Givens Communities is looking for a full-time Human Resources Generalist to help support our Growing Home First Team. Essential Duties and Responsibilities:
* To ensure adequate supply of clean utensils and dishes and pots and pans for use.
* Scrap trays, soak dishes, utensils, and assemble dish machine.
* Operate an industrial dishwasher.
* Set up and wash pots and pans.
* Removal of trash from kitchen.
* Operate an industrial dishwasher.
* To ensure proper safety and sanitation guidelines are used.
* Perform opening and closing side work.
* Clean up spills or broken glassware immediately.
* To provide quality food to residents, visitors, employees and guests in a timely manner.
* Making tea and coffee, cleaning coffee machines in health care.
* Working on tray line, setting up the trays with utensils, napkins, condiments, read and call out the diet cards, some bending over for tray and lifting up to 10 pounds.
* Putting plates, desserts, and drink on tray then put on cart, some bending is required as well
* Cleaning area and putting away milk and juice.
* Empty and clean tea urns and coffee pots.
* Pick up trays and trash from floor
* Assist with sweeping and mopping the floor.
* Must be able to lift trash cans.
* Reports all unusual conditions, behaviors, and/or resident reactions to the appropriate Licensed personnel in a timely manner.
* Responds and reacts in a kind, polite, and professional manner to residents, families, visitors, coworkers, and supervision.
* Responsible for protecting any confidential information concerning residents, family, and/or the organization.
* Must demonstrate knowledge of and adhere to all Safety procedures and facility Fire, Disaster, and Bomb Threat procedures.
* Adheres to all organizational, departmental, and nursing procedures in the performance of position responsibilities.
* Must attend organizational or departmental conferences and/or in-services as scheduled.

Facilitate the admission process to ensure regulatory compliance, resident safety, and smooth transition into Memory Care.
Coordinate and complete all psychosocial components of Memory Care admissions.
Meet with residents and families at admission to assess psychosocial status, adjustment needs, support systems, and advance directives.
Complete psychosocial assessments and document findings in the electronic medical record (EMR) in accordance with regulatory requirements.
Educate residents and families regarding resident rights, grievance procedures, and care planning processes.

Collaborate with the interdisciplinary team to ensure discharge planning begins upon admission and continues throughout the resident's stay.
Monitor rehabilitation progress to support appropriate length of stay and level of care determinations.
Meet with residents and families to discuss discharge options, preferences, and post-discharge needs.
Arrange and coordinate post-discharge services, including home health therapy, durable medical equipment, transportation, and community resources.
Obtain physician orders for home health services, face-to-face documentation, and equipment as required.
Complete discharge summaries and required documentation in the electronic medical record (EMR).
Communicate discharge plans and updates to nursing, therapy, physicians, and the care plan team.
Conduct post-discharge follow-up calls to residents and/or families to assess adjustment, identify concerns, and ensure a safe and smooth transition of care.
Communicate identified post-discharge issues to appropriate team members for follow-up.
Memory Care Assisted Living Responsibilities

Qualifications
Master's degree in Social Work (MSW) required.
Current state licensure or eligibility as required.

EMT - Essential Duties and Responsibilities:

Givens Communities is looking for a full-time Clinical Educator to join our Training and Development team in supporting our health services teams. The primary purpose of this position is to oversee and facilitate an on-site state approved certified Nurse Aide I training program and to oversee all aspects of clinical education for Givens Communities. This position will be based out of our Black Mountain campus, Givens Highland Farms, with moderate travel required to support our Asheville campus, Givens Estates.
What you'll do:
Serve as the Program Coordinator and primary instructor for the state approved Nurse Aide I training program
Become a subject matter expert in all components and requirements of the state-approved Nurse Aide I training program and ensure compliance
Evolve and implement Nurse Aide I training program in compliance will all DHHS requirements
Facilitate and instruct Nurse Aide I training students and ensure the highest probability of success for each student possible
Collaborate with the Health Services Directors, Directors of Nursing, Home First Executive Director, Staff Development Coordinators, and the Training and Development Director to meet the needs of clinical education
Initiate, plan, develop, organize, coordinate, and present clinical education programs for Givens Communities clinical staff and non-clinical staff working in clinical areas
In coordination with DONS, actively develop and participate in annual nursing skills days for all campuses
Provide medication aide training as requested
Develop survey readiness training and conduct survey readiness audits in collaboration with DONs
Offer the annual state Infection Control Course for adult care homes
Develop a Givens Communities clinical course catalog of training materials and programs with the support and direction of the Training and Development Director
Perform any other duties and/or tasks that may be assigned on an as-needed basis by an authorized supervisor
What you'll need:
Must have current unencumbered RN license in the state of North Carolina
2 years (4000 hours) as an RN, with 1 year (2000 hours) in a Long Term Care facility licensed as a skilled nursing facility
Must have completed or be willing to complete a course in teaching adults or have experience in teaching adults or supervising nurse aides
Must have the ability to become a Med Aide and CPR instructor
