Qualifications Position Title: Case Manager Location: Colorado Springs, CO (In-person) Employment Type: Full-Time Schedule: 40 hours per week, schedule set by supervisor Job Summary: As an integral part of a multi-disciplinary team, the case manager works collaboratively with the treatment team and serves as a primary point of contact for the client and the client's service providers. The case manager supports the identification of the immediate and ongoing barriers to ongoing access to mental health and substance use treatment. Conducts a functional assessment of both skills and resource related needs to identify targets requiring additional support to enhance treatment as well as reduce the need for the use of higher levels of care. Utilizes client's identified strengths and knowledge base to develop or enhance ongoing stability of pinpointed areas of needs. Under clinical direction, the case manager addresses specific skill and resource deficits and build competencies including but not limited to: age appropriate life skills, conflict resolution skills, self-advocacy skills, housing, access to medical care, and job-seeking. The case manager participates in collaborative meetings with internal and external members of the client's treatment team to ensure continuity in care and link clients with the resources/skills necessary to promote stability. Works in various settings, to include but not limited to: outpatient, home, or community-based environments. Essential Functions: * Completes functional needs assessment to assess both resource barriers as well as skill deficits impacting treatment stability and overall wellness. Contributes to the development of treatment plan based on assessments. * Ability to enhance engagement throughout the client experience with the organization. * Collaborates with clients, internal and external partners as well as families or other supports involved with clients for the purposes of coordinating care. * Facilitates individual and/or group sessions as appropriate to enhance skills and resource access. * Actively evaluates the client's progress and provides feedback to the client and/or client's family and treatment team. * Population served will be dependent upon program assignment and could include offender-based populations, first episode psychosis, inpatient psychiatry, substance abuse clients. * Maintains accurate, concise, and timely documentation of services provided. * Operation of personal vehicle may be required to drive between offices or community sites depending on program assigned. Transport of clients may be required utilizing Diversus Health vehicles. The Environment at Diversus Health is fluid. Roles and responsibilities may be altered to accommodate changing business conditions and objectives. Employees may be asked to perform duties that are outside of the specific work that is listed. This position may require you to work standard hours, as well as flexible hours before and after standard hours, and overtime in excess of 40 hours in a work week. Qualifications Job Summary: The Behavioral Health RN is a member of an inter-disciplinary treatment team that provides inpatient, home and community-based medical/mental health treatment within the scope of the Registered Nurses' Practice Act and/or , under Prescriber orders or ordered protocols. The RN manages psychiatric crisis, carries out written orders from the prescriber, manages patient medication needs including but not limited to pharmacy orders, prior authorizations, assisting with management of medication side effects under prescriber orders, maintaining medication administration records, medication storage areas, providing ordered injections, working collaboratively with Crisis Services, Emergency Rooms, with Community Providers, and providing patient medication and health education. The RN provides care coordination needs through rounds, staffings, external provider communications, and inter-team communications. The RN supports and provides quality client care through effective assessment, treatment planning, supportive therapies, evaluation, triage, and documentation of client care and findings, and working collaboratively with external and internal treatment team members. Essential Functions: * Performs independent nursing assessments accurately documenting findings, communicating critical information, differentiating conditions/symptoms and reporting findings to the treatment team and following up with triaged, appropriate clinical interventions to stabilize and treat clients. * Conducts vital signs, medication reconciliation, check in and check out processes and/or admissions and discharges under ordered protocols, conducts testing and assessments under ordered protocols * Analyzes and documents findings of medical and medication management, performs mental and substance use assessments, collaborates with other team members to conduct comprehensive, consumer assessments and to develop treatment plans. * Provides consumers and families with education on psychotropic medications, related health conditions and monitors medication compliance; supports and provides quality client care through effective assessment, individual and supportive therapies. * Provides direct client care to include but not limited to vital signs, medication injections, basic nursing and hygiene care, EKG, lab specimen collections, medication administration with follow up assessments, crisis care, CPR/ First Aid as clinically indicated and/or ordered by prescriber. * Demonstrates the ability to analyze and document findings for medical and medication management; performs mental health and substance abuse assessments; * Assists in management of psychiatric crisis; prepares written orders from the physician. * Provides concurrent billing and medical/legal documentation meeting Diversus Health time and accuracy requirements. * Performs client care optimizing client safety and risk reduction to include but not limited to reduction in health care errors, safety planning, following safety check protocols as ordered, seeking team collaboration or supervisor input for potentially risk situations, and continuously observing client behaviors, mental status, and activities. * Uses accurate knowledge of Crisis Prevention Intervention (CPI) in safely managing crisis situations with escalated clients and responds according to approved protocols for crisis situations and restraint/seclusion events. Knowledge/Skills/Abilities: * RN Licensure in good standing and with no restrictions in the state of Colorado. * Must have sufficient language skills and be able to communicate both verbally and in writing to accurately and professionally implement and document the services provided to clients. * Possess basic computer skills and type 25+ w.p.m. for the input and output of client information, using electronic record. * Experience working therapeutically with specified population. * Experience using various treatment modalities appropriate for specified population, utilizing strength-based therapeutic approaches. * Experience working in a team environment.
