REPORTS TO: Nurse on duty or Supervisors; annual performance evaluation by DON/ADON with input from Unit Nurses.
DUTIES & RESPONSIBILITIES:
* Actively participates as a "team" member showing respect, dignity,
accountability, professionalism and ethical values toward residents, family
and staff while maintaining confidentiality.
* Follows hierarchy and respects and performs all duties asked of them
within the standards of practice.
3. Meets resident ADL needs including personal hygiene, oral/denture care,
bathing, grooming, and dressing as per policies.
4. Follows and comprehends resident plan of care regarding physical and
emotional well-being of each resident including but not limited to all
mobility, hygiene needs, and emotional care as directed by individual plan
of care.
5. Has a working knowledge and demonstrates usage of Resident Rights, abuse
and neglect policy, including report requirements and protocol.
6. Follows all policies regarding safety measures and equipment competency,
including supply needs and replenishment.
7. Consistently follows all Infection Control policies and measures regarding
hand washing standard precautions, linens, clean and orderly environment
including all resident care areas.
8. Meets elimination needs of all residents per policies and documents
toileting times and initials incontinence and continence. Follows routine
bowel regimes, documents and reports to Nurse.
9. Follows Rehab Nursing measures. Some examples are encouraging
independence and decision making, range of motion, ambulating, positioning
and alignment, transfers and transport, moving resident up in bed, etc.)
Pays special attention to good body mechanics and safe lifting techniques.
* Is responsible to look at the activity calendar at the beginning of
each shift and assures that all residents are transferred to and from
all activity programs in a consistent, timely manner.
* Assists with admissions, discharges, transfers, and transport to
appointments.
* Reports any needs of resident, signs or symptoms of illness or distress,
injuries or accidents including falls, skin tears, bruises, and red areas
to Nurse on duty.
13. Measures, records and documents vital signs, height/weight, I&O's and any
information that is pertinent to resident care plan, resident care profile
and reports all out of limit findings to the Nurse on duty.
* Participates in resident care conferences as appropriate and the process
of care planning in a positive and constructive manner.
15. Follows all pertinent personnel policies re: attendance, breaks, meals,
staff meetings, mandated requirements per policy, float assignments,
dress code etc. Work schedules may be subject to change based on the
needs of the department to provide the highest quality care to our
residents.
* Has a working knowledge of Corporate Compliance regulations and promptly
reports any potential Corporate Compliance violations to the Corporate
Compliance Officer.
* Is familiar with and demonstrates Continuous Quality Improvement
principles and participates in the process as appropriate, focusing on
meeting the customer service needs of the internal and external customer.
18. Must possess the highest ethical standards with respect to discretion and
regard for confidential information.
19. Provides age appropriate care to adult and geriatric patients/residents.
20. Is responsible for attending all mandatory educational programs as
required by position including successful completion of orientation
program and maintains 12 CEU's per year. (6 CEU's per 6 months)
* In order to provide the highest quality care to our patients and
residents, individuals may be required to work hours beyond those normally
scheduled. Work schedules may be subject to change based on the needs of
the department.

EDUCATION/TRAINING: Certified as a Nurse Aide with the New York State Department of Health. Current Heartsaver certification required.

EMPLOYMENT SCHEDULE: Required to work weekends, holidays and various shifts as needed.
PHYSICAL/MENTAL DEMANDS: Involves standing and walking most of the day. Required to lift, move, position and transfer/transport equipment and residents. Ability to adjust to various staff members, assignments and clinical areas.
WORKING CONDITIONS: Exposed regularly to disagreeable odors, sights, body wastes, etc.

What you will do:

* Provide Resident Care;
* Assists resident with hygiene needs (baths, mouth care, hair care, removes facial hair-shaving, nail care, back care).
* Assists with feeding of those residents able to chew and swallow without difficulty.
* Employs special oral feeding techniques for residents with special needs/difficulties.
* Measures intake and output accurately and documents on worksheet and graphic sheet.
* Check and report bowel movements and character of stools as instructed.
* Performs routine colostomy care.
* Answer resident calls promptly.
* Ensure that residents who are unable to call for help are checked frequently.
* Weighs resident using upright, chair and bed scale.
* Measure and record temperatures, pulse, and respirations, as instructed.
* Maintains safety measures.
* Transfers or weighs residents using appropriate lift.
* Assists residents in use of bathroom, shower urinal, bedpan and commode for residents.
* Keep residents dry (i.e. change gown, clothing, linen, etc. when it becomes wet or soiled).
* Change bed linens, make beds, provide extra covers as requested. Keep linens tight to avoid wrinkles from forming under the resident.
* Ambulates residents, including those using canes and walkers, as instructed.
* Assist in transporting residents to/from appointments, activity and social programs, etc.
* Position bedfast residents in correct and comfortable position, assist with turning residents, and assist with lifting, turning, moving, positioning, and transporting residents into and out of beds, chairs, bathtubs, wheelchairs, lifts, etc.
* Performs skin assessment and notifies Charge Nurse of any abnormalities in skin integrity.
* Measures and documents vital signs and weights. Report any variation in vital signs and weights. Report any variation in vital signs to Charge Nurse.
* Apply cold packs and warm compresses.
* Upholds residents Bill of Rights.
* Documents resident-related tasks in appropriate records.
* Assists with postmortem care and discharge of resident transfer.
* Assists with postmortem care and caring for personal belongings of the deceased.
* Attend and participate in scheduled training, in-services, and educational classes a required.
* Maintain confidentiality of all resident care information.
* Other duties as assigned.

Certificate of Completion will be maintained and placed in the staff members Human Resource file.
Completion of 5 days (40hrs) of orientation with the RCA Competency being completed before the staff member works on their own.
Staff members should be completing their orientation (40 hrs) and working simultaneously on the RCA Modules on Relias.
Staff members may work on their own after completing their (40 hrs) of orientation and the RCA Competency is completed upon completion of orientation. They can continue working on the Relias modules and post tests while working and must complete them within 21 days from their start date.

Assist residents with self-administration of medications in accordance with current policies and procedures. This includes supervision, assisting, and charting of medications.
Order medication from appropriate pharmacy as needed, and pick up if required. Log receipt and changes to medications in appropriate charts.
If required, transport residents to and/or from medical appointments, emergency room.
Insure safe and secure environment for residents and employees. Be aware of visitors to the facility, and report any unusual behavior or requests to the appropriate Case Manager or Team Leader.
Monitor resident rooms to insure a safe and hygienic condition.
Report all unusual or out of character observations of residents to the appropriate
Case Manager or Team Leader.
Maintain awareness of resident's whereabouts, and respond and assist in medical emergencies. Be aware of residents requiring a wander alert bracelet, and how to reset alarms.
Maintain polite, sensitive, mature, cooperative relationships with residents, resident's visitors, community members, and employees.
Coordinate with other facility staff to insure resident's health and welfare are being met.
Attend all mandatory In-service training meetings.
Be knowledgeable of, and implement as required, Woodbrook's fire and evacuation procedures. Be familiar with fire alarm locations, and routes of evacuation.
If required, assist with scheduled activity programs. Encourage residents to participate in community events.
Protect the personal belongings of each resident including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, memorabilia, etc. Promptly report missing items according to established policy and participate in efforts to locate missing items.
Ensure resident medication changes are properly implemented.
Must maintain regular and punctual attendance.
Must maintain confidentiality as it pertains to Health Insurance Portability and Accountability Act (HIPPA) requirements.
Physical Demands
Regularly speak, see, or hear.
Frequently sit, stand, walk, use fingers to handle, or feel, reach with hands and arms.
Occasionally lift, move, push, carry, or pull up to 25 pounds.
Specific vision abilities required include close vision, distance vision, peripheral vision, and ability to adjust focus.
