Position Purpose
Performs the day-to-day activities of housekeeping for Berry Dining Hall and kitchen areas in accordance with current Federal, State and Local standards and will be directed by the Culinary Operations Director II or Culinary Operations Kitchen Manager I to ensure that the facility is maintained in a clean, safe, and comfortable manner.

Major Duties and Responsibilities
Performs day-to-day housekeeping functions and specific tasks as assigned; cleans, washes, polishes, sanitizes, deodorizes and disinfects all areas and items in the facility including furnishings, fixtures, ashtrays, trash bins, windows, mirrors, floors, carpets, walls, ceilings, bathrooms, etc., using proper cleaning and disinfecting solutions and procedures; ensures that work areas are maintained in a clean, safe, comfortable and attractive manner, clearly marked with caution or safety signs as required; ensures that work areas are free of all hazardous objects and that all equipment, tools and supplies are properly stored at all times; ensures that established Universal Precautions and infection control, isolation, fire, safety and sanitation practices and procedures are maintained and followed when performing housekeeping duties; and promptly reports all hazardous conditions and equipment to the Culinary Operations Director II or Culinary Operations Kitchen Manager I.
Maintains resident confidentiality; treats villagers with kindness, dignity, and respect; and knows and complies with the Resident's Rights rules.
Ensures that work and cleaning schedules are followed and reports all incidents and accidents to the Culinary Operations Director II or Culinary Operations Kitchen Manager I.
Ensures that equipment, work, and supply areas are properly cleaned and stored; ensures that an adequate level of housekeeping supplies is always maintained and advises the Culinary Operations Director II or Culinary Operations Kitchen Manager I.
Attends and participates in orientation, training, in-service educational activities, and staff meetings as assigned.

Serves as Data System Administrator and provides oversite of all physical and electronic resident data. This includes serving as a super user of the Electronic Health Record system.

Assists in developing for each resident, in compliance with federal/state guidelines, a preliminary and comprehensive assessment and written care plan; encourages residents and families to participate in the development and review of care plans; and ensures that all staff are aware of the care plan and that they are used in providing daily nursing services to the resident.
Develops and participates in the planning, implementing, conducting, and scheduling orientation, training, and in-service educational activities for nursing service personnel to maintain professional competence as well as current licenses.

Valid Cosmetology License preferred; esthetician or barber license preferred.

Hygiene Care Tracking and Documentation

Share relevant updates with case managers and nursing staff for inclusion in service plans and reviews.
Interdepartmental Collaboration

Promote a culture of pride, confidence, and dignity through personal appearance and care routines,
Fiduciary Responsibilities
Complete and process Green Sheets and related financial documentation for all hygiene and personal care services.
Ensure services such as haircuts, skincare, or podiatry visits are paid accurately and timely through villager trust funds.
Maintain detailed records of expenditures and submit them in accordance with organizational fiscal policies.

Uses counseling methods, assures the best quality of life for the residents, and serves as an intermediary with the clients, families, Annandale staff, and other agencies. Bachelor's degree in social work or related human services field with at least 2 years' experience in a long-term care setting
Experience working with adults with developmental disabilities preferred.
Knowledge of current standards in Long Term Care Social Services, Activities, and MDS 3.0 documentation requirements.
Must be at least 21 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.

Major Duties and Responsibilities
Interviews residents with issues relating to personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of the issue and helps residents resolve.
Secures information such as medical, psychological, and social factors which contribute to resident's situation and evaluates these and resident's capacities.
Maintains working knowledge of Social Security, Medicaid, Medicare, and private insurance requirements and assists family, as needed, to obtain or apply for benefits for their resident.
Reports abuse or suspicion of abuse as required by D. Scott Hudgens Center for Skilled Nursing policy and licensing agencies.
Counsel's residents individually, and in family or small groups, regarding plans for meeting needs.
Facilitates Care Plan scheduling, prepares weekly reports, and follows up on concerns expressed by team members.
Attends quarterly Quality Assurance meetings and prepares report for improvement plan.
Serves as member of Admissions Team.
Works with physicians, agencies, and other organizations to meet the ever-changing needs of residents.
Coordinates with Programs, Skilled Nursing Center, contracted Therapists, and volunteers to provide residents meaningful community inclusion and/or re-integration experiences based on individual goals and interests.
Schedules residents' appointments and coordinates with the C.N.A. Transportation Aide for transport to and from appointments and will serve as a back-up to transport residents to appointments as needed
Develops, facilitates, and/or provides in-service training to implement a person-centered Social Services/Activity Program based on state standards and guidelines with input from Director of Nursing and Administrator.
Completes MDS 3.0 documentation and maintains current progress notes.
Generates curriculum and distributes monthly special activities calendar.
Attends all required and assigned trainings and meetings.
Leads or assists, as needed, in a crisis intervention situation.
Performs other duties as needed that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care needs of the facility residents.

Responsible for providing a supportive home-like environment for the Villagers residing in his/her residence. Will participate in the necessary skills training for Villagers to meet their goals through the AFLS assessment process.

Trains and assists Villagers on appropriate social skills, problem solving, etc.
Completes and documents for each villager all care tracking tasks in Electronic Medical Record.
Assists Villagers with ambulating across campus depending on villagers physical abilities/needs.
Is knowledgeable of and adheres to villager safety/behavior plans.
Provides watchful oversight of Villagers well-being throughout shift.
Immediately reports incidents of inappropriate behavior or accidents involving villagers to the appropriate staff.
Completes weekly inventory of villagers ADL Supplies, clothing, and snacks.

Assist with the planning and facilitation of activities, including both on-campus and off-campus events, that support life and vocational skill development.
Contribute to the development of behavioral support plans and ensure that each Villager's goals are being met through consistent teaching methods and evaluations.
Support the planning and execution of special events, field trips, and community activities that foster life skills and job readiness.
Provide monitoring in the dining hall as needed, assisting with meal preparation and promoting kitchen safety and etiquette.

Responsible for planning and implementing a quality recreation, leisure and physical fitness program for the Villagers taking into consideration their interests and abilities, including participation in community activities. This position will be part of the AFLS curriculum delivery team through developing meaningful activities and instruction to reinforce skills acquisition and maintenance. Responsible for planning and implementing Community Inclusion/Outreach opportunities for the Villagers.
Required Qualifications
Bachelor's Degree or AA Degree in Physical Education, Therapeutic Recreation, Special Education or related field with a minimum of 2 years proven experience working with IDD population or related field.
Must be at least 18 years of age, have a valid Class C Georgia Driver's License, proof of vehicle insurance, and meet company policy regarding MVR requirements.

Major Duties and Responsibilities

Coordinates community activities such as the annual Golden Games activities.
Acts as agency coordinator for Special Olympics; plans and chaperones all Special Olympics/Team Activities events in which the Villagers participate.
Plans and/or assists in planning parties, field trips and special activities to include monitoring of Villagers at these events.
Places requisitions for recreational equipment and materials needed for classroom instruction and special events that are within budget.
Provide feedback for Annual Service Review meetings for Villager, evaluating and giving input regarding Villagers' progress and goal setting as needed.
Responsible for upkeep of recreation equipment, supplies and other operating materials. Facility and materials need to be safe and clean at all times.
Assists with maintaining the cleanliness and safety of the Program Center and recreation equipment.
Assists Villagers with personal hygiene as needed.
Works with Assessment and Curriculum Coordinator to provide identified skill building classes and activities.
Attends all required and assigned trainings and meetings.
Provides engaging programming on assigned Saturdays.
Provides monitoring at dining hall as assigned.
Must be available to work nights and weekends.

Required Qualifications
Must be a Certified Medication Aide (CMA) in good standing and meet all applicable federal and Georgia state certification requirements.

Major Duties and Responsibilities
Will act as the Designated Person of Authority in the absence of the Administrator and Nurse Manager.
Ensures that the narcotic medication counts are done appropriately and in accordance to policy.
Assists in the administration of medications as needed
Is familiar with the Progressive Discipline Protocol for medication errors
Provide leadership and oversight when the Administrator and Nurse Manager is not available.
Responsible for staffing to meet ratios set by regulations and staffing profiles as set by Annandale, filling empty shifts, and covering call outs.
Assist with break coverage for team members.
Ensure completion and accuracy of shift documentation (MAR, POC, PCC NUTRITION, INCIDENT REPORTS, etc)
Report daily Assisted Living Community shift updates to Administrator and Nurse Manager.
Assist with monitoring and checking the physical condition and upkeep of the Assisted Living Community using the Assisted Living Community Inspection check list.
Assists the Administrator and Nurse Manager in training new employees as needed.
Communicates issues/concerns between shift changes, and to the Administrator and Nurse Manager.
Attends all required and assigned trainings and meetings.
Participates in social events and activities as needed, such as parties, outings, etc.
Performs other duties as assigned that may be necessary in the best interest of Annandale to meet the personal service and care needs of the Villagers.
Place Maintenance work orders for repairs as needed.
Ensure appropriate documentation is entered into Point Click Care daily by Assisted Living Community staff.
Ensure that Incident Reports are completed on time and properly within the shift it occurred.
Excellent interpersonal skills and the ability to work well with all levels of internal management and staff.
Ensure that all staff are performing their assigned duties in a quality manner and providing good customer service to Villagers, families, and co-workers.
Advance computer skills with the ability to train others in system usage.
Monitors meals in the dining hall and during special events as needed/assigned.
Monitors and assists with community volunteers that are on campus.

Position Purpose
Uses counseling methods, assures the best quality of life for the residents, and serves as an intermediary with the clients, families, Annandale staff, and other agencies. Interviews residents with issues relating to personal and family adjustments, finances, employment, food, clothing, housing, and physical and mental impairments to determine the nature and degree of the issue and helps residents resolve.
Secures information such as medical, psychological, and social factors which contribute to resident's situation and evaluates these and resident's capacities.
Maintains working knowledge of Social Security, Medicaid, Medicare, and private insurance requirements and assists family, as needed, to obtain or apply for benefits for their resident.
Reports abuse or suspicion of abuse as required by D. Scott Hudgens Center for Skilled Nursing policy and licensing agencies.
Counsel's residents individually, and in family or small groups, regarding plans for meeting needs.
Facilitates Care Plan scheduling, prepares weekly reports, and follows up on concerns expressed by team members.
Attends quarterly Quality Assurance meetings and prepares report for improvement plan.
Serves as member of Admissions Team.
Works with physicians, agencies, and other organizations to meet the ever-changing needs of residents.
Coordinates with Programs, Skilled Nursing Center, contracted Therapists, and volunteers to provide residents meaningful community inclusion and/or re-integration experiences based on individual goals and interests.
Schedules residents' appointments and coordinates with the C.N.A. Transportation Aide for transport to and from appointments and will serve as a back-up to transport residents to appointments as needed
Develops, facilitates, and/or provides in-service training to implement a person-centered Social Services/Activity Program based on state standards and guidelines with input from Director of Nursing and Administrator.
Completes MDS 3.0 documentation and maintains current progress notes.
Generates curriculum and distributes monthly special activities calendar.
Attends all required and assigned trainings and meetings.
Leads or assists, as needed, in a crisis intervention situation.

Position Purpose
Responsible for providing a supportive home-like environment for the Villagers residing in his/her residence. Provides skills training to promote independence including, but not limited to; maintaining a safe and clean room, laundry skills, personal hygiene, and interpersonal relationships with their housemates. Will participate in the necessary skills training for Villagers to meet their goals through the AFLS assessment process.

Maintains and adheres to the laundry and room cleaning schedule for each villager, and provides assistance/training to those who are unable to complete this task on their own.
Ensures villagers are appropriately dressed for the weather, before leaving the residence.
Trains and assists Villagers on appropriate social skills, problem solving, etc.
Completes and documents for each villager all care tracking tasks in Electronic Medical Record.
Assists Villagers with ambulating across campus depending on villagers physical abilities/needs.
Is knowledgeable of and adheres to villager safety/behavior plans.
Provides watchful oversight of Villagers well-being throughout shift.
Immediately reports incidents of inappropriate behavior or accidents involving villagers to the appropriate staff.
Completes weekly inventory of villagers ADL Supplies, clothing, and snacks.
Communicates and documents villager concerns involving villager behaviors or needs to the villager's case manager.
Assists villagers to create grocery lists with healthy food choices, per specific diets to shop in the community.
Performs other duties that may be necessary in the best interest of Annandale at Suwanee, Inc. to meet the personal service and care of the Villagers.
