+ Assist Unit Leader in staffing and scheduling decisions, including: controlling overtime, coordinating meals and breaks, etc.
+ Round with purpose on each patient each shift
+ Communicate and round effectively with physicians and patient families to resolves difficult situations.
+ Work with Care Coordinators for safe and timely discharge of patients.
+ Support interdepartmental delivery of care to optimize outcomes; correlates nursing competencies with patient needs and assigns staff

The Clinical Manager (Internal; Clinical Team Leader) is responsible for general management, supervision and leadership of a hospice transdisciplinary team. Demonstrates knowledge of and commitment to organization's mission, culture, fiscal, customer service and management principles. Assists in staff development and educational programs for the team.
Assists in the coordination of volunteers and volunteer training.
Oversees continuing compliance with internal and external regulatory standards including Medicare, Medicaid, licensure, JCAHO, corporate compliance and agency policies/procedures.
Possesses a thorough understanding of Medicare hospice regulations and other regulatory hospice requirements as it relates internally and to all external relationships
Participates as required in the QA process pertinent to hospice service; focus on Quality Scores and Public Reporting
Participates in public relations, community outreach and promotion of HWR and Advisory Councils
Assists with all survey preparation and coordination
Collaboration with Centralized Medical Records for oversight of clinical records
Demonstrates fiscal responsibility for the team budget through the judicious allocation and monitoring use of resources. Review variance reports and maintain Compass standards
Demonstrates and promotes positive customer service responsiveness internally and externally according to agency standards
Participates in on-call as assigned.

**Responsibilities And Duties:**
50% Patient Care:
1. Addresses concerns and resolves problems. Uses data from various sources to initiate continuous quality improvement within the department/unit. Participates in process improvement activities and root cause analysis investigations. Assists the Manager with fiscal responsibility at the unit level.

3. Assists Manager with planning, reviewing and coordinating staffing time schedules and allocating staff as appropriate for volume and patient care needs. Assists Manager in daily staffing plans.

25% Operations and Personnel Management:
1. Maintains daily unit operations including the status of staffing, patient visits and/or admissions, discharges and transfers, serving as a resource to department/unit staff to guide patient care delivery.
2. Participates in recruitment, selection, retention and evaluation of personnel. Participates in staff performance via written performance appraisals and disciplinary procedures. Ensures appropriate orientation, training, competence, continuing education, and professional growth and development of personnel. Maintains staff records.
3. Assists manager in planning and contributing to fiscal management of unit by utilizing human and material resources and supplies in an efficient, cost effective manner. Assists Manager in development and implementation of services.
15% Professional Development and Leadership:
1. Practices as colleague with medical staff, other members of the interdisciplinary team, and other disciplines to initiate and support collaborative and cooperative clinical management practices. Actively participates in interdepartmental relationship building.
