JOB PURPOSE:

KEY RESPONSIBILITIES:
1. Proven ability to perform comprehensive, adult/geriatric admission history and physical exam
2. Demonstrates ability to identify, diagnose and document episodic, acute and chronic disease in adults
3. Demonstrates ability to recognize conditions, characteristics, and behaviors that place seniors at risk
4. Proven ability to pursue feedback, continuing education and professional development
5. Ability to effectively reconcile medications
6. Demonstrates ability to identify and document the needs of clinically complex patients
7. Work collaboratively with physicians, case managers, social workers, family members, key care givers and any appropriate ancillary medical personnel as appropriate
8. Participates in
Quality Assurance Performance Improvement
(QAPI) activities
9. Assesses the medical necessity/effectiveness of ancillary services to determine the appropriate initiation of benefit events and communicate the process to providers and appropriate team members
10. Coordinates care as member's transition through different levels of care and care settings, ensures that the ICP is communicated immediately and/or no later than 24 hours to the receiving care setting
MINIMUM EDUCATION REQUIRED:
Graduate of an accredited Adult or Family NursePractitioner Master's Degree program or Physician Assistant Studies
MINIMUM EXPERIENCE REQUIRED:
Minimum 2 years clinical experience as a NursePractitioner or Physician Assistant in geriatric, adult or family practice setting. Proven ability to perform comprehensive adult/geriatric physical exams.
MINIMUM LICENSURE/CERTIFICATION REQUIRED BY LAW:
Current and unrestricted, RN and NP licensure, or PA licensure in state of practice. Board certification by AANP or ANCC, in Adult, Family Practice and Geriatrics or Adult-Gerontology Primary Care. Current CPR certification
• Active DEA license
Must have an active national provider number (NPI) and an active Medicare provider number
Must be eligible to participate in the Medicare program as a provider and registered
ADDITIONAL QUALIFICATIONS: (Preferred qualifications)
Graduate of an accredited Geriatric, Adult or Family NursePractitioner Master's Degree program. Previous long term care experience strongly preferred.
KNOWLEDGE, SKILLS, ABILITIES
1. Pertinent Clinical Work Experience which is relevant to population being served.
2. Reviews and interprets the medical and social history of members
3. Interprets laboratory data, recommend appropriate follow-up, reconcile medications
4. Manages both medical and behavioral chronic and acute conditions effectively in collaboration with a physician or specialty provider
5. Synthesizes information and easily diagnoses and creates a comprehensive plan of care.
6. Complete all regulatory required monthly, quarterly, and/or annual visits and acute visits as needed.
7. Maintain appropriate and compliant documentation within our EMR (electronic medical record) software
8. Ensures all diagnoses are ICD-10 coded accurately and documented appropriately to support the diagnosis at that visit
9. Reviews orders and interventions for appropriateness and response to treatment to identify the most effective plan of care that aligns with the member's needs and wishes
10. Participate in family conferences and provide advanced care planning and education when needed
11. Address any pharmacy recommendations or pharmacist communications regarding residents
12. Providers are expected to be present in assigned facilities during work hours to ensure that facilities have adequate face-to-face coverage to meet resident care and facility needs
13. Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification
14. Rotating on-call responsibilities and after-hour coverage is required.

Your Impact
Using your skills in this position will allow you to deliver personalized compassionate medical care to individuals mainly with CKD and/or ESRD/ESKD. You will also be responsible for caring for patients, maintaining accurate and current patient records and scheduling, and administering follow-up appointments to patients as required. Roles and Responsibilities
* Conducts assessments, which includes comprehensive annual wellness exams on patients both in the patients' home and in the virtual environment
* Counsels and educates patients and families about benefits and programs available to help them live healthier lives
* Documents items such as: appropriate chief complaint, all applicable diagnosis, past medical, family, and social history, review of systems, examinations, medications, allergies, assessment, and plan
* Responsible for the coordination of care with primary care providers, specialists, and appropriate ancillary services
* Completes all documentation and paperwork in a timely manner
* Maintains quality of care standards as defined by the practice
* This position will not be office-based but will be remote in state in which employed and will need to attend periodic training/meetings outside of that state
* Deliver evidence-based, timely care in a manner that reduces avoidable hospitalizations, maximizes quality of life, and puts patient health and satisfaction first
* Prescribe medications, order tests, and collaborate with patient's Monogram physician
* Perform effectively, as reflected by improved patient quality outcomes, which will be measured and reported daily
* Facilitates closing gaps in care by educating patients about preventive monitoring and working with physician practices to schedule diagnostic testing
* Assists patients with enrolling to access educational videos
* Participates in the integrated care team meetings
* Knowledge of disease diagnosis and prevention
* Make assessment of patient's health status
* Develop treatment plan
* Implement a plan consistent with appropriate plan of care
* Follow-up and evaluate patient's status
* Other duties as assigned
Position Requirements
* Active and unrestricted Registered Nurse and NursePractitioner or Physician Assistant license
* Board certified for appropriate licensure (NP: ANCC/AANP; PA: NCCPA)
* Current and unrestricted DEA certificate
* Ability to work without direct supervision and practice autonomously
* Access to transportation, a valid driver's license, and car insurance
* Must be proficient with medical instruments and equipment required by the work
* Knowledge of computer-based data management programs and information systems, as well as medical records and point-of-interview technology
* Ability to communicate effectively in verbal and written form with retail and medical partners at various levels, patients, family members, physicians and representatives of the community
