Registered Nurse Care Manager
SUMMARY: The RN Care Manager is responsible for assessing the care needs of participants, provides nursing and healthcare interventions, and evaluates outcomes of care of participants on an ongoing basis. In collaboration with the interdisciplinary team (IDT), develops plans of care to meet participants' needs. Delegates tasks to clinic, center, and homecare aides according to participant needs and care plans. Collaborates and communicates with the primary care provider, clinic staff, and other members of the IDT. Provides care to participants in the clinic, center, and participant homes as needed.
ESSESNTIAL DUTIES AND RESPONSIBILITIES:
Provide high quality clinical care and serves as a member of the PACE interdisciplinary team (IDT).
Provide nursing care in the center, clinic, contracted facilities, and participants' homes according to each participant's plan of care. (NJ: in accordance with the State of New Jersey Nursing Practice Act, N.J.S.A. 45:11-23 et seq., as interpreted by the New Jersey State Board of Nursing, and written job descriptions. Services provided shall be documented in the participant's medical record).
Participate in 24/7 “on-call” process for triage of participants and their needs.
Assess, plan, and coordinate participants' home care services. Provide input to the IDT in developing home care plan interventions. The nursing care needs of the participant shall be assessed only by a registered professional nurse.
Monitor participants' acute and chronic care needs in all settings. Provide coordination and direct care as indicated to promote continued care in the community or promote optimal institutional care (Assisted Living, Nursing Home, Hospital, etc.) as needed.
Ensure timely follow-up by providers on specialist visits and will assist with obtaining specialist reports, facility documentation, and labs if needed.
Reconcile facility MARs for your assigned panel of participants monthly to ensure accuracy and medication adherence, notify provider of any discrepancies.
Notify participants of normal test results.
Complete timely and accurate nursing assessments in accordance with policies and regulatory requirements.
Implement nursing-related care plan interventions.
Teach participants, caregivers and families about self-care, medications, healthy lifestyles, infection control and safety to promote optimal health and safety.
Review and revises goals and approaches to participants' care in coordination with participant, family, caregiver and interdisciplinary team.
Works collaboratively with the interdisciplinary team (IDT) to develop and implement comprehensive plans of care for participants.
Develop and maintain positive relationships and communication with co-workers, participants and their families/significant others, and members of the community.
Participate in all interdisciplinary team meetings.
Assist the interdisciplinary team members in understanding the significant nursing, self-care and functional needs related to the participant's health problems.
Supports OT as aback up to performing the duties of Home Care Coordinator on the IDT as needed/when assigned. May perform the duties of other IDT members based on professional licensing, competencies, and experience as needed
Actively participates in utilization review meetings and quality improvement projects / meetings.
Evaluates the competence of CNAs and Home Care Aides and delegates tasks and duties to them as indicated.
Participates in family meetings, staff meetings, in-service and training and orientation programs as required.
Follows all PACE Program Policies and Procedures and Occupational Safety and Health Administration (OSHA) safety guidelines.
Protects privacy and maintains confidentiality of all company procedures and information about employees, participants and families.
Practices standard precautions and follows PACE Program Infection Control protocols.
Performs other duties as required or requested.
EXPERIENCE, EDUCATION AND CERTIFICATIONS:
Bachelor of Science in Nursing Degree preferred.
State RN License required
**NJ: Licensed by the New Jersey State Board of Nursing.
BLS required (have within90 days of employment).
1 year of experience working with a frail or elderly population preferred. If this is not present, training will be provided upon hiring (If applicable for the role).
Experience in home care, long-term care and /or managed care preferred.
1 year experience providing care as an RN required.
PRE-EMPLOYMENT REQUIREMENTS:
Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
Be medically cleared for communicable diseases and have all immunizations up to date before engaging in direct participant contact.

Job Summary
The Care Manager - Nursing Field (RN CM) will ensure that all members receive timely care management (CM) across the continuum, including transitions of care, care coordination and navigation, complex case management, population health and wellness interventions, and disease/chronic condition management per department guidelines. Job Description
Key Responsibilities/Duties - what you will be doing
Perform telephonic member outreach and/or face-to-face encounter utilizing key motivational interviewing skills to facilitate program enrollment. Perform departmental assessments and evaluate member holistically to identify needs, health goals, and barriers to wellness. Through assessment and collaboration with member/caregiver and providers, develop a member-specific plan of care, implement member-specific care manager interventions, and revise plan of care as needed. Complete documentation in applicable platform according to departmental policy and regulatory standards.
Provide targeted health education, proactive strategies for condition management, and communication with key providers and vendors actively involved in the member's care.
Collaborate with member/caregiver and the facility care team to coordinate a safe transition to the next level of care, which includes but is not limited to ensure understanding post-hospital discharge instructions, facilitate needed services and follow-up, and implement strategies to prevent re-admission.
Collaborates and liaises with the interdisciplinary care team, to improve member outcomes (i.e., Utilization Management, Medical Director, pharmacy, community health workers, dementia care specialists, wellness, and Behavioral Health CM). Attending and presenting (as appropriate) high risk members at interdisciplinary rounds forum.
Maintain professional growth and development through self-directed learning activities.

WHAT LEADS TO SUCCESS

This position is full-time, 8am-5pm, working from home, with in person visits as well.
Care Manager Responsibilities:
Assists individuals and their families with personal or environmental needs through care coordination, navigating care systems, planning for care transitions, or discharge to next level of care.
Ensures and facilitates access to community resources, such as transportation, insurance/supplemental income sources, care giving, food, etc.
Facilitates individual and family meetings for the purposes of goal setting, advance care planning, coordination of community supports, and plan of care development, based on the individual's needs, values and preferences.
Collaborates with other PHC and Ohio's Hospice divisions as appropriate to improve communication of needs; if to outpatient support center, coordinates plan of care for follow up and instructions on AVS; if hospice care is desired, initiates referral process.
Coordinates plan of care between settings (home, hospital/inpatient, outpatient services, etc) to ensure continuity of care and ability to obtain necessary medical treatment, equipment, and physical/emotional needs.
Documents assessment, plan and interactions with individual, family, community resources, hospital or facility staff, and physicians and establishes/builds account in electronic medical record.
Participates in the development of processes to improve the clinical experience for referral sources, individuals/families, care management teams, etc. across the continuum of care.
Regularly attends IDT meetings or other multidisciplinary team meetings based on Individual/family needs or organizational needs.
Fosters collaborative working relationships within division, organization, hospitals, and community facilities and/or with groups with whom we are engaged.
Attendance at division meetings 80%; accountability for 100% of information provided via minutes.
Annual education completed by deadline to ensure staff has the information and tools to complete their job satisfactorily and meet regulatory compliance.
Adheres to all applicable federal and state regulations, including HIPAA, compliance, and employment regulations.
Maintains strong knowledge of and adherence to NCQA standards for Complex Care Management
Care Manager Qualifications:
Active Registered Nurse license in the State of Ohio without any board actions.
Three to Five years clinical experience in Case Management, Care Coordination, Discharge Planning, Medicaid and/or Medicare managed care, LTC experience and/or community health. Mental Health Experience is a plus.
CM certification preferred.
Notary designation or intention to complete within six months of hire.
CPR Certified preferred, required in orientation at hire.
Computer skills sufficient to properly document services and care.
Ability to drive during daytime, nighttime, or inclement weather. This position requires travel to different work locations and within the counties we serve.
Valid Driver's License with Safe Driving Record with successful clearance of a driving record check
Successful clearance of a criminal background check

Key Responsibilities:
Manage a caseload of patients longitudinally, tracking health status, progress, and care needs.
