The Key Program is seeking a Part-Time Registered Nurse (20-24 hours per week) to work in our DCF residential programs in Springfield, MA. The work schedule would be consistent week to week and will be established based on the program's needs and candidates' preferences. Each residential program provides services for up to nine adolescent age youth.

Overview of the RN Position
The RN is responsible for providing quality assurance by monitoring each residential programs' Medication Administration Program (MAP) system to ensure compliance with the Massachusetts DPH and DCF/DMH. If the candidate is not a MA MAP Approved trainer, this training will be provided to the RN upon hire. This is a highly administrative position but also provides opportunity to work directly with the children on individual needs and interests related to personal health and wellbeing.

What the RN brings:
The Nurse Monitor position requires a Registered Nurse (RN) with two years' RN experience, must be 21 years of age or older, and have a valid driver's license and registered and insured vehicle for work use.
What the RN does:
The RN provides clinical monitoring of the Massachusetts Medication Administration Program (MAP) to address the distinct needs and challenges presented in administering medication to minors by (not all inclusive):
Observing all MAP Certified staff prepare and administer medication in a manner consistent with all MAP policies, procedures, and protocols.
Verify HCP orders are current, properly transcribed, and are being administered accordingly
Verify that medication is properly procured, counted and stored
Reviewing documentation of medication being administered at the site to ensure that proper clinical protocols are in place and are being observed
Ensures submission and follow-up of Medication Occurrence Reports and determine areas where further training or other intervention may be needed
Communicate with State MAP Coordinators regarding findings relative to Technical Assistance reviews/audits performed at the site
Works collaboratively with program supervisory and clinical staff to identify address, individually and collectively, medication administration-related training needs for certified staff
Facilitate MA MAP Certification training and MAP recertification training to staff as well as other required trainings such as vital signs, Epi pen, routes, etc.

Job Summary
The Pharmacy Director functions as the Pharmacist in Charge and is responsible for operational management of the Pharmacy Department including functioning as the primary pharmacist. The primary function of this position is to manage the department in compliance with the philosophy, policies, procedures, goals, and budget of the department along with oversight of any pharmacy department staff. The director incorporates and demonstrates customer service strategies to achieve the organization's goals. Prepares, monitors, and evaluates departmental budgets, and ensures that the department operates in compliance with allocated funding.

OBJECTIVES/PURPOSE
Responsible for all Sample Processing job skills, including, preparing plasma units for freezing, preparing samples of plasma for testing, and preparing plasma units and samples for shipment.
All below listed responsibilities must be completed in compliance with federal, state, local and company-specific regulations related to quality of product, employee and donor safety, and to the proper performance of day-to-day
activities. Employees must also maintain complete and accurate records, in accordance with cGMP.
ACCOUNTABILITIES
Provide exceptional customer service to internal and external customers (5%)
Perform all required duties in the area of Sample Processing (including, but not limited to): (95%)
Prepare units for frozen storage.
Collect and store samples from plasma units for testing.
Perform routine maintenance on the freezer including monitoring freezer and refrigerator temperatures and removing ice buildup.
Prepare frozen plasma units and samples for shipping following established protocol.
Organize plasma boxes/documents for scheduled shipments and assist in loading shipments.
Manage supplies, assist management team with inventory control procedures, break down empty cartons and assist with proper disposal.

Boston Health Care for the Homeless Program (BHCHP) is offering a wonderful opportunity to work as a part of a vibrant multidisciplinary HIV team. As a program recognized nationally for its innovative programming and provision of the highest quality health care for people experiencing homelessness, the Medical Case Manager will work to address key social determinants of health for people living with HIV and substance use disorder who are experiencing homelessness; and do so within a collaborative team-based community health center context. This position is funded until the end of February 2028 with the possibility for renewal. If this funding is not renewed, this position will end February 28, 2028.
Responsibilities:
Assess new HIV patients for financial, psycho-social, housing, and other needs. Develop comprehensive service care plan with the patient; assess Ryan White eligibility and care plan at intake and every 6 months; attend all required Boston Public Health Commission Ryan White trainings (including on eligibility and program updates)
Work with patients to reduce barriers to full engagement in HIV care and treatment, including via collaboration with legal and housing advocates; assist patients in attending important appointments including medical, benefits, mental health, etc., by arranging transportation and addressing other barriers; accompany patients as needed to important housing, legal and social service appointments; as well as to medical and behavioral health appointments when indicated; track patients who miss appointments and reschedule as necessary, document care appropriately in medical record
Identify, in collaboration with rest of the multidisciplinary team, which patients will benefit from outreach visits at shelters, outside agencies, jail, or their place of residence; participate in HIV care team to exchange information, develop an integrated care plan, etc., in partner notification program, and in multidisciplinary HIV care team meetings to collaborate on integrated care plans and coordinate care
Conduct intensive outreach visits on the streets, drop in spaces, outside agencies, jail, or patients' homes to connect with patients who are newly diagnosed with HIV and not yet in care, or who have fallen out of care
Qualifications:
Associate's or bachelor's degree in a related field and/ or equivalent two years relevant human services experience
Experience working with people with substance use disorder is required, experience working with people experiencing homelessness, incarceration and/or sexual violence is a plus; lived experiences are welcomed and valued
Commitment to harm reduction, knowledge of harm reduction strategies and trauma informed approaches is required; previous experiences conducting street level outreach with at-risk populations a plus; knowledge of community resources, particularly addiction related resources and supports a plus
Commitment to equitable and culturally appropriate care for a wide range of diverse populations, including (but not limited to) communities of color, LGBTQIA communities, non-English speaking populations, people with histories of incarceration, people with substance use disorders, and people experiencing homelessness
Strong organizational, interpersonal, written, and verbal communication skills; self-directed, motivated, and flexible with the ability to work independently as well as in a team-based setting
Bi-lingual fluency in Spanish is preferred
Compensation and Benefits:
The compensation increases based on years of experience and ranges from $22.07 - $35.31 hourly.
