Job Description
Overall Responsibility: Administers therapy services to patients requiring intermittent
physical therapy service and teaches and/or supervises the family and other members of
the home health team. These services are performed in accordance with the physician's
orders.
Duties Include:
Responsible for supervising/training personnel such as PTAs or HHAs.
Following receipt of doctor's referral, the therapist is responsible for evaluating
the patient within 72 hours of written referral.
Will provide therapy services as outlined in the Plan of Care.
After evaluating the patient, the therapist is responsible for planning
administering and supervising the PTA and HHA in accordance with the
treatment plan.
Will communicate with the referring physician and member of the rehabilitation
team regarding the patient's total treatment program.
Will maintain accurate daily records of treatments given to patients.
Will provide in-service education to facilities, support staff, students and
community.
Will follow established policies and procedures of Nightingale Home Healthcare.
Charts those services the Therapist has rendered to the patient, notes changes in
the patient' condition and/or family and home situation, makes revisions in the
care plan as needed, records supervisory visits, evaluates patient care and progress
and closes charts of discharged patients by preparing Discharge Summaries and
obtaining Discharge orders.
Maintains confidentiality of client's situation, records and services provided
according to HIPPA regulations
Evaluates the effectiveness of his/her therapy service to the individual and family.
Consults with the attending physician concerning alterations of the plan of
treatment.
Submits all patient notes and/or visit records on a timely basis.
Discusses with the Director of Clinical Services problems concerning the patients
and how these may best be resolved.
Discusses with the Director of Clinical Services changes in patient needs and how
these can be best met.
Discusses with the Director of Clinical Services the need for involvement of other
members of health team.
Obtains orders for paramedical services and submits referral to the Director of
Clinical Services, i.e., nursing services, medical social services, etc.
Coordinates total patient care.
Cooperates with other agencies providing nursing or related services to provide
continuation of care and to implement a comprehensive care plan.

Continually strives to improve therapy care by broadening and deepening
knowledge through formal education, attendance at workshops, conferences and
active participation in professional and related organizations and reading.
Participates in interdisciplinary team conferences.
Attends on-the-job and in-service training.
Qualifications
Education:
Licensed Physical Therapist in the state of California.
Experience:
Physical therapy experience preferred

We are seeking a dedicated Hospice Nurse Practitioner to perform Face-to-Face (F2F) encounters essential for hospice eligibility certification and recertification. In this role, you will gather comprehensive clinical information, perform detailed assessments, and collaborate closely with hospice physicians and interdisciplinary teams to ensure quality patient care and regulatory compliance.
Key Responsibilities:
+ Perform and complete physical assessments for hospice F2F encounters within CMS-established regulatory timeframes.
+ Document clinical findings accurately and attest F2F encounters in patient medical records.
+ Utilize Local Coverage Determination (LCD) criteria per hospice policy and regulatory requirements.
+ Communicate clinical findings, review treatment plans, lab results, diagnostics, and medications with certifying physicians.
+ Collaborate effectively with physicians, interdisciplinary group (IDG) members, patients, caregivers, and families.
+ Provide clinical recommendations to the IDG based on F2F assessment findings.
+ Conduct assessments in the patient's place of residence, including home, skilled nursing, or other settings.
+ Notify the hospice agency promptly if unable to complete F2F visits as scheduled.
+ May provide F2F coverage for multiple provider numbers with approval.
About You
Qualifications - What You'll Bring:
+ Master's or higher degree in Nursing
+ Completion of an accredited graduate-level nurse practitioner education program
+ Current registered nurse license in state of practice
+ Current advanced practice nurse license or certification
+ National Nurse Practitioner certification (ANCC or AANP)
+ Collaborative agreement in place as required
+ Current CPR certification
+ Valid driver's license and automobile insurance
+ Strong understanding of hospice care principles and terminally ill patient support systems
+ Excellent patient assessment and clinical evaluation skills
+ Familiarity with LCD criteria and hospice documentation requirements
+ Knowledge of Medicare, Medicaid, ACHC, and other regulatory guidelines
+ Strong oral and written communication skills
+ Compassionate and sensitive to end-of-life issues impacting patients and families
+ Ability to provide emotional support during times of crisis
+ Flexible and adaptable with ability to manage diverse tasks efficiently
+ Effective communicator across diverse socioeconomic and cultural backgrounds
+ Cooperative and tactful team member adhering to OSHA, state, and federal hospice regulations
Preferred (Not Required):
+ Experience in acute care, hospital, oncology, home health, skilled nursing, or hospice settings

Job Description
Overall Responsibility: Administers therapy services to patients requiring intermittent
physical therapy service and teaches and/or supervises the family and other members of
the home health team. These services are performed in accordance with the physician's
orders.
Duties Include:
Responsible for supervising/training personnel such as PTAs or HHAs.
Following receipt of doctor's referral, the therapist is responsible for evaluating
the patient within 72 hours of written referral.
Will provide therapy services as outlined in the Plan of Care.
After evaluating the patient, the therapist is responsible for planning
administering and supervising the PTA and HHA in accordance with the
treatment plan.
Will communicate with the referring physician and member of the rehabilitation
team regarding the patient's total treatment program.
Will maintain accurate daily records of treatments given to patients.
Will provide in-service education to facilities, support staff, students and
community.
Will follow established policies and procedures of Nightingale Home Healthcare.
Charts those services the Therapist has rendered to the patient, notes changes in
the patient' condition and/or family and home situation, makes revisions in the
care plan as needed, records supervisory visits, evaluates patient care and progress
and closes charts of discharged patients by preparing Discharge Summaries and
obtaining Discharge orders.
Maintains confidentiality of client's situation, records and services provided
according to HIPPA regulations
Evaluates the effectiveness of his/her therapy service to the individual and family.
Consults with the attending physician concerning alterations of the plan of
treatment.
Submits all patient notes and/or visit records on a timely basis.
Discusses with the Director of Clinical Services problems concerning the patients
and how these may best be resolved.
Discusses with the Director of Clinical Services changes in patient needs and how
these can be best met.
Discusses with the Director of Clinical Services the need for involvement of other
members of health team.
Obtains orders for paramedical services and submits referral to the Director of
Clinical Services, i.e., nursing services, medical social services, etc.
Coordinates total patient care.
Cooperates with other agencies providing nursing or related services to provide
continuation of care and to implement a comprehensive care plan.

Continually strives to improve therapy care by broadening and deepening
knowledge through formal education, attendance at workshops, conferences and
active participation in professional and related organizations and reading.
Participates in interdisciplinary team conferences.
Attends on-the-job and in-service training.
Qualifications
Education:
Licensed Physical Therapist in the state of California.
Experience:
Physical therapy experience preferred
Additional Requirements:
Physical within 6 months prior to first patient contact
Current TB results (within last 12 months)
Current CPR Certification
Must have valid driver's license
