Reports directly to the ACC Greenhaven Terrace Maintenance Manager. This is a non-exempt (hourly) position. ESSENTIAL JOB FUNCTIONS: Maintains facilities in a sanitary and infection-free condition through washing, cleaning, and replacement of equipment and furnishings. Operates various mechanized cleaning equipment, such as vacuums, polishers, buffers, etc. Responsible for the disposal of trash, waste, and other disposable materials. Removes trash (including recycling, taking shredding bag to nursing home as required). Follows a printed schedule to complete all task on a daily, weekly monthly or as-needed schedule. Provides housekeeping to individual resident units according to a monthly schedule. Assists with unit turnover tasks such as floor care, cleaning appliances, and washing interior windows. Handles various cleaning solvents, chemicals, etc. Must comply with all government regulations (OSHA, EPA, State Health Department, etc.). Reports any needed repairs immediately to supervisor. Provides daily interior /exterior cleaning of the community room & related common areas. Mops floors with appropriate (germicidal) solution; vacuums floors and edges. Dust mops floor with treated dust mop. Dusts furniture and fixtures, mini-blinds and horizontal dusting. Cleans bathrooms (commodes, sinks, vanity areas, mirrors, etc.). Cleans windows and window tracks. Maintains and cares for all equipment on Housekeeping Cart. Maintains good public relations with residents, program participants and volunteers. Stocks Housekeeping Cart at end of shift. Has a basic understanding of infection control. Has a basic knowledge of chemical usage. Other tasks as assigned. Qualifications QUALIFICATIONS: Must be 18 or older High school diploma or GED required. Vocational training and CPR and First Aid preferred. Effective English communication skills, both verbal and written. Experience in housekeeping activities and use of supplies preferred. Basic knowledge of infection control; understanding of chemical usage and safety body mechanics. Flexibility and willingness to work evenings or weekends as needed. Knowledge and use of proper body mechanics and safety mechanics. Compliance with all OSHA (Occupational Safety and Health Act) regulations is required. Ability to lift and/or carry 50 pounds, move furniture or equipment (with use of proper Personal Protective Equipment and assistance). Ability to communicate in one or more Asian or Pacific Islander languages desired. Some knowledge in maintenance preferred.

The RCM coordinates staffing, resident care, and clinical services in collaboration with the Executive Director, Wellness Nurse, and the Health Services Director. They work closely with the Assisted Living (AL) RCM to ensure streamlined processes across both AL and MC, and serve as backup to the RCM, AL when needed. ESSENTIAL JOB FUNCTIONS: Oversees memory care (MC) residents care plans; monitors changes in health, cognition, or behavior, and ensures proper documentation. Implements person-centered care strategies for residents with dementia and other cognitive impairments. Monitors and manages staffing schedules to ensure resident needs are met; responds to staff call-offs and coverage needs. Serves as primary clinical contact for families, physicians, and other healthcare providers regarding memory care residents. PEARLS is an evidence-based program that helps older adults lead healthier and more fulfilling lives by educating them about depression and supporting them in developing the skills needed for self-sufficiency and increased engagements in social activities. The program is offered in home or community-based settings that are accessible and comfortable for older adults. The ADRC serves as single point of entry into the long-term services and supports (LTSS) system for older adults, people with disabilities, caregivers, veterans, and families under a “no wrong door” policy. ACC is a core partner of the ADRC of Sacramento County with Agency on Aging Area 4 and Resources for Independent Living. The ADRC provides four core services: Enhanced Information and Referral (I&R), Person-Centered Options Counseling, Short-Term Service Coordination, and Person-Centered Transition Support. The position applies social work values, principles, and skills to support older adults and their families. The practice of social work requires knowledge of human development and behavior; social, economic, and cultural institutions; and the ways in which these factors interact. This is a non-exempt (hourly), full-time position and reports to the Program Manager. ESSENTIAL JOB FUNCTIONS: Implements all components of PEARLS; completes required coach certification and training for program delivery including PEARLS coach training, weekly clinical supervision, the California Learning Collaborative, monthly TA calls, and related activities. Meets with participants; provides care via home visits, community, phone or video conference. Conducts outreach to recruit and screen older adults for program eligibility; provide referrals for treatment or additional care options as needed. THE ROLE The Clinical Program Director oversees the agency's clinical programs, ensuring impactful service delivery, programmatic innovation, regulatory compliance, staff development, and alignment with best practices in aging, homelessness, and community building spheres. As a Licensed Clinical Social Worker or other licensed professional, the Clinical Program Director ensures that St. Mary's Center programs support Seniors in becoming and remaining housed, building community, and improving their health and well-being. The position manages manager-level teams in Senior Housing and Mental Health Services, representing approximately 12 professional and paraprofessional staff who work to find, support, and house Seniors; create a supportive community environment that improves physical and mental health; and engage in data collection and analysis to support ongoing improvement in program design and delivery. THE OPPORTUNITY St. Mary's Center has robust, focused program offerings that meet the needs of acutely low-income Seniors for housing security, mental health services, and social supports. The successful candidate will utilize years of experience in direct services, program planning, contract analysis and management, evaluation, and leadership to lead program teams toward dynamic, data-driven programming. Program Planning, Oversight, and Evaluation (50%) Lead the development, implementation, and ongoing refinement of a specialty mental health program tailored to the unique needs of acutely low-income and unhoused Seniors, ensuring alignment with Medi-Cal and behavioral health standards. Ensure that all services and programs are designed and delivered in full compliance with the requirements, goals, objectives, outcomes, and standards of public and private funding sources.
