Family Medicine, Internal Medicine, and IM PEDS SUMMARY The purpose of this position is to provide high quality patient care and ensure effective clinic operations. Patient care includes providing comprehensive medical services for members of family. Services include but are not limited to: preventive care, immunizations, primary care and treatment, health education, prescribing of medications, ordering and interpreting lab testing, physical examinations, STD screenings, vision/hearing, women's health, family planning, prenatal services, nutrition. Provider may be used in a variety of locations depending upon the need of the organization. Clinic shifts include 1 evening a week and 1-2 Saturdays/month. This position may also include supervising physician assistants and/or nurse practitioners and providing training to resident physicians or other health provider interns/externs. REQURED EDUCATION, EXPERIENCE and COMPETENCIES Provide continuous, comprehensive primary care for infants, children, youth, and adults according to current standards of care. Take patient histories. Perform or request diagnostic tests. Make diagnoses and prescribe medication as necessary. Administer vaccinations and perform follow-up examinations. Educate patients in disease prevention and health maintenance. Prepare all medical records, clinical reports, and correspondences. Support quality initiatives. This position will be a part of the City of Peoria Social Services Unit (SSU). SSU personnel are tasked with assisting those who come into contact with, or are referred to, the SSU. The Pharmacist provides pharmaceutical care to all patients of Carle Foundation Hospital and affiliated health care providers. Qualifications Certifications: Licensed Pharmacist - Illinois Department of Financial and Professional Regulation (IDFPR) Illinois Department of Financial and Professional Regulation (IDFPR), Education: Bachelor's Degree: Pharmacy (Required), Work Experience: Responsibilities Assist in focused studies and drug usage evaluations as assigned. Provide direct patient care by intervening as appropriate to improve prescribing, administration, and monitoring practices in the facility as related to medications. Represent the pharmacy on committees or work groups as assigned. Interpret physician medication orders and send the appropriate medication in a timely and accurate manner. Screen medication orders for potential interactions with other medications, diet orders, disease state, or stated allergies. Supervise the technicians in the preparation and dispensing of medications and parenterals. Make dose recommendations based on age, weight and clearance function. Assist in the training and orientation of all new employees as assigned. Provide accurate and complete accounting of all controlled substances.
