Position SummaryThe Health, Outreach, Prevention, and Education (HOPE) Case Manager plays a vital role in empowering individuals to achieve optimal health and well-being through community-based support services. This position is responsible for conducting intake assessments, developing personalized care plans, making referrals, re-evaluating progress, and coordinating discharge planning for individuals enrolled in HOPE Services. With a strengths-based, client-centered approach, the HOPE Case Manager ensures services are delivered with compassion and effectiveness, helping clients navigate barriers to care and access critical resources. Additionally, this role is responsible for program implementation, evaluation, and compliance with funder requirements while fostering strong partnerships with key stakeholders to enhance community impact. This position supports individuals across multiple counties, and work hours may be contingent upon grant funding. Education and Experience * Bachelor's Degree in a Human Services related field from a college/university accredited by the US Department of Education required or equivalent HIV-related work experience may be considered with approval from funding source * One (1) year experience in case management, client management, or approved work experience from funding source required * Must have knowledge of and ability to relate to community resources in order to best serve clients * Basic knowledge of HIV, Hepatitis C, and Harm Reduction Principles highly preferred * Knowledge and understanding of Indiana Medicaid, HIP 2.0, and Marketplace coverage highly preferred Certification * May be required to obtain and maintain Outreach Testing certification, OraSure /OraQuick , or other approved testing technology certification within 90 days of hire date. This will be coordinated through Aspire after hire. Qualifications: Required: Education Level - Master's degree with a concentration in human services, counseling, clinical psychology, or social work. To work with the Consulting Psychiatrist and Primary Care Providers collaboratively and within scope of practice regarding psychotropic medication management in an effort to coordinate care for the effective treatment of complex and multiple diagnosis. To meet clinical benchmarks for quality care. To conduct assessments and provide individual and group services to residents requiring additional support as a result of co-occurring disorders and/or trauma. To work in collaboration with the Quality Assurance & Improvement Coordinator and under the direction of the Program Director develop and implement behavioral/mental health Quality Assurance processes as required in compliance with all Federal, State, County, and Funder requirements. To assess professional capabilities of Substance Use Specialists to provide behavioral/mental health and assessment services to Adult and Older Adult residents as directed and provide performance evaluations in compliance with the Gándara Center standards. To conduct case conferences with Substance Use Specialists and other clinical staff weekly. To participate as a member/facilitate the multi-disciplinary team meeting when needed. To fulfill any other duties as assigned. Minimum Qualifications: Master's Degree in one of the following disciplines: Clinical Psychology, Mental Health Counseling, Education-Counseling, Medicine, Psychology, Psychiatric Nursing, Rehabilitative Counseling, Social Work, Human Services or related field required and/or Must have substance use experience.
