Role Summary
As a Clinical Health Coach, your role is central to SCM's clinical efforts, collectively designed to deploy best in class clinical methodologies and coaching methods to improve member health. You'll be interacting with members at all stages of kidney disease: pre-disease (addressing common co-morbidities); CKD Stages 1-5; ESRD. A Clinical Health Coach has strong assessment skills, a firm command of the nursing process and a passion for empowering members to improve their self-care as the means to identify and reduce their risk for ESRD.
Clinical Health Coaches make proactive outreach and regular follow-up calls to members who carry complex (and often multi-morbid pathophysiology) with compassion and effectiveness. Our RNs are expected to work to the top of their licensure for the benefit of members, families, and employer groups served by our kidney disease management programs. In addition, you'll act as the clinical expert to support Sales and Operations teams from time-to-time.
How You'll Contribute
Possess a firm command of nursing processes and work full-circle with each member:
Obtain member health assessment and history via telephone or secure video chat using motivational interviewing and a coaching framework.
Gather and utilize pertinent medical records including laboratory data and communicate discrepancies back to MD/providers.
Utilize industry guidelines to provide and ensure high-quality and appropriate care.
Utilize tools/formulas to calculate key metrics and facilitate risk identification (GFR, KFRS, UPCR-UACR, etc.)
Develop care plans and evaluations to ensure your members are on the right track.
Develop good relationships with providers, family/support persons, and all members of the care team to provide education and support.
Ensure timely nephrology, community and hospice referrals, optimal start for dialysis/avoidance of emergency start dialysis, and be able to identify and communicate variance in guidelines associated with kidney and dialysis care.
Facilitate and coordinate care with other providers and large case management.
Perform chart reviews and contribute to continuous quality improvement and ongoing program development.
Provide Medicare advocacy and education for members with ESRD.
Perform claims reviews and eligibility verification of case load each month.
Track key personal performance metrics each quarter and maintain a minimum of 35% member participation rates with assigned case load for our CKD management program.

An innovative premise.
Located in Flint, Michigan, the Charles Stewart Mott Department of Public Health seeks to build on strong and energetic growth in research. Flint has a long history of community activism and involvement. It is one of the birthplaces of community-based participatory research (CBPR) and includes nationally recognized community pioneers and citizen scientists. For example, the first community representative to be President of the American Public Health Association in its 100+ year history, beginning her term in 2023, is a member of the Flint community. In 2011, the Flint community (including Flint-area hospitals) approached MSU with a proposal to create an academic Department of Public Health in Flint. Their idea was that the department would improve health through community-identified public health solutions, provide an economic driver for Flint, and lead the nation in health equity informed policy change. MSU agreed and took an unprecedented community-participatory approach, building the Flint community into the Department and its governance, including focus areas, priorities, and faculty to hire. Through 1000+ surveys and more than 100 interviews, community members identified the top public health needs of Flint, providing the areas of focus for the new unit, which began as a Division in 2015. Community members make up and continue to make up much of the faculty search committee tasked with hiring the best public health researchers in the country to address these issues.
This radical experiment in community-partnered departmental administration has been wildly successful both in funding and in real-world impact. Departmental faculty have obtained over $175 million in extramural funding. Among its many projects, the Department currently includes two NIH-funded Centers (one in maternal health equity and the other in suicide prevention) and the first city-wide unconditional and universal cash prescription program for pregnant/postpartum people and infants. ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Maintain close contact with the student's teachers, counselor, or the program coordinator to receive and implement appropriate educational programs.
Deliver assignments and instruct the student as necessary.
Be present for all testing except those tests that are designated as take-home.
Regularly report academic progress to parents, teachers, and school counselors.
Maintain an accurate record of the hours of instruction provided for each assigned student and file such information with the Office of Testing, Accountability, and Alt. Ed.
Reports missed sessions to Student Services Assistant (missed by student or teacher) and include this information on the Homebound/Home-based timesheet.
Submit timesheets to the Student Services Assistant at Abrams Academy by the designated deadline.

Submit the student's completed work to the designated school representative in a timely manner.

Rate of Pay:
The anticipated rate is $650/ELU for non-terminally prepared adjunct instructors (term-by-term) and $750/ELU for terminally prepared adjunct instructors (term-by-term). Graduate-level courses may be paid at a higher rate. A typical 4-credit course is loaded at 4 ELU.
Full-Time-Equivalency:
Typically, less than 7.5 ELU, but up to 15 ELU per term or 45 ELU per year for 1.0 full-time equivalence.
POSITION DESCRIPTION:
Individuals appointed from this pool will have the primary responsibility to teach courses within Science including:
Biology - Adjuncts teach undergraduate biology courses, primarily laboratory sections of non-major courses in general biology, and human anatomy and physiology. Chemistry - Adjuncts teach undergraduate chemistry courses, primarily laboratories and non-major lecture courses. Specific topics include those typically covered in environmental, general, and organic chemistry courses. In your cover letter, please address your areas of expertise. Environmental Science, Policy & Sustainability - SOU offers a Bachelor of Science in both Sustainability and Environmental Science & Policy that prepares students for a wide variety of employment in such organizations as governmental agencies, industry, non-profits, and environmental consulting firms. A BS in Environmental Science & Policy prepares students for graduate and professional schools in areas such as environmental policy and management, ecology, geology, law, public policy, and environmental science. The program integrates natural sciences, physical sciences, geospatial sciences, and social sciences to address complex environmental issues. Adjunct Assistant Professor:
Initial appointment at the rank of Adjunct Assistant Professor requires a terminal degree in the discipline taught or a related field; OR a Master's degree in the discipline taught, or a related field, and is in the process of completing the appropriate terminal degree.

Duties (100%)
The primary responsibility of a faculty member is teaching. Teaching encompasses normal instructional activities beyond the teaching of assigned courses such as academic advising, working with students outside of class, posting and maintaining appropriate office hours, course and curriculum planning, etc.
Efforts associated with teaching may include, but are not limited to: preparation; direct instruction; assessment; student contact outside of class (office hours, appointments, email); presenting complex concepts to a group of diverse learners; listening; understanding and responding to student questions in real-time/in a classroom environment; and applying critical judgment to student work, both written and verbal delivery of information.
Efforts associated with lab/workshop may include: providing students with practical experience using instrumentation and equipment in a way that enables student learning and ensures safety; simultaneously monitoring and managing multiple students individually and in groups; demonstrating and executing complex procedures; and using complex instrumentation and hazardous materials.
Incumbents appointed to multiple terms of teaching may require professional development and service.
Professional Development includes maintaining currency in the discipline taught.
Service includes program and institutional administrative, committee, and student support and retention activities.
In addition to teaching and service, the primary responsibilities at the professorial level include participation in scholarly and/or creative activities, including formal research.

Activities may involve standing, sitting, working at a computer, and lecturing.

Teaching assignments may include First Year Seminar, online and/or hybrid courses, Medford and Ashland campus classes, and day, evening and weekend courses.
Must be willing to travel and attend training programs off-site for occasional professional development.
Must be able to work additional hours and adjust working hours to meet special jobs. May be called back periodically to perform work as needed on an emergency basis.
Must be able to successfully pass a pre-employment background check.
