Position Title: Medical Case Management Care Coordinator
Department: HealthWorks Reports To: Director of HealthWorks Location: Park Forest, IL Hourly Rate: $18.00/hour
SUMMARY
The Medical Case Management (MCM) Care Coordinator ensures timely access and assignment of youth to ongoing Medical Case Management services. This role reviews interim case activities, monitors performance benchmarks, prepares case materials, manages data entry, and supports the Interim Care Coordination Unit as needed. The Care Coordinator plays a key role in ensuring accurate documentation, compliance with timelines, and effective coordination of health information across systems.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Ensure timely access and assignment of youth for ongoing Medical Case Management (MCM).
Assist in final review of completed interim critical case activities.
Review and monitor daily, weekly, and monthly reports to identify cases approaching funder benchmarks.
Assign and reassign youth to MCM services within required timeframes, including:
New custody cases within 5 business days after the 45-day interim period (95% benchmark).
Reassignments within 14 days of receipt from the Ongoing Medical Case Management Agency.
Conduct outreach to PCP/Dental providers for transfers involving youth ages 0-5 and pregnant youth.
Prepare and forward complete MCM assignment packets-including health information, placement details, and contact information-according to contractual timelines.
Generate and distribute written correspondence informing caregivers, youth, caseworkers, and providers of assigned MCM services.
Work with the Medical Records Clerk to ensure closed cases are properly prepared for storage and long-term retention.
Produce standard and custom reports using information system tools as directed by the HealthWorks supervisor.
Gather and maintain data for reporting and analytics needs within HealthWorks.
Complete all assigned data entry in the HealthWorks Database/SACWIS and Athena systems.
Review and complete tasks associated with SACWIS reports and other DCFS reporting requirements.
Use system reports to support data integrity and sanitization of the information system.
Submit daily production reports to the immediate supervisor detailing assignment status, pending items, and issues.
Perform all other duties as assigned.
QUALIFICATIONS Education and/or Experience
Bachelor's or Associate's degree in Social Work, Computer Science, Human Services, or related field; OR equivalent education and experience.
Three years of professional experience in social services preferred .
Language Skills
Ability to read, analyze, and interpret data and case documentation.
Strong verbal and written communication skills.
Mathematical & Computer Skills
Ability to solve practical problems and work with concrete variables.
Ability to compute rates, ratios, and percentages; interpret charts and graphs.

SUMMARY
The purpose of the Juvenile Justice Clinician position is to act as a stabilizer and provide assessment, counseling and service linkage to youth at risk of becoming involved in the juvenile justice system and/or at risk of delinquency. * Ensure all required intake documentation is complete and accurate.
* Responsible for maintaining active CCBYS client files with documents not exceeding more than 2 weeks past due
* Monitor and supervise clients to ensure their safety and well-being during programming hours and transport.
* Assist Crisis Intervention Specialist with Juvenile Justice partner collaboration and outreach
* Collaborate with Reporting Center staff regarding participant referrals

Areas of Accountability - What you will be doing:
Provide direct service to individuals. This includes implementing initial treatment plans, providing ongoing therapy, updating treatment plans, continuing stay reviews, counseling case management, and providing crisis intervention services and referrals if needed.

Monitor toxicology screens and work with the Medical Department to adjust privileges according to client track records.

Maintain client records using electronic medical records. Must be documented as required by program standards and funding requirements including documentation of clinical interactions, behavioral health assessments and individual service plans.
Adhere to all state and federal privacy regulations, including HIPAA and 42 CFR Part 2, and to CCC Policies and agreements regarding confidentiality, privacy, and security. Support.
Compliance with all privacy and security requirements pursuant to community partners' and outside providers' patient confidentiality agreements, including privacy and security requirements for EMR access. This includes immediate reporting of any PHI breach of CCC or outside provider health records to the CCC Legal and Quality departments, as well as the program administrator.
Any staff holding specialized certifications should exercise the certification in a clinical setting, i.e., staff processing a QHEIC certification should attend at least 10 hours of HIV training annually and at least 1 test per month (if applicable).

* Accept all crisis referrals from the CARES line within 30 minutes-no-decline policy applies.

* Provide immediate short-term follow-up, transition planning, and linkage to necessary services.
* Complete MCR intake documentation at the point of crisis and ensure all records are accurate and timely.
* Facilitate safe outcomes such as family reunification, temporary placement, or psychiatric hospitalization.
* Coordinate referrals and follow-ups based on client and family needs.
* Remain reachable via mobile phone during on-call hours; maintain a home phone line as a backup contact method.

ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned:
* Execute routes as assigned by team leader in either an agency vehicle or one's personal vehicle.
* This may include preparing the agency vehicles for daily use (I.e. putting the keys in the vehicles.)
* Assist in the planning of cultural, educational and/or social outings on a regular basis.
Implement the curriculum as designated by the schedule. Facilitating/co-facilitating group discussions.
* Report any deviation of activities to your supervisor immediately (within 24 hours).
Address individual youth and group behaviors using motivational interviewing and de-escalation skills.
* Build and maintain healthy relationship with the youth.
* Provide information to the team leader relevant to any unaddressed, perceived client needs as part of on-going assessment of referred youth.
* Provides and fosters excellent service to clients.
* Ensure safety and well-being of clients during all programming hours.
* Assume duties including but not limited to driving and maintaining vans, transporting clients to and from program sites, transporting food to program sites, and assisting in facilitating groups.
* Manage conflict and crisis that occur during program hours.
* Enter client information into Casey Life Skill tracking system within 72 hours of intake.
* Collaborate and network with law enforcement, probation, employers, family members, schools, health providers and any other providers involved in the client's service planning.
* Create and maintain individual case plan for all youth involved in program.
* Prepare clear, concise, well written documents (i.e. case notes) and incident reports to depict a clear image of client behaviors and other program related incidents.
* Ensure programming is implemented within the designated hours.
* Prepare and submit all paperwork accurately and within the prescribed timeline.
* Complete all Core Curriculum training(s) required for this position. Complete on-going training as designated by program requirements.
QUALIFICATIONS

EDUCATION and/or EXPERIENCE
High School diploma or GED and one to two years' experience working with at-risk youth. A bachelor's degree (BA/BS/BSW) is preferred.
OTHER QUALIFICATIONS
You will be required to have a reliable vehicle to transport clients and maintain a clean driving record. You must also be able to accept and incorporate constructive feedback.
Strong desire to work with youth.
Excellent writing and communication skills.
Experience working with aggressive youth is preferred.
CERTIFICATES, LICENSES, REGISTRATIONS
Valid driver's license with a clear driving record.
Documentation verifying current automobile insurance.
