insurance.
Responsibilities:
Provide compassionate, hands-on care to hospice patients, including assistance with personal hygiene, repositioning, and mobility support for individuals who may require lifting (up to 200 lbs with assistance).

Ensures proper notification of admission to assigned interdisciplinary team (IDT) and ancillary providers.
Enters admission data into a third-party system.
Ensures patient chart, ensuring all documentation is signed and dated and turns chart over to Team Coordinator.
Markets referral sources.
Contacts long term care staff, hospital staff, and physicians to promote hospice services.
Provides back-up support for Hospice Care Consultant.
Documents marketing contacts, as directed.
Performs other duties as assigned.
Education and/or Experience
Associate or Bachelor's degree in Nursing strongly preferred, LPN considered.
Two (2) years of experience as a registered nurse in a clinical care setting or home health or hospice preferred.
Experience communicating with physicians, nurses and other healthcare personnel highly preferred.
Experience in insurance and Medicare/Medicaid billing and reimbursement preferred
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.

This role focuses on growing hospice admissions by building strong relationships with healthcare providers, identifying eligible patients, and helping to ensure they are admitted quickly and appropriately. These goals are met by identifying and opening new accounts, and by growing share within existing accounts. Success in this role requires more than strong sales abilities. • Drive significant growth within the assigned book of business by opening new accounts and expanding existing accounts. Leverage Compassus-provided tools to identify new opportunities through detailed market analysis
• Secure referrals from physicians, staff, and administrators and partner with the operations team to ensure all clinically-eligible patients are rapidly admitted
• Take full ownership of meeting or exceeding weekly performance goals. Grow productivity levels on / ahead of pace with tenure-based company expectations
• Cultivate and manage strong, trust-based relationships with referral sources leveraging a personal, needs-based consultative sales approach
• Show genuine compassion and empathy to patients and families. Maintain resilience in the face of the significant emotional challenges inherent to working in hospice care
• Always prioritize the patient. Be ready / willing to work outside standard business hours, including evenings and weekends, to ensure timely support and service for patients in need
• Seek out feedback from referral sources on service levels, quality, patients, etc. and demonstrate rapid, attentive responsiveness to protect the referral source relationship. Relay valuable insights to the operations team and help facilitate change where it is needed
• Demonstrate adaptability and readily embrace change (e.g. business priorities, team structure, and market dynamics) if / when it arises. Be ready to do what's required to best-support the team / business
• Provide clinical training and education to referral partners to help improve understanding of hospice care
Education and/or Experience
Bachelor's degree in Business strongly preferred.
Five (5) plus years of successful sales experience in relevant roles
Proven track record of meeting / exceeding sales goals, with the ability to find and grow new accounts

Focus on the medical needs and treatment of infants, children, and/or adolescents primarily in their home according to the active plan of care, as given by the physician and other appropriate personnel. Maintain positive internal and external customer service relationships.
Conduct on-going patient care and assessments.
Administration of prescribed medications, treatments, and therapies.
Coordination of care.
Educate family members/caregivers on patient clinical care to enhance positive outcomes.
Preventative initiatives to protect quality of care for patient.
Assess patients' post-acute care needs in collaboration with the Clinical Care Managers and the patients discharge care providers to ensure continuity of care when discharging home.
Provide interim care to patients experiencing gaps due to call offs, vacations and staff shortages with varying acuity and scheduling needs.
Provide one-on-one training to nurses in the field to ensure a smooth clinical and non-clinical transition assuring continuity of care when the Halo's Home Team exists the case.
In conjunction with support staff, serve as a liaison and patient advocate on complicated cases.
