Job Summary:
The Nursing Unit Manager is responsible for leading and managing a clinical unit with 24/7 accountability. This includes supervising staff (RNs, CNAs, etc.), ensuring quality care, maintaining regulatory compliance, and managing operations, budget, and performance.
Key Responsibilities:
Clinical Oversight: Ensure safe, evidence-based patient care; act as clinical resource; support care coordination across teams.
Staff Management: Hire, train, mentor, and evaluate nursing staff; support performance improvement and continuing education.
Operations & Compliance: Manage daily operations, budgets, staffing, and regulatory readiness; lead quality and safety initiatives.
Leadership: Facilitate unit meetings, drive continuous improvement, and maintain open communication with teams and leadership.
Qualifications:
Education: Bachelor's degree in Nursing (required)
Experience: Minimum 2 years in a nursing leadership role
Licenses/Certs:
Active Illinois RN license
Current BLS certification
Skills:
Strong leadership, communication, and organizational skills
Experience with EMR systems (EPIC and MEDITECH preferred)

Leads Risk Management Committee in documenting, analyzing, and addressing clinically related complaints including patient satisfaction.
Leads Risk Management Committee in setting annual risk management goals and tracking progress toward those goals.
Completes annual risk management report for the governing body and key management staff on health care risk management activities and progress in meeting goals.
ADMINISTRATIVE RESPONSIBILITIES

What you will be doing?
The Manager of Community Health Services is responsible to coordinate community health services for individuals in I/DD Service Programs for assigned regions according to DHS, HCBS, and CARF requirements. The essential job responsibilities include collaborating with internal and external health care providers in the implementation of individualized strategies that mitigate identified health risks and maximize the client's progress toward selected outcomes and community inclusion.
The Manager is responsible for the hiring, training, ongoing supervision, and performance evaluation of the Community Health Services team. This program support team ensures timely scheduling, completion, and documentation of routine, required, and urgent client medical appointments per applicable standards. The team supports clients who require intensive care including but not limited to toileting and bathing, special diets with personal and/or enteral feeding, and enhanced funding. This position is expected to participate in direct service as needed.
The Manager of Community Health Services position requires on-call responsibilities to assure quality services 24 hours a day and 7 days a week, which is handled via an on-call rotation. The Manager is expected to serve as a role model; demonstrating the organization's core competencies while carrying out essential job responsibilities. They are responsible for creating and maintaining a positive, professional, and safe environment for clients and staff to live, learn, and work.
Essential Job Responsibilities:
• Ability and willingness to serve as a team leader, provide team support, participate in group problem solving, and collaborate with agency and stakeholder staff.
• Monitors and ensures staff compliance with all agency, state, and federal training regulations; reports any concerns to Program Director as appropriate.
• Provides and/or arranges for staff training on client plans and strategies; maintains documentation records of staff trainings.
• Conducts monthly staff meetings; submits attendance and minutes to the Program Director; reviews meeting topics with all staff not in attendance at the meetings.

• Drafts and presents corrective counseling documents to address staff performance deficits; discusses matters with staff in a manner that preserves dignity and encourages staff development and behavioral change; maintains records of discussions and documentation.
• Participate in service planning meetings annually and as needed per timelines established by licensure requirements.

• Participate in providing direct services to clients as assigned in order to meet client and program needs; this includes cross training and coverage in other programs to ensure the agency's continued success. Maintain documentation of services provided in client case record.
• Participate in the assessment of clients per program and regulatory standards.
• Obtain medication administration authorization at assigned sites to include additional authorizations for insulin, injections, and enteral feeding and care.
• Maintain medication administration authorization by attending annual training, following all policies and procedures, and minimizing preventable medication errors.
• Facilitate access to and administration of routine and as-needed prescription and over-the-counter medications for clients including but not limited to coordination with internal staff and external providers (pharmacies, DME suppliers, etc). Maintain documentation of services provided in client case record.
• Collaborates with supervisor, department staff, stakeholders, and/or other providers to ensure timely refills, prior authorizations, consents, etc. are obtained.
• Collaborates with supervisor, department staff, clients, and/or guardians to troubleshoot medication issues (not covered, pending insurance matters, etc); obtains authorization and secures payment in order to address urgent or emergent medications.
• Participates in medication check-in processes upon receipt of monthly refills, verifying accurate and proper labeling and packaging of client medications; coordinates delivery of medication to program locations upon completion of check-in.
• Ensures completion of monthly audits of client PRN medication supplies at assigned locations; takes appropriate action to resolve any identified issues regarding supply, expired medications, etc.
• Collaborate with Program Managers to ensure that medications are stored and secured appropriately per licensure and/or regulatory requirements at program locations.
• Participate in client referral and admissions processes using solutions-focused language and positive approaches.
• Receive and review medication error reports to ensure completion and accuracy; coordinate with RNT, Program Manager, and/or Director for further actions and/or follow-up as needed. Maintain documentation per procedure.
• Audits medication errors in programs; delegates and/or completes additional error reports as required.
• Participates in ongoing quality assurance measures including but not limited to reviews of: medication orders, medication administration records, client health records, and client health assessment tools to ensure compliance with licensure requirements.
• Exercise sound judgement in recognizing and responding to emergency situations involving clients, staff, and/or the physical environment/location; take appropriate action according to agency policies and procedures including completing required incident reporting or documentation.
• Collaborate with program staff and management to develop and implement a plan of action using solutions-focused language and positive approaches in the event of client emergencies or critical needs
• Consults with Program Director for approvals as required including proposed changes in programs, procedures, or schedules prior to implementing changes.
• Participate in regular staff meetings at program locations using solutions-focused language and positive approaches; reviews meeting topics with all staff not in attendance at the meetings for assigned caseload.
• Lead by example in implementing Home and Community Based Settings Rules and applying principles to client, family, and stakeholder interactions.
• Report problems or concerns observed regarding physical environment, clients, and/or staff to site/program Manager and Client Services Director to ensure they are addressed immediately. Provide any performance and discipline feedback regarding staff to program Manager.
