Job Duties:
Assists Veterans/Members with transfers, ambulation, eating, bathing, dressing, toileting, personal hygiene and other ADLs
Follows instructions to use simple procedures and devices to strengthen Veterans'/Members' limbs and maintain flexibility
Assists nursing staff with simple medical procedures such as suppositories, enemas, treatments, post-surgical care and post-mortem care
Assists in monitoring physical and mental status of Veterans/Members by utilizing appropriate medical equipment and techniques to take vital signs (temperature, pulse, respirations); obtain urine, sputum or stool samples; and track input/output
Responsible for medical record and worksheet documentation on all assigned Veterans/Members. Keeps records and prepares simple reports.
Assists Veterans/Members with dining to include transporting to the dining room, informing them about menu choices available for the day, communicating dining staff about Veterans'/Members' choices and serves Veterans/Members their meals in courses, assisting them if necessary
Provides resident care and comfort by responding to patient requests (verbal or call light) for assistance timely in accordance with the Resident Bill of Rights
Maintains cleanliness of areas neat and sanitary by changing bedding, linens and other items
Reports to and assists licensed nurses in emergency situations rapidly and appropriately following established policies and procedures
Performs other related duties such as transporting Veterans/Members to appointments outside the facility and clerical duties (e.g., answering phones, filing, etc.) as needed
Actively involved in AMS processes such Huddle Boards, Problem Solving, Job Breakdown Sheets, Success Tracking, and Standardized Work
Participates fully in the QAPI process, our Quality Assurance and Performance Improvement program mandate by CMS which incorporates AMS
Knowledge, Skills & Abilities (KSAs):
Knowledge of:
Principles, practices and techniques of para-professional nursing care, treatment and services appropriate to the work setting
State and agency laws and regulations governing para-professional nursing practices

Current Negative TB skin test (Mantoux skin test) within the last twelve months or a written statement from a physician, physician's assistant or a registered nurse practitioner indicating freedom from Tuberculosis, if in the past has had a positive skin test or Tuberculosis
Must be able to obtain and retain a fingerprint clearance card issued by the Arizona Department of Public Safety
Current CPR card that was obtained by physical assessment

works under the general direction of the
Day Center Lead.
Responsibilities include but are not limited to providing participants with assistance with personal care, and assisting with center activities and meal service.
Qualifications:
High School Diploma/GED and CPR certification required. Current Certified Nursing Assistant License preferred. Minimum one-year experience with older adults required. Strong interpersonal skills with the ability to handle unusual situations.
Essential Functions:
Assist participants with basic activities of daily living including but not limited to bathing, dressing, incontinent care, toileting, etc. as assigned.
Assist participant with transfers and mobility with or without assistive devices.
Assist with oral feedings of the participants.
Assist with center activities and or restorative therapies as needed.
Serve snacks and meals and obtain Serve Safe Handler certification.
Participate in training, in-service education and required Center-based meetings.
Assist in maintaining the cleanliness of the facility.
Assist in the transportation of participants for facility outings.
Report changes in a participant's condition to the Day Center Lead
Communicate with PACE team members.
Participate in training and in-service education programs.
Assist patients in their homes with personal care needs on an as needed basis.
Complete other tasks as assigned by supervisor.
Working Conditions:
Perform participant transfers regularly with the use of mechanical devices as indicated.
Work with participants with varying cognitive levels, physical and sensory impairments, limited social supports, declining health and at end-of-life.
Communicate and interact with PACE center staff, participants' caregivers and family members on daily basis
Prepare for possibility of unexpected crisis situation to occur.
