Ability to work from home on the weekends and holidays (after training)! Engage patients in checking their vital signs, weight, and to answer questions related to their medical condition. Evaluate data to determine if any medical intervention is warranted, and outreach to the provider for orders. Follow-up care coordination via phone/video. We serve patients in portions of 9 counties in Northeast Wisconsin. Salary is based on experience, and will be provided before interview. Shift Hours: Casual Part-Time, 8 hours per pay period. Day shift 8:00am-4:30pm, every other weekend and 1-2 holidays per year Job Description: Join our amazing home health team! Nurses use assessment skills to review and complete interventions related to patient clinical data (vital signs, weight, and health related questions). Patients in the ICU have a wide range of needs as these are Level I trauma, critical care and CRRT dialysis patients requiring one-on-one care. As a Patient Care Nursing Assistant (PCNA) on the team, you will perform various tasks, ensuring patients receive frequent attention and observation as needed. This is a hands-on role that requires a present, attentive and patient caregiver who is willing to learn new medical technologies and thrive in a fast-paced environment. On this team, you will gain a wide variety of skills you can use in advanced positions or other departments. A caregiver in this position works 7:00 a.m. - 3:30 p.m. with every other weekend and 3 holidays per year requirements. A caregiver who excels in this role will: Prepare units, patient rooms and patient beds for receiving patient admission, returning surgical patients, transfers or emergencies. Provide and document assigned care, such as bathing, changing/refreshing bed linens, dressing/undressing, feeding, etc. Take and document the patient's temperature, pulse, respirations, blood pressure, pulse oximetry, weight and height. Perform treatment procedures, such as venous blood draws, enemas, application of ice and heat packs, non-sterile compresses, skin preps, setting up oxygen equipment, bed scale weights, setting up overhead trapeze and Buck's traction and removing urinary Foley Catheters. Assist with pre-operative and post-operative care, such as turning and positioning patients with proper body mechanics, applying/utilizing special equipment and ambulating the patient as directed in a safe, prescribed manner. Assist and accompany patients during admission, transfer and discharge. Perform hourly safety rounds, anticipate patient needs and respond to patient call lights promptly. Collect, properly label and transport specimens to proper area. Document patient care in the electronic medical record.
