Key Responsibilities
Your Skills and Duties:
* Work collaboratively to engage the Youth and other support team members in the identification of strengths and needs which support the development and implementation of a service plan.
* Provide weekly case contact and support to assess program compliance, regulatory standards, and progress.
* Conduct assessment and screenings to ensure that identified needs and safety issues are attended to via appropriate and timely referrals, follow-up, and safety planning.
Qualifications
Education & Work Experience
* Bachelors of Arts or Science degree in a related field, 2 years of experience working with children and /or transition age youth and knowledge of the foster care system.
Licenses & Certifications
* Meets all state required conditions of employment as set forth by funder and regulatory body; i.e. Fingerprint and Child Abuse Index Clearance, TB/Health Physical, valid CDL and clean driving record.

Your Impact
The Child Care Worker II is responsible for the care and supervision of children and youth placed at the Receiving Center. Community Solutions, Inc. is seeking a dedicated and passionate Case Manager Supervisor to join our team at our new Multi-Dimensional Anti-Recidivism Partnership (MAP) located in San Bernardino, CA. Adult MAP is an innovative program dedicated to engaging the community and providing rehabilitative programming for justice-involved individuals. We focus on promoting long-term, self-sustaining skills through vocational training and personal development services. This unique model hosts a variety of sub-contractors at one site to reduce barriers for clients. This position will provide general and supervisory case management duties and will ensure community safety and effective program operation through internal and external monitoring of client activities and program services.
SALARY: $68,640 annually
ESSENTIAL FUNCTIONS:
* Perform all duties and responsibilities of case manager, including carrying a caseload as required by program needs, as relates to counseling, case record management, intake assessment, discharge preparation, aftercare services, security, resource management and facility maintenance.
* Supervise Case Manager staff, relating to both security and case management.
* Oversee staff response to all shift problems and emergency incidents.
* Review all client related paperwork, regarding content, timeliness and quality.
* Review all client files on a monthly basis for organization and content.
* Monitor client compliance with treatment stipulations and recommend modifications to program director.
* Monitor staff performance and provide regular feedback to program director on all staff.

* Monitor shift operation for compliance with Agency policy and procedure, applicable standards and relevant contractual policy.
* Monitor staff compliance with all policies and procedures pertaining to the safety and security of clients, Agency property and the community.
* Assist program director in setting agenda and participate in weekly staff meetings.
* Perform the functions of human service worker/case manager such as client supervision, custody, searches, facility rounds, counts, and urine surveillance as needed.
* Perform all duties of program director during periods of his/her absence.
* Participate in on call agency duty officer rotation as assigned.
* Maintain a working knowledge of and abide by the Agency's programs, policies, and procedures.
* Perform other duties as assigned by the supervisor.

We currently have a full-time opening for a Night Counselors at Casa Pacific Residential Center. Casa Pacific is an 11-bed residential program for individuals with mental illness and co-occurring substance use issues. This is an overnight awake position. The program is part of an innovative project which centers on work and meaningful daily activities as the core of the treatment approach, and collaborates closely with Community Connection's Mariposa Center. The Night Counselor is available to work four 10-hour shifts 10:30PM to 8:00AM.
Responsibilities include insuring the safety and well-being of the residents and the facility and support the program operations. Encompass will train you to provide a host of services at our residential mental health and substance use disorder treatment programs.
We are looking for individuals who have direct experience working with individuals who struggle with mental illness as well as people with a passion to serve this population. We are looking for night counselors with the skill and ability to work with clients who have multiple complex health needs and who are able to manage multiple tasks.
DUTIES & RESPONSIBILITIES:
Insure adequate security for program residents and property.
Responsible for dealing with client behavioral and/or medical issues and facility fire, or natural emergencies while on duty.
Medication Passing and Inventory
May provide therapeutic crisis management.
May provide therapeutic intervention services based on best practices & selected evidence based practices for mental health or substance use disorders and/or co-occurring disorders.
Services may include administrative functions in support of clients and their families, evaluation, assessment, counseling, and/or assisting in the implementation of appropriate treatment plan and/or the facilitation of individual, group, family, and crisis counseling.

Qualifications

High school diploma or GED, some college in health and human services or addiction studies preferred (proof of education will be required upon hire)
A minimum of 1 year of full-time experience (or 2080 hours) in Drug and Alcohol Counseling, preferably working in a counseling or treatment setting with clients struggling with substance use, mental health, and criminal justice system involvement
Bilingual in Spanish preferred
Applicant must have active driver's license and the ability to drive to and from the offices in Watsonville and Santa Cruz.

Job Summary:
We are looking for an on-call Mental Health Counselor for the Santa Cruz Downtown Outreach Worker program. Wages are $24.00 -$26.0 per hour, depending on experience, plus $1.00/hour differential for bilingual Spanish competence. Reporting to the DOW Program Manager, the Downtown Outreach Worker will provide services to people in the downtown corridor experiencing homelessness, including those experiencing co-occurring psychiatric illness and substance use disorders. The program supports these individuals in reducing the risk for psychiatric hospitalization and incarceration, while accessing services that can support them on the path to housing. Services include outreach, assessment and linkage to appropriate provider agencies and community resources. The Downtown Outreach Worker will have the knowledge and ability to make independent decisions. Special focus will be on the individual's needs related to housing, health care, and issues of chemical dependency and mental health. Services will be delivered with dignity and respect for each individual and a commitment to their right of choice.
This position is scheduled for 8-hour shifts Monday-Sunday as needed. Position requires availability to cover weekend shifts.
Duties & Responsibilities:
Outreach and engage persons experiencing homelessness in the Santa Cruz downtown corridor, including those with co-occurring mental health and substance use disorders.
Provide crisis support and intervention.
Establish and maintain a working relationship with the downtown merchants and business community, law enforcement, downtown hospitality guides, the Veteran's Administration and city officials.
Develop relationships with, and link target population to appropriate support resources, including drug and alcohol treatment, health care, housing, mental health services, and homeless services.
Provide education regarding homelessness and mental health to the downtown business community.
Transport individuals (who may be malodorous or unkempt) to service providers.
Complete required billing, referral and data documentation in a timely manner as required by the County, State, and agency.
Qualifications:
Bachelor's degree in a related field + 1 years' professional experience working with homeless, mentally ill & substance use disordered populations
OR 5 year's professional experience working with homeless, mentally ill & substance use disordered populations
Knowledge of community resources in Santa Cruz County
Knowledge of homelessness and surrounding issues in this County.
Knowledge of Harm Reduction and a core value of client-centered service provision.
Knowledge of Housing First model
Ability to work in an intense environment dealing with varied personalities and cultures.
Valid CA driver's license and clearance for agency vehicle insurance coverage.
Criminal record clearance
Demonstrated ability to constructively deal with a high degree of physical and emotional stress.

The Multi-Site Residential Counselor is a full-time position with benefits and is scheduled to work Swing and NOC Shifts (including weekends) as needed for coverage at our three mental health residential sites. Counseling duties include ensuring the safety and well-being of our residents, assisting with treatment needs, and supporting program operations. We are looking for individuals who have some experience or education working with individuals who struggle with mental illness and substance use disorders, and people with a passion to serve this population. DUTIES & RESPONSIBILITIES:
Availability to cover needed Swing, NOC, and weekend shifts
Manage client behavioral and/or medical issues, safety issues, or other natural emergencies while on duty, with support of an On-Call Manager.
May provide therapeutic crisis management.
May provide therapeutic intervention services based on best practices & selected evidence based practices for mental health or substance use disorders and/or co-occurring disorders.
Services may include administrative functions in support of clients, including evaluations, assessments, counseling, and/or assisting in the implementation of appropriate treatment plan interventions and/or the facilitation of individual, group, family, and crisis counseling.
Assisting with medication, incident reporting, shift change reporting, and household duties as required.
Must communicate effectively and be able to use current technology in the following areas: e-mail, case notes, incident reports, correspondence with community service providers.
