SUMMARY The purpose of the Community Support Specialist II position is to act as a stabilizer and provide assessment, counseling and service linkage to youth at risk for running away, being beyond the control of their parents and/or from becoming involved in the juvenile justice system. This position provides individual, family and/or group therapy, school advocacy, life skills training and educational & therapeutic group activities as defined by client need to 10-17-year-old youth within the program service area. ESSENTIAL DUTIES AND RESPONSIBILITIES Daily Job Duties: * Respond to all emails, texts, and phone calls from Program Coordinator within a 15-minute time frame. * Ensure the Outlook calendar updated with daily tasks and/or travel in real time. * Maintain an active caseload of 20-25 with a maximum of 15 individual participants with demonstrated higher level of acuity or risk related to trauma, suicide risk, mental health, etc. and as assigned by Program Coordinator. * Attend any scheduled Clinical Team meetings within assigned area for purposes of ongoing program development related to clinical efficacy, case review/staffing, and professional development * Facilitate groups for discretionary clients within designated LANs * Ensure all required crisis intervention and/or intake documentation is complete and accurate. * Responsible for maintaining active CCBYS client files with documents not exceeding more than 2 weeks past due * Responsible for updating and maintaining their designated LAN client, MRAI/referral, outreach, and quarterly success stories spreadsheet. * Monitor and supervise clients to ensure their safety and well-being during programming hours and transport. Other Duties: * Assist the Community Liaison with the upkeep and development of community relationships * Assist the Community Liaison with planning, budgeting, and executing monthly client baskets and events, as well as, delivering to clients or other LAN's; when necessary or assigned by Program Coordinator. * Assist the Community Liaison in planning and executing CCBYS all staff events and assigned lunch and learn presentations; when necessary or assigned by Program Coordinator. * Assist the Community Liaison with delivering files to the correct LAN or Program Coordinator for monthly division audits and level audits; when necessary or assigned by Program Coordinator. * Assist the Community Liaison with in coordinating and attend annual community outreach meetings; when necessary or assigned by Program Coordinator. * Obtain Letters of Support (LOS) and Memorandums of Understanding (MOU) from community partners annually. * Participate in individual, group supervision, team meetings, program staff meetings, and trainings as scheduled to address client issues and revised documentation, placement options, crisis schedules, etc. * Provide required documentation (i.e., mileage, time sheets, CQIs, surveys, etc.) as indicated by agency protocols. * Complete all necessary paperwork and documentation, i.e., timesheets, vouchers, progress notes, incident reports, etc., and submit in a timely manner. * Other Duties as assigned by Program Coordinator, Program Manager, and/or Program Director QUALIFICATIONS To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
