Job DescriptionSalary: $12.50 hourly MA Job Purpose: A combination of clerical and administrative duties as a Medical Assistant. Welcomes patients and visitors by greeting patients and visitors, in person or on the telephone; answering or referring inquiries; Optimizes patients' satisfaction, provider time, and treatment room utilization by scheduling appointments in person or by telephone. Helps patients in distress by responding to emergencies; Protects patients' rights by maintaining the confidentiality of personal and financial information; Maintains operations by following policies and procedures and reporting needed changes. The following inquiries by consumers are those that can be assisted with by MA : Provide information to consumers that are found on EHR, such as: Consumer demographics Appointment information Status on prescription sent to the pharmacy Pharmacy on file for consumer. The outpatient building includes three main client-serving programs including a mental health outpatient clinic, a medication-assisted treatment clinic, and a substance use disorder outpatient program. In addition to the administrative offices, this location also serves as the base for the outreach team. Assist and guide participants towards the identification and achievement of specific goals defined by the peer in the treatment/recovery plan. Peer specialists promote community socialization, recovery, self-advocacy, self-help, and development of natural support. Assist in the orientation process for peers receiving mental health or co-occurring disorder services. Educate and engage individuals in the Wellness Recovery Action Plan process as a means to recognize triggers and signs of relapse. Accompany peers as they transition to community integration and continue the process towards their recovery goals. Provide education and advocacy in the community to promote awareness of mental health disorders while recognizing both the successes and challenges the disorders can present. Participate in treatment team discussions about individuals served and provide insight to the team about strengths, accomplishments, and obstacles in the recovery process. Complete all required documentation in a timely, legible manner. Utilize a nonjudgmental approach, effective listening, good eye contact, and positive interactions with peers. Provide instruction and support to peers in the development of coping skills. Interact with every peer on the crisis residential unit. Assist peers on the crisis residential unit with their discharge plan and reintegration into the community. Assist peers in residential substance use treatment with their discharge plan and reintegration into the community. Be available by phone to provide peer support. Follow up with peers after discharge and maintain contact and support. Initiate and maintain chart documentation on each peer. Develop a working knowledge of housing options and resources and increase collaborative relationships with staff connected with each option. Maintain a valid Texas driver's license and vehicle liability insurance. Transport peers to medical appointments, job sites, meetings, social activities, discharge locations, and other places as needed. Participate in in-service programs and attend all annual trainings in a timely manner. Be prepared to therapeutically intervene in potential crisis situations. Homeward Bound, a mental health and substance use facility for adults, is hiring a Qualified Mental Health Professional to work on the crisis residential unit where clients reside for 7 days or more while they get stabilized due to being severely depressed or suicidal, homicidal towards specific individuals, or psychotic without the acuity of needing a locked down facility. Candidates must have a minimum of a bachelor's level education in psychology, counseling, sociology, social work, or related field. Job duties will include Complete an initial assessment on each client within 24 hours of arrival on the crisis unit. Assist clients with all case management needs including, but not limited to, appropriate housing upon discharge, connection with positive social support, and coordination with necessary resources. Coordinate client care between crisis residential and mental health outpatient and other aftercare providers. Complete intake documentation with clients including financial information. Prepare clients in crisis residential for discharge and work with the treatment team to eliminate barriers to success. Create person-centered treatment plans with individualized goals, objectives, and strategies. Assist in direct client care on the unit including observation, checking in and out belongings, facilitating client phone calls, and attending meals. Facilitate psychoeducational groups daily and complete documentation. This position is primarily responsible to contact persons referred for services within 24 hours. Implements strictly FSLA no wait time policy. Conducts initial intake activities according to FSLA protocol. Conducts a thorough assessment and completes a Diagnostic Assessment report. The assessment must reflect person-centered, culturally sensitive and trauma informed clinical evaluation; relevant current and historical clinical information including lethality and propensity for violence. The assessment must include an evaluation of the client's mental status; client's strengths, needs, abilities, and preferences. The assessment includes the functioning level including risk and protective factors. Determines the most appropriate level and intensity of service(s)/treatment that is medically necessary. Otherwise, the Bilingual Licensed Intake Coordinator identifies the need for referral (e.g. SUD treatment), further assessment (e.g. r/o developmental disability accounting for the symptoms) or make a statement that no treatment is clinically recommended. Formulates a diagnostic impression based on DSM-5 diagnostic criteria. Primarily responsible for making initial contact with the school system, primary care physicians, previous mental health providers and other care/service providers. Gathers other documents including legal such as guardianship and advance directive unless such activities have been delegated to other FSLA service provider to facilitate the intake process and maintain smooth flow of operation. Completes an individualized treatment plan (ITP) as soon as possible but no later than 7 days. Services “prescribed” must meet the definition of “medically necessary services” based on applicable regulatory guidelines. All ISPs must be completed with the active participation of the person served. ISPs are person-centered, strength-based, focused on the defined medical needs and preferences of the person/client. Treatment planning is conducted in a culturally and trauma-sensitive way. All activities must be medically permissible according to applicable rules and regulations. Diagnostic Assessment and Individualized Treatment Plan are completed according to the FSLA protocol. Submits properly completed and signed Diagnostic Assessment report and Individualized Treatment Plan (ITP) for review of the Clinical Supervisor within 24 hours. Primarily responsible for coordinating medically necessary “medication evaluation/management appointments” unless specifically delegated to another FSLA service provider to facilitate and promote coordinated care outside of the FSLA system of care. Takes lead role in care coordination if within the scope of practice and as privileged by the clinical supervisor. ISP must be approved by the supervisor prior to care coordination. Submits completed client record to the Administrative Assistant to be scanned into the electronic health record. Original hard copies are maintained by the Medical Records Department. Maintains a working knowledge of significant developments and trends in the field pertaining to job duties and responsibilities (e.g. assessment tools, evidence-based interviewing techniques, trauma-informed assessment, etc.) May occasionally provide outpatient therapy, crisis intervention, Community Psychiatric Support Services (CPST), Therapeutic Behavioral Services (TBS), Psychosocial Rehabilitation (PSR) and other services as directed by the Clinical Supervisor. The Bilingual Licensed Intake Coordinator will utilize only generally accepted interviewing techniques that are researched/evidence-based or best practice models and permissible under the CMS and other regulatory guidelines.
