Hospice Liaison RN - Position Summary
The Asante Hospice Liaison RN, evaluates and facilitates Hospice referrals from the systems hospitals, APP clinics and community referrals. This RN assesses patients for hospice eligibility and coordinates the transfer of inpatient information to the hospice office; collects and processes information for Hospice referrals; and alerts appropriate team members for necessary coordination of care.
As a Hospice Liaison RN for Asante - you will have the opportunity to:
* Collaborate with referral sources, resource management team members, Hospice Medical Director as well as discharging hospitalists, DME provider and/or scheduler to facilitate transition of care.
* Assess patient and families physical, psychosocial and emotional condition to determine patients' safety and the care needs in the home setting or setting in which the patient will reside.
* Promote a positive working relationship by collaborating with peers throughout the system by maintaining open lines of communication and accepting and providing constructive feedback in a supportive and nonjudgmental manner.
* Provide ongoing assessment of patients receiving hospice general inpatient level of care or a non-related condition and provide coordination in transfer of patients to GIP status.
* Contact patient/family to explain the hospice benefits/services and identify if the patient/family agree with hospice and palliative care vs curative or aggressive treatment.
* Discuss goals of care conversations with prospective hospice patients and their family members.
* Develop and maintain relationships with resource management team members, community referral sources such as ALF, RCF, NF and Memory care facilities throughout the communities in which Asante Hospice serves and facilitate and provide education to the care facilities and provide support for team members as needed utilizing the Hospice Team members as indicated.

Education
* Bachelor's in nursing (BSN) preferred
Experience
* 2 years clinical hospital experience as an RN and 1 year current Hospice, Palliative care, or Home Care experience; required
License/Certification
* RegisteredNurse (RN) licensed by the Oregon State Board of Nursing required upon start
* Basic Life Support for the Healthcare Provider (BLS) certified through the American Heart Association (AHA) required upon start
* Possession of a valid Driver's License with proof of safe driving record for the past ten years and proof of insurance required

Coordinates and facilitates a quality assurance program for resident health care services and collaborates with RSC.
Participates with the facility administrator and other department directors in development and maintenance of practices and procedures that promote infection control, fire safety, quality assurance and hazard reduction.
Facilitates educational programs in accordance with licensure and facility needs, including new employee orientation and annual in-service requirements, and monthly resident education.
Is available 24/7 by telephone for staff consultation - returns phone calls in timely manner.

The RN shall be informed regarding resident's conditions, including when a significant change in a resident's condition warrants nursing or medical intervention. Assures that health services staff are informed of resident change in conditions.
Participates in those committee meetings as assigned by the Administrator.
Provides Hepatitis B shots to staff and monitors for TB and other health conditions as necessary.
