The primary purpose of your job position is to direct the overall daily operation of the Admissions Department in accordance with current existing federal, state, and local standards, as well as our established policies and procedures, to assure that the medical and financial appropriateness has been considered for each resident prior to providing the services needed.
DELEGATION OF AUTHORITY:
As Admissions Coordinator, you are delegated the administrative authority, responsibility, and accountability necessary for carrying out your designated duties. You report directly to the facility Director of Admissions.
JOB FUNCTIONS:

DutiesandResponsibilities:
AdministrativeFunctions:
1.Plan, develop, organize, implement, evaluate, and direct the admissions activities of this facility.
2.Assist in the development, administering, and coordinating of department policies and procedures.
3.Keep abreast of current federal and state regulations, as well as professional standards, and make recommendations on changes in policies and procedures to the Director of Admissions.
4.Review department policies and procedures, at least annually, and participate in making recommended changes.
5.Develop and implement policies and procedures for reviewing, identifying and evaluating the needs of the facility to ensure our census is 100% capacity
6.Consider new avenues and ideas for census development including partnership with sister facilities.
7.Assess incoming patients and make sure they meet the medical facility's admission criteria and verify insurance viability for potential admission.
8.Participate in community planning related to the interests of the facility and marketing the services of the facility.
9.Participate in admission planning, development and implementation to assess the medical and financial needs and appropriateness of potential residents.
10.Interview residents/families in hospital settings, at the facility or at a mutual acceptable location.
11.Perform administrative requirements, such as completing necessary forms, reports, etc., and submitting such to the Director of Admissions as required
12.Ensure all admissions paperwork is reviewed and complete by resident or responsible party based on state, federal and company requirements
13.Arrange for the assistance of the Social Worker, Director of Nursing, Business Office Manager if needed to explain medical, social or financial issues that might impact the residents stay to the resident or responsible party.
14.Provide information to resident/family as to Medicare/Medicaid, and other financial assistance programs available to the resident if needed
15.Provide assistance to families of hospital patients who might not be familiar with long term care describing services and benefits of a rehab / long term care admission if necessary.
16. Assume the authority, responsibility, and accountability to ensure all admission contracts and paperwork are completed timely as possible
17. Maintain a quality working relationship with the medical profession and other health-related facilities, referral sources and organizations.
18. Compile written and oral reports/recommendations to the Director of Admissions concerning the Admissions department.
19. Interpret the department's policies and procedures to employees, residents, visitors, government agencies, etc.
20. Keep abreast of economic conditions/situations making adjustments as required in strategies as needed to ensure strong census for the facility.
21. Be available for facility tours for family members, referral sources, discharge planner or community leaders.
22. Continue to nurture and develop new referral sources including hospitals, senior programs/centers, churches, and community associations.
23. Be sure to forward any complaints and grievances from residents, family members, referral sources to the Director of Admissions. Follow facility's established procedures.
24. Participate in facility surveys (inspections) made by authorized government agencies
25. Follow up with residents, family members, as necessary, to assist with adjustment to the facility. Involve the Social Worker Director is necessary.
26. The Admission Coordinator may be needed to speak or conduct meetings to promote the facility and services.
27. Update the census board on a daily basis to include all admission, transfers and discharges.
Committee Function:
1. Serve on, participate in, and attend various committees of the facility (i.e. Infection Control, Policy Advisory, Pharmaceutical, Budget, Quality Assessment and Assurance, etc.) as required, and as appointed by the Director of Admissions.
2. Provide written and/or oral reports on Admissions activities as required.
3. Consider committee recommendations as they pertain to Admissions and Census Development.
4. Attend department head meetings, etc., as scheduled or as may be called.
5. Schedule and announce departmental meeting times, dates, places, etc.
Personnel Functions:
1. Assist as requested by the Director of Admissions in recruitment, interviewing, and selection of Admission department personnel.
2. Assist in standardizing the methods in which Admission tasks will be performed.
3. Maintain a productive working relationship with other department supervisors and staff and coordinate admission procedures to assure that daily referrals can be performed without interruption.
4. Report occupational exposures to blood, body fluids, infectious materials and hazardous chemicals in accordance with the facility's policies and procedures governing accidents and incidents.
5. Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment, throughout the facility.
Staff Development
1. Develop and participate in the planning, conducting, scheduling of timely in-service training classes that provide instructions on “how to do the job,” and ensure a well-educated admissions department.
2. Assist admissions in developing, implementing, and conducting in-service training programs that relate to the admissions department for other departments.
3. Attend and participate in continuing educational programs designed to keep you abreast of changes in your profession, as well as to maintain your current status.
4. Ensure that all admissions personnel attend and participate in annual OSHA and CDC in-service training programs for hazard communication, TB management, and bloodborne pathogens standard as required.
Safety and Sanitation:
1. Assist the Safety Officer in developing safety standards for the admissions department.
2. Ensure that the department's policy and procedure manual identifies safety precautions and equipment to use when performing tasks that may result in bodily injury.
3. Monitor admission personnel to assure that they are following established regulations in the use of equipment and supplies.
4. Ensure that admission work areas are maintained in a clean and sanitary manner.
5. Ensure that all admission personnel follow established departmental policies and procedures, including appropriate dress codes.
6. Ensure that admission personnel participate in and conduct all fire safety and disaster preparedness drills in a safe and professional manner.
7. Ensure that admission personnel follow established infection control procedures when isolation precautions become necessary.
8. Ensure that department personnel follow established procedures governing the use of labels and MSDSs.
9. Report missing/illegible labels and MSDSs to the safety officer or other designated person.
10. Ensure that admission personnel performing tasks that may involve potential exposure to blood, body fluids, infectious disease, etc., attend appropriate in-service training classes prior to performing such tasks.
11. Maintain a procedure for reporting hazardous conditions or equipment.
12. Report any exposure to blood/body fluids.
Equipment and supply functions:
1. Recommend to the Director of Admissions the equipment and supply needs of the department.
2. Place orders for equipment and supplies as necessary or as may be required
3. Make periodic rounds to check equipment and to assure that necessary equipment is available and working properly.
4. Ensure admission supplies are used in an efficient manner to avoid waste.
Budget and Planning Functions:
1. Forecast needs of the department as requested.
2. Assist in preparing and planning the admission department's budget for equipment, supplies and submit it to the Director of Admissions for review, recommendations, and approval.
3. Maintain current written records of department expenditures and assure that adequate financial records and cost reports are submitted to the Director of Admissions upon request or as necessary.
4. Make departmental adjustments in order to conform to approved budget.
Resident Rights:
1. Maintain the confidentiality of all resident care information.
2. Knock before entering a resident's room.
3. Ensure admission personnel are knowledgeable of the resident's rights and responsibilities, including the right of refusal.
4. Review complaints and grievances made by the resident and make a written/oral report to the Director of Admissions indicating what action(s) were taken to resolve the complaint or grievance. Follow facility's established procedures.
5. Maintain a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.
6. Provide residents with information concerning resident rights, living wills, etc. Explain as necessary.
7. Participate in resident/group council meetings as requested.
Miscellaneous:
1. Be prepared to handle emergencies as they come up or as directed by the Director of Admissions.
2. Work with the facility's consultants as necessary and implement recommended changes as required.
Other Duties:

Working Conditions:
Works in office areas as well as throughout the facility. Moves intermittently during working hours. Is subject to frequent interruptions. Is involved with residents, personnel, visitors, government agencies/ personnel, etc., under all conditions and circumstances. Is subject to hostile and emotionally upset residents, family members, etc. Communicates with the medical staff, nursing staff, and other department supervisors. Works beyond normal working hours, on weekends, and in other positions temporarily, when necessary. Is subject to call-back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.). Attends and participates in continuing educational programs. Is subject to injury from falls, burns from equipment, odors, etc., throughout the workday, as well as to reactions from dust, disinfectants, tobacco smoke, and other air contaminants. Is subject to exposure to infectious waste, diseases, conditions, etc., including TB and the AIDS and Hepatitis B viruses. Maintains a liaison with other department supervisors to adequately plan for social services/activities. May be subject to the handling of and exposure to hazardous chemicals.
Education Requirements:
RN preferred, must possess, as a minimum a Bachelor's Degree or five years related experience in a similar position and responsibility.
Experience:
Must have five years experience in an admissions coordinator capacity in a hospital, skilled nursing care facility, or other related medical facility.
Specific Requirements:
Must be able to read, write, speak, and understand the English language. Must possess the ability to make independent decisions when circumstances warrant such action. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must possess leadership ability and willingness to work harmoniously with and supervise other personnel. Must have patience, tact, cheerful disposition and enthusiasm, as well as be willing to handle residents based on whatever maturity level in which they are currently functioning. Must possess the ability to seek out new methods and principles and be willing to incorporate them into existing social services. Must not pose a direct threat to the health or safety of the individuals in the workplace.

Work will be performed primarily indoors at one of our long-term healthcare facilities, throughout all areas, including in resident rooms, and on carpeted and/or tiled floors. Work will be performed there routinely around other co-workers, healthcare staff, residents, and guests. Due to the nature of facility's business, worker may be exposed to occasional slippery floors, object on floors, chemicals, sharp objects, hazardous materials and waste (including human), blood borne pathogens, and communicable diseases, as well as high-stress medical and/or life-threatening situations.
Essential Duties & Responsibilities:

Perform respiratory care assessment of patients in an accurate, thorough, and timely manner to assure the delivery of effective therapy. Monitor patient's rehabilitation goals for appropriateness and consistency of therapy.
Provide ongoing care to patients requiring oxygen therapy, artificial airway management, mechanical ventilation and weaning, among others, according to B.R.C., AARC, and department policy and procedure.
Administer respiratory related medication to the tracheobronchial tree according to physician orders, B.R.C., AARC standards, and department policy and procedures. Accurately document in the medication administration records (MARS) the administration of medication according to physician's orders.
Consistently and accurately document in the patient medical records patient assessment with clinical parameters such as, breath sounds, tracheostomy tube data, sputum production, ventilator settings based on physician orders, weaning and tolerance, in compliance with department daily documentation records.
Provide the in-coming shift with room-to-room walking detailed, verbal report on the patient condition, any change in orders and tolerance to care provided. Consistently and accurately provide information regarding Spo2, Fio2, SVT, RR, sputum production, breath sounds, mode of therapy, medication administration and tolerance, among others. For patients on ventilatory support, latest ventilator settings, tolerance to weaning and Fio2 titration.
Perform diagnostic procedures in accordance with physician orders such as, sputum collection and arterial blood gases following B.R.C. safety standards and department policy and procedure.
Perform physician rounds when they are in the facility and update orders accordingly in the respiratory care daily assignment record.
Always maintain equipment and supply room clean and organized. When bulk supplies are in the equipment and supply room, remove supplies from packaging and properly place them on the shelves for easy access. No beverages and/or food consumption is allowed in the equipment and supply room.
When patients are transferred or discharged from the unit, immediately remove all equipment from the patient room and proceed to clean, disinfect and placed in the appropriate clean area for easy access. When patients are transferred or discharged from the unit or you are changing supplies according to the department change-out policy and procedure, discard all soiled disposable supplies in a RED biohazard bag. Only soiled, disposable supplies are to be disposed of in the red biohazard bags. Biohazard bags are ONLY to be disposed of in the dirty soiled utility room. All other trash is to be placed in a clear, bag-lined trash receptacle.
At all times, follow universal precautions according to CDC guidelines and the facility infection control policy and procedures to comply with regulatory and safety standards for the protection of self and other employees and patients.

· Placing appropriate hot and cold food items on residents' trays at mealtime according to menu, diet cards, or designated by the manager or cook.
· Prepare sandwiches, juices, special and meal items according to diet cards, and place on trays at meal times. Some items are but not limited to: peanut butter & jelly, meats sandwich, thickened milk, juice, coffee, tea, fruit, yogurt, and ginger ale.
· Prepare and/or deliver food items for restorative program as designated by the policy posted on the juice refrigerator.
· Deliver all meal trays to appropriate destinations on the nursing floor at appropriate times, retrieve trays at appropriate times, and collect any trays or dishware from floor before end of shift.
· Must breakdown all trays and items on trays by putting through dish machine, putting items away in appropriate places, and restock machine with appropriate chemicals.
· Must take out all trashcans after each meal, sweep, mop, and sanitize work areas according to checklists or as deemed appropriate by the supervisor or cook.

Ensure that narcotic counts are accurate before and after every shift.

Provide feedback and recommendations for probationary and annual employee performance evaluations for CNAs/GNAs and other nursing personnel. Prepare probationary and annual employee performance evaluations for CNAs/GNAs and other nursing personnel. Meet with CNAs/GNAs and other nursing personnel to review performance.
Develop work assignments and/or assist in completing and performing such assignments. Reassign assigned CNAs/GNAs and other nursing personnel based upon skills and qualifications.
Provide leadership, education, counseling, and discipline, when appropriate, to assigned CNAs/GNAs and other nursing personnel.
Make daily rounds of your unit/shift to ensure that assigned CNAs/GNAs and other nursing personnel are performing their work assignments in accordance with acceptable nursing standards. Make changes to assignments based upon resident needs and availability of skilled staff.
Evaluate daily performance of assigned CNAs/GNAs and other nursing personnel. Document any disciplinary issues and report problem areas or disciplinary actions to the Nurse Supervisor and/or Unit Manager.
Meet with your assigned CNAs/GNAs and other nursing personnel, on a regularly scheduled basis, to assist in identifying and correcting problem areas, and/or to improve services. Assign personnel accordingly to insure quality resident care while allowing personnel to improve in areas where they may be deficient, with appropriate supervision.
Ensure that CNAs/GNAs, department personnel, residents, and visitors follow the department's established policies and procedures at all times.
Resolve personnel disputes and issues within the unit and reassign or remove personnel, if necessary, to insure patient safety.
Develop and maintain a good working rapport with inter-departmental personnel, as well as other departments within the Facility to ensure that nursing services and activities can be adequately maintained to meet the needs of the residents.
Review complaints and grievances made or filed by your assigned CNAs/GNAs or other nursing personnel. Make appropriate reports to the Nurse Supervisor and/or Unit Manager as required or as may be necessary. Follow Facility's established procedures.
Receive/provide the nursing report upon reporting in and ending shift duty hours.
Report occupational exposures to blood, body fluids, infectious materials, and hazardous chemicals in accordance with the Facility's policies and procedures governing accidents and incidents.
Nursing Care Functions
Inform nursing personnel including assigned CNAs/GNAs of new admissions, their expected time of arrival, room assignment, etc.
Ensure that rooms are ready for new admissions.
Greet newly admitted residents upon admission. Escort them to their rooms, as necessary.
Participate in the orientation of new residents and family members to the Facility.
Make rounds with physicians, as necessary.
Requisition and arrange for diagnostic and therapeutic services, as ordered by the physician, and in accordance with our established procedures.
Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary.

Make periodic checks to ensure that certified nursing assistants, CNAs/GNAs, and/or other nursing personnel are properly administering prescribed treatments and to evaluate the resident's physical and emotional status.
Ensure that direct nursing care be provided by a licensed nurse, a certified nursing assistant, CNA/GNA, and/or a nurse aide trainee qualified to perform the procedure.
Cooperate with and coordinate social and activity programs with nursing service schedules.
Notify the resident's attending physician when the resident is involved in an accident or incident.

Carry out restorative and rehabilitative programs to include self-help and care.

Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings and bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead and dying, etc., as required.
Use restraints when necessary and in accordance with established policies and procedures and applicable state laws and guidelines.
Use an Automated External Defibrillator, as required.
Obtain sputum, urine, blood, and other specimens for lab tests, as ordered
Take and record TPRs, blood pressures, etc., as necessary.
Perform Cardiopulmonary resuscitation (CPR), as necessary.
Monitor seriously ill residents as necessary.
Check foods brought into the Facility by the resident's family or visitors to ensure that it is within the resident's dietary allowances. Report problem areas to the Nurse Supervisor and/or Unit Manager and Dietary Supervisor.
Ensure that personnel providing direct care to residents are providing such care in accordance with the resident's care plan and wishes.
Ensure that residents who are unable to call for help are checked frequently.
Meet with residents, and/or family members, as necessary. Report problem areas to the Nurse Supervisor and/or Unit Manager.
Admit, transfer and discharge residents, as necessary.
Assist in arranging transportation for discharged residents, as necessary.
Ensure that discharged residents are escorted to the pick-up area.
Inform family members of the death of the resident.
Call funeral homes when requested by the family. Ensure that established post- mortem procedures are followed.
Staff Development

Recommend to the Nurse Supervisor the equipment and supply needs of your unit or shift.
Ensure that an adequate stock level of medications, medical supplies, equipment, etc., is maintained on your unit or shift at all times to meet the needs of the residents.
Participate in the development and implementation of the procedures for the safe operation of all nursing service equipment.
Ensure that only trained and authorized personnel operate your unit or shift's equipment.
Ensure that all personnel, including CNAs/GNAs and other nursing personnel assigned during your shift operate nursing service equipment in a safe manner.
Monitor nursing procedures to ensure that nursing service supplies are used in an efficient manner to avoid waste.
Ensure that appropriate MSDSs are on file for hazardous chemicals used in the nursing service department.
Care Plan and Assessment Functions
· Review care plans daily to ensure that appropriate care is being rendered.
· Inform the Nurse Supervisor and/or Unit Manager of any changes that need to be made on the care plan.
· Ensure that your nurses' notes reflect that the care plan is being followed when administering nursing care or treatment.
· Review resident care plans for appropriate resident goals, problems, approaches, and revisions based on nursing needs.
· Ensure that assigned CNAs/GNAs and other nursing personnel are aware of the resident care plans. Ensure that the CNAs/GNAs and other nursing personnel refer to the resident's care plan prior to administering daily care to the resident.
· Supervise assigned CNAs/GNAs and other nursing personnel activities included in the care plan. Provide CNAs/GNAs and other nursing personnel with direction and counseling necessary to provide services in accordance with the care plan.
· Assist in planning, scheduling, and revising the MDS, including the implementation of RAPs and Triggers.
Resident Rights
Ensure that all nursing service personnel including assigned CNAs/GNAs are knowledgeable of the residents' responsibilities and rights including the right to refuse treatment.
Review complaints and grievances made by the resident and make a written or oral report to the Nurse Supervisor indicating what action(s) were taken to resolve the complaint or grievance. Follow the Facility's established procedures.
Maintain a written record of the resident's complaints and/or grievances that indicates the action taken to resolve the complaint and the current status of the complaint.
Report and investigate all allegations of resident abuse and/or misappropriation of resident property.
Ensure that nursing staff personnel including assigned CNAs/GNAs honor the resident's refusal of treatment request. Ensure that such requests are in accordance with the Facility's policies governing advance directives.
Must adhere to all HIPAA requirements.
Miscellaneous
Provide data to the Quality Assurance & Assessment Committee as requested.
Working Conditions
Works in office area(s) as well as throughout the nursing service area (i.e., drug rooms, nurses' stations, resident rooms, etc.).
Involved with residents, personnel, visitors, government agencies or personnel, etc., under all conditions and circumstances.
May be subject to hostile and emotionally upset residents, family members, and visitors.
Communicate with the medical staff, nursing personnel, and other department directors.
May be required to work beyond normal working hours, and in other positions temporarily, when necessary.
May be subject to call back during emergency conditions (e.g., severe weather, evacuation, post-disaster, etc.).
Attend and participate in continuing educational programs.
May be subject to the handling of and exposure to hazardous chemicals.
Maintains a liaison with the residents, their families, support departments, etc., to adequately plan for the residents' needs.
Education
Must possess, at a minimum, a Nursing Degree from an accredited college or university, or graduate from an approved LPN/LVN/RN program.

Must possess a current, unencumbered, active license to practice as an RN or LPN/LVN in this state.
Must be able to read, write, speak, and understand the English language.
