At Athletico, the PhysicalTherapist's role is to evaluate, design and implement a patient's plan of care. Each Plan of Care is created to increase, maintain or restore function, minimize or prevent disability, and assist patients in reaching their maximum performance levels.
Essential Duties and Responsibilities:
* Providing interpretation of prescriptions, performing an initial evaluation, and establishing a treatment plan with functional and/or work related goals
* Appropriately progressing the treatment and goals
* Periodic and timely written communication to physicians
* Appropriate discharge management and patient follow‐up
* Supervision of rehabilitation aides and administrative staff as needed
* Supervision of PTAs when applicable per the appropriate state practice act
* Documentation: All PT's are responsible for documenting all treatment and completing all written communications in a timely manner, including but not limited to, initial evaluations, progress notes, discharge summaries, daily S.O.A.P. notes, functional status reports, functional capacity evaluations and insurance/plan of care forms as necessary

* Degree in Physical Therapy from an accredited school

Responsibilities
* In-Home Staff PhysicalTherapist evaluates, plans, and administers physical therapy treatment for patients suffering from injuries, muscle, nerve, joint, and bone disorders or diagnoses to restore function, relieve pain, and prevent disability.
* Travel is required to different home care settings, between clinics, doctor's offices, and to patient's homes.
* Evaluates the home environment for rehab and make appropriate recommendations to increase the independence of the client.
* Develop, prepare, and maintain individualized client care progress records with accuracy, timeliness, and according to agency policy and procedures.
* Supervise physical therapy assistants by directing, participating in, and presenting continuing education programs; and assume administrative/supervisory responsibilities in the absence of the director.
* Educate client/caregiver and health care teams in the phases of physical therapy in which they may assist the client, in the use of prosthetic and orthotic devices and durable medical equipment, as appropriate.
Qualifications
* Licensed physicaltherapist in the state of employment.
* Minimum two years of experience as a physicaltherapist.
* Automobile liability insurance and current unrestricted driver's license.
* CPR certification or complete training required.
