Become the Case Management Director you've always aspired to be
- Assume responsibility for the day-to-day operations and human resource management of the Case Management department.
- Oversee the interdisciplinary plan of care and the discharge planning process to ensure the effectiveness and appropriateness of services with a central focus on census management, patient care outcomes, and key care indicators.
- Act as a patient and family advocate, ensuring that services are delivered to meet the needs of patients and their families.
- Provide guidance and support to Case Managers and other staff, including training on managing caseloads and interpreting regulations, policies, operational procedures, and objectives. Review operations to ensure a high level of quality consistent with organizational standards.
- Build relationships with insurance companies, self-insured employers, case management firms, and other healthcare networks.

Qualifications
- Current CCM or ACMTM certification is preferred.
- Must be qualified to independently complete an assessment within the scope of practice of his/her discipline.
- If licensure is required for the discipline within the hospital's state, individual must hold an active license.
- For Nursing, must possess bachelor's degree in nursing (BSN) with RN licensure.
- For other eligible health care professionals, must possess a minimum of a bachelor's degree; a graduate degree is preferred.
- Three years of hospital-based Case Management experience, including Utilization Review and Discharge Planning experience.
- May be required to work weekdays and/or weekends, evenings and/or night shifts.
- May be required to work on religious and/or legal holidays on scheduled days/shifts.

Do you have the education and experience to be a Family Engagement Specialist I?
Bachelor's degree in Social Work, Nursing, Psychology, Public Health, Human Services, Communications, Counseling, Education, or a related field; Master's degree preferred.
Minimum of 2-3 years' experience in family support, grief counseling, or a related field, preferably in healthcare or organ donation settings.
Backgrounds that demonstrate advanced interpersonal communication, conflict resolution, and the ability to guide individuals through complex, emotionally charged decisions
Knowledge of the organ donation process and relevant regulatory frameworks is highly desirable.
Ability to navigate sensitive and complex situations with professionalism and poise.
Proficient in Microsoft Office Suite and experience with data management systems.
Bilingual (English/Spanish) is a plus.

The Lubbock County Juvenile Justice Center Food Preparer shall prepare nutritious meals in accordance with daily menus in a timely manner. The Food Preparer shall maintain sanitary kitchen conditions, in accordance with local, state and federal guidelines.
* Follow daily menu plans and approved recipes. Request supervisory approval of any menu changes.
* Serve all meals in accordance with assigned meal times.
* Maintain daily record of all meals served. This includes but is not limited to meal production sheets for all meals served.
* Utilize appropriate equipment to prepare and serve each meal in a nutritious manner.
* Prepare nutritious snacks for the residents each evening.
* Check all incoming food products for proper packaging and damage before storing.
* Store opened packages of dry goods in plastic canisters that are labeled as to contents.
* Store all food products including but not limited to: fruit, potatoes, and onions, in appropriate bins or at least six inches off of the kitchen floor.
* Maintain temperature records on refrigerators and deep freezers to make sure food is properly cooled.

* Dust all storage shelves and cabinets by removing stored food or supplies on a weekly basis.

* Maintain shift inventory and security of all knives and other objects in knife drawer. Keep knife drawer locked at all times. Notify Food Service Supervisor or Support Services Manager immediately of any missing knives, other potential weapons, or any violent, dangerous situation.
