As a Human Resource (HR) Coordinator, here's a big-picture view of what you'll do:

JOB DUTIES
Serving as the primary contact for the Department for I&A services development, provision, and promotion
Managing Member/Authorized Representative referral process for I&A
Training or facilitating I&A staff onboarding and training
Providing I&A to Members/Authorized Representatives with high needs or complex needs not quarterly
Coordinating with the Program Lead on I&A staff recruitment
Receive and process I&A referrals from Members or other community resources and follow-up appropriately by making initial contact, overview of I&A services, and establishing goals
Communicate with appropriate entities, as necessary and appropriate, about the progress of Member or Authorized
Representative's plan and goals
Review training curriculum documents for accuracy and completion while communicating with clients or Authorized
Representatives with any needed adjustments and/or submission to case manager for final approval

Conduct state-wide I&A training within the established requirements

QUALIFICATIONS
At least two years of experience with Consumer Direction is required

At least two years of experience with using common technology, such as smartphones, basic spreadsheets, email, and word processing software

Responsibilities:
This position may work with multiple clients throughout the day providing outstanding service
Caregiver shifts range from Sunday - Saturday
You will help your client maintain their independence, dignity and respect by assisting them with any personal care needs
Support clients with their mobility needs, aiding in transfers and ambulation
Facilitate engagement in activities that enhance physical and emotional well-being
Assist in meal preparation, ensuring dietary needs are met, and provide help with eating when required
Perform light housekeeping to maintain a clean and safe living environment
Provide companionship, fostering a positive and supportive relationship with clients
Accompany clients to medical appointments and other outings, ensuring their safety and well-being outside the home
Adhere strictly to all healthcare regulations and organizational policies
Experience in caregiving or a related field preferred but not required
Ability to adapt to the individual needs and preferences of clients
Complete State required training undergo training and continuous education as 12 hrs is required by the state yearly
Candidate requirements:
High school diploma or equivalent
Must be at least 18 years of age
Ability to read and speak English
Establishes productive organizational skills
