Natchaug Hospital provides transportation from school to the Joshua Center Program, and then to the student's home.
Job Summary
Lead Behavioral Health Clinician works in coordination with interdisciplinary teams to organize, prepare, and deliver evidence based clinical services in the Child/Adolescent partial hospitalization program. Clinical services include individual, group and family therapy. Maintains ethical principles and professional standards of practice in adherence to the discipline's professional code of ethics. Assumes leadership responsibilities as assigned by clinical leadership. May assume clinical oversight of daily operations in the absence of Program Manager.
Key responsibilities of Lead Behavioral Health Clinician:
Conducts a comprehensive bio-psychosocial assessment and establishes a care plan, as appropriate.

As a member of the Hartford Hospital Palliative Care Department, the Palliative Care social worker will provide comprehensive clinical and psychosocial assessment and intervention for patients with serious illness. The Palliative Care social worker provides support around adjustment to illness, decision making, eliciting goals of care and family coping along the illness trajectory. Job Responsibilities:
1. Performs comprehensive psychosocial assessments of hospitalized patients with serious illness. Assists the health care team to integrate psychosocial factors into treatment plans: Ability to analyze significant psychological, physical, emotional, social and economic issues of patients/families and to effectively communicate the relevance of such to other health care providers. Ability to identify, analyze and suggest interventions for difficult patient/family management situations.
2. Provides social work intervention including counseling and support to assist patients/families. Demonstrates ability to form empathic relationships with patients/ families who have significant emotional adjustment and coping issues.
3. Effectively addresses quality of life and end of life issues within a culturally competent framework to support the equitable delivery of excellence in health care to all patients and families.
4. Assess patient/ family's level of understanding, barriers to learning, and to provide age-specific and culturally competent education as needed. Able to effectively assess and discuss the psychosocial issues of palliative care to patients/families
5. Arranges, attends and actively participates in patient/family conferences, in person, as well as via available telehealth resources.
6. Demonstrates commitment to continuous improvement by seeking ways of improving care, effectiveness, and efficiency within the Palliative Care Service through active participation in H3W Huddles and meetings.
7. Maintains patient records and other documentation in a manner consistent with standards of clinical care, hospital, department, third party and regulating agencies' policies and procedures and standards of risk management.
8. Attends Palliative Care Team rounds and all required departmental meetings.
9. Demonstrates awareness of, and adherence to, Hartford Hospital policies and procedures. Maintains confidentiality and complies with professional ethics according to professional departmental and organizational standards and maintains current Connecticut license to practice social work.
10. Participates in continuing education opportunities to enhance and broaden skills relevant to job performance; maintains documentation of continuing education and in-service activities provided.
11. Participates in educational programs for Hartford Hospital staff as an expert on psychosocial issues in palliative care.
12. Works with members of the team to provide bereavement follow up for our palliative care patients.
13. · Hold a master's degree from a social work program accredited by the Council on Social Work Education. Completion of Post-MSW Fellowship or Certification Program in Palliative Care preferred.
· Two years post MSW work experience in a healthcare setting.
· Work experience in the field of palliative care, hospice care or relevant disease-specific specialty care.
· Licensed Clinical Social Worker (LCSW) with valid licensure in the State of Connecticut.
· Seek Advanced Palliative and Hospice Social Worker - Certified (APHSW-C) certification within 2 years.

Several types of complex cardiac procedures are performed in our Cath Lab, this includes structural heart procedures, some structural heart procedures are performed in the Hybrid OR in collaboration with the OR team. Great team work is not only encouraged but it is essential. The Interventional Radiology Technologist is an integral part of the Cath Lab team! He or she will develop a complex skill set that is not only valued by the entire team, but also contributes to positive patient outcomes. This technologist will learn to function in different roles during the invasive cardiac procedures. These roles are comprised of the circulating role, monitoring role, and scrub role. This individual will develop a comprehensive knowledge base of cardiac anatomy and physiology.
The technologist will learn to work with a wide array of equipment such as contrast injectors, ultrasounds, IVUS, mechanical thrombectomy devices and atherectomy supplies and become knowledgeable in a vast array of supplies including stents, balloons, catheters, coils, etc.
He or she will be responsible for entering patient information into the x-ray equipment, annotating, post processing and, sending patient images to the PACs system, adhering to the Radiation Safety and Standards such ALARA principles.

Manages daily operations of patient meal services
Partners with nursing to meet patients' needs by doing education with nursing and ensuring we are following appropriate guidelines

Implements established clinical best practices to deliver exceptional care and service at the appropriate cost.

Facilitates all human resources functions including hiring, training, mentoring, evaluating, conflict resolution, constructive discipline, and termination.
Manages employees to ensure that food safety standards and regulatory guidelines are met.

The Clinical Coordinator is responsible for assisting with management and operation of clinic. This position assumes full responsibility for the dialysis clinic in the absence of the Administrator.
Growth:
Assist with clinical and operational processes to improve patient health and minimize hospitalizations and missed treatments.
Assist with planning/coordinating patient scheduling to assure timely acceptance of patients and effective staffing levels.
Demonstrate effective use of supplies and staff labor hours.
Responsible for updating all logs and ensuring that dialysis run sheets and logs are sent to billing.
Perform duties as assigned to meet the patient care or operational needs of the clinic.
Outcomes:
Coordinate the functions of all departments and develop standards and methods of measuring patient care services, including a chronological record of services provided to patients as required by the ESRD Network Coordinating Council and Medicare.
Work with Administrator to maintain chronological, thorough, and appropriate documentation in the patient record of all treatments, activities, and communication with the patient, physician, and other healthcare professionals.
Promote quality management program through education and involvement of staff and patients in outcomes as well as overall responsibility to achieve corporate goals for quality.
Assist with program's target goals for patient outcomes in accordance with quality patient care and Company goals.
Review all incident reports; make recommendations and take action relative to incidents as appropriate; report at monthly QAPI meeting as delegated by Administrator or Management.
Operational Readiness:
Knowledge of and remain current with federal, state, local laws and regulations, including health care professionals practice act requirements.
Work with Administrator to assure clinic is in compliance with all applicable federal, state, and local laws and regulations.
Assist Administrator with development, implementation and follow up of Corrective Action Plans required for internal and external surveys.
May assume Charge Nurse's responsibilities as needed.
May fulfill responsibility of facility Alternate CEO as delegated by Governing Body.
Conduct and/or participate in both formal and informal meetings with the governing body, Regional Director, Medical Director and the staff as delegated. Assure that appropriate staff meetings, in-service education meetings, and team patient care planning meetings are held monthly as delegated. Assure that Quality Assessment & Performance Improvement Program is current at all times as delegated.
Establish, maintain, and submit all required records and reports concerning staff, patients, and the operational affairs of the center if delegated by Administrator.
Continuously monitor to ensure that a safe and sanitary physical environment is maintained throughout the facility; that all equipment is maintained and functions properly; and that adequate and appropriate inventory levels of all supplies are available and used correctly.
Oversee the maintenance of equipment and supplies to meet current laws and regulations.
Responds to all emergencies in clinic. Familiar with emergency equipment and all emergency operational procedures.
Supervise the maintenance of equipment, building areas occupied by the center and other property belonging to the center.
Partnerships:
Maintain collaborative working relationship with Medical Director and physicians.
Establish and maintain a positive relationship with all Administrators, area hospitals, agencies, vendors and the community.
Actively promote GUEST customer service standards; develop effective relationships at all levels of the organization.
Respond effectively to inquiries or complaints.
Staff Development:
Ensure all staff meet required qualifications for position held and perform duties within limitations established by and in accordance with company policy/procedures, health care professionals practice acts, applicable state and federal laws and regulations.
Serves as a resource/subject matter expert for patient needs and concerns, staff education and in-service sessions as necessary.
Assists with recruitment, training, development, and supervision of all personnel.
Assists with maintaining effective personnel management and employee relations, including evaluating the performance of all personnel and counseling employees.
Uphold management goals of corporation by leading staff in team concepts and promoting a team effort.
Effectively communicate expectations; accept accountability and hold others accountable for performance.
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. Requirements include:
Current RN license in applicable state. License must be maintained as current and in good standing.
12 months experience in providing nursing care, including 3 months of experience in providing nursing care to patients on maintenance dialysis.
CPR certification required within 90 days of hire.
Confirmation of ability to distinguish all primary colors.
Demonstrated ability to function in a leadership position and to perform in new and emergent situations with sound judgment.
Demonstrated working knowledge of the English language and ability to communicate verbally and in writing.
Must have basic computer skills, including Microsoft Office (Word, Excel, Outlook); proficiency in all USRC clinical applications required within 90 days of hire.

3. Provides social work interventions as appropriate.
a. Ensures that psychosocial assessments identify issues that are impacted by the terminal diagnosis and the symptoms of the patient's disease.
b. Conducts ongoing assessment during any contact or interaction related to the patient, caregiver and family.
c. Provides short-term individual and family counseling for specific problems solving or symptom relief.
d. Provides advocacy as needed. Ensures that interventions are initiated according to patient and family preferences.
e. Enhances the strengths of the family's system.
f. Assesses and refers for ongoing bereavement services.
g. Maintains the dignity of the dying patient and the grieving family.
h. Supports the patient's, caregiver's and family's unique spiritual and cultural beliefs.
i. Provides holistic family-centered care across treatment settings.
j. Provides appropriate crisis intervention as needed.
k. Assists with decision making and care planning and prepares advance directives as needed.
l. Provides information and assistance with decisions for funeral planning according to patient and family preferences.

4. Documents the evaluation and assessment of patients, caregiver's and family's psychosocial status, needs and interventions provided.
a. Documents clinical notes after every patient contact and includes basic content of each session, progress and future plans.
b. Completes a plan of social work treatment including goals, plans for intervention and implementation
c. Develops Plan of Care in consultation with patient, caregiver, family and IDG members resulting from psychosocial assessments and evaluations.
d. Updates/initiates MSW physician orders
e. Participates in the development and revision of the Plan of Care.
5. Maintains professional knowledge and skills by attending educational meetings, workshops and in-services, maintaining knowledge of current literature and resources, and incorporating learning into practice.
6. Participates in continuing education programs as requested or as required under state and federal regulations.
7. Provides in-service training as needed.
8. Ensures that the goals of the strategic plan that relate to community visibility and development are met.
a. Responds to community requests for information and education.
b. Participates in public speaking engagements as needed.
c. Serves as liaison to community extended care facilities and inpatient providers.
Qualifications
Master's degree in social work from a school accredited by the Council on Social Work Education with a minimum of three (3) years clinical experience in Social Work preferred.
2. Maintains current state license for LCSW, if applicable.
3. Knowledge of social work ethics and accepted social work practice standards.
4. Willing to work the hours necessary to enable Hospice to meet its commitments to the patients/families.
5. Interest in keeping abreast of trends and developments in social work, Hospice care and related fields.
7. Ability to work as part of a team as well as function independently and coordinate activities as appropriate to accurately complete tasks in a timely manner.
8. Understands and maintains confidentiality.
9. Complies with HIPAA regulations.
10. Ability to relate well with staff, general public or individuals with a life limiting illness and their families during a stressful period.
11. Knowledge of, committed to and adherence to Center for Hospice Care concepts, philosophy, policies and procedures and reimbursement methods.
12. Computer literate, ability to learn new data bases, able to operate office equipment.
13. Maintains a current state driver's license.
