Jordan Health is now seeking an Advance Practice Provider FNP/PA-Family Medicine who, under the guidance of the Chief Medical Officer, is primarily responsible for functioning as a member of a medical team to provide comprehensive care and education to patient and families. Assists families in identifying community resources, reducing barriers to access care utilizing support systems. Coordinates services for high-risk, multi-problem families.
