Assists residents in preparing for and transporting to and from medical appointments, activities, and social programs. Maintain Restorative Records for each guests/residents. Maintains knowledge of federal, state/provincial and local resident care regulations Quality Indicators Strive for excellent quality care and service delivery as measured in the Quality Assurance and Performance Improvement (QAPI) process. Competent of behavior interventions and management. Demonstrate and knowledgeable of the following key quality improvement areas of the DNA/C.N.A. role: ADL, skin care, dining assistance, bowel/bladder elimination Falls and fall prevention Skilled nursing transformation and Resident Centered Care model Guest/Resident, Family and Visitor Relations Respond to guest/resident, family, visitor's inquiries, concerns in a timely and professional manner using the Problem Resolution process. Risk Management and General Safety Partner with community team to ensure community is in compliance with national/provincial regulations pertaining to occupational health and safety requirements and promotion of Risk Management programs and policies; adherence to safety rules and regulations. Practice safety procedures at all times including Personal Protective Equipment (PPE), fire extinguishers, Safety Data Sheets (SDS) and Lockout Tagout procedures. Understand and practice the proper method of attending to and disposing of and the possibility of exposure to blood borne pathogens, bodily fluids, infectious waste, sharp sticks and hazardous materials. Report occupational exposures to blood, body fluids, infectious materials, sharp sticks and hazardous chemicals immediately. Ensure oxygen tanks are stored safely, exchanges guest/residents tanks when empty and monitors to make sure liters of oxygen are at prescribed levels Training, and Contributing to Team Success Participate actively as a member of a team and committed to working toward team goals. Job Description Assists residents with activities of daily living such as bathing, dressing, and personal hygiene. Assists residents in preparing for and transporting to and from medical appointments, activity and social programs. Checks residents' vital signs, measures weight and height, and records data as instructed; reports any variances to Charge Nurse. Answers resident call lights promptly to attend to needs; frequently checks those who are unable to call for help. Performs restorative and rehabilitative procedures, as instructed. Assists residents to walk with or without self-help devices, as instructed.
