Position Title: HFW Care Coordinator
Location: Park Forest, IL Hourly Rate: -$21.63
SUMMARY
Under the direct supervision of the CCS Supervisor , the HFW Care Coordinator serves as a liaison between patients, families, and the healthcare system to ensure the coordination of primary care and behavioral health services. This role supports patients through high-fidelity wraparound services, ensuring access to care, consistency in treatment, and connection to community resources.
The HFW Care Coordinator provides exceptional customer service, ensures a safe and efficient environment of care, and demonstrates the ability to manage multiple priorities while maintaining accuracy, professionalism, and compassion in all interactions.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Maintain patient confidentiality and adhere to ethical and professional standards at all times.
Deliver outstanding customer service in accordance with the AIDET model, exceeding expectations to ensure the best possible outcomes.
Manage a high-fidelity wraparound caseload, ensuring behavioral health needs are consistently addressed and coordinated.
Schedule and track medical, dental, and behavioral health appointments for patients.
Conduct required in-person and telephonic contacts with patients and families, including monthly family team meetings.
Develop individualized care plans in collaboration with patients, families, and interdisciplinary care teams.
Maintain accurate and timely data entry across various databases and electronic portals.
Serve as a liaison between behavioral health and primary care teams by attending case consultations, care team meetings, and clinical huddles.
Apply knowledge of medical and mental health conditions to support patient care planning and intervention.
Complete evidence-based assessments, including tools such as SBIRT, PHQ-2/PHQ-9, GAD-7, and Vanderbilt.
Provide case management and advocacy, connecting patients to internal and external mental health, substance use, and community support services.
Collaborate with health plans, hospitals, specialists, and providers to ensure comprehensive, coordinated care.
Attend required trainings and meetings, and complete all Core Curriculum courses related to this position.
Perform all other duties as assigned.

The compensation range for this position is $17.67per hour - $28.63per hour. This represents a good faith minimum and maximum range for the role at the time of posting by Carle Health. Schedule: Will join Surgical Transport and Control Desk cost center (1439) supporting the role of an OR Pavilion Scheduling Specialist - Feinberg OR. Will cross-train across various control desks. Orientation schedule will be from 8:00am-4:30pm. After orientation shift will remain Monday through Friday 8:00am-4:30pm. Coverage - responsible for filling shift gaps, call-offs, vacations, and absences for all OR Pavilion Scheduling Specialists and Feinberg Control Desk. Rotation - weekend, shift, and holiday coverage as needed by the department.

The OR Pavilion Scheduling Specialist, Operating Room supports the Operating Room (OR) scheduling production process through planning and coordinating OR surgery schedule to maximize patient access and efficient use of operating rooms, equipment, and staff under supervision of the Medical Director/Anesthesia Coordinator/Director/Manager/Resource Coordinator. This position will act as an office scheduling liaison for the pavilion operating room staff, physicians, and management to relay patient throughput and scheduling information requiring interfacing with supporting departments.
Responsibilities:
Scheduling Functions:
* Accesses protected health information (PHI) and ensures all job duties are in accordance with NM confidentiality policies and procedures and HIPPA guidelines.
* Demonstrate in-depth scheduling working knowledge and expertise with clinical based computer systems by initiating, coordinating and maintaining the schedule over the operating room pavilion(s).
* Monitors tentative operating room schedules and identifies scheduling barriers (e.g., surgical procedures/procedure name and discrepancies in type of operating room (OR)/pavilion assignment) and contacts appropriate scheduling office/service and implements solutions to improve scheduling workflow.

* Determines order in which surgical procedures are scheduled, by reviewing procedure type/category/case level, duration time, and OR availability and schedules cases in accordance with Surgical Services scheduling guidelines.

* Collaborates and participates in the OR daily huddle with Pavilion Medical Director/Manager for final approval of the next day surgical schedule.

* Effectively communicates information with other departments, staff, management and physicians.
* Recognizes and responds appropriately to semi-urgent/urgent/emergent scheduling situations per protocols.
* Assists with rescheduling surgeries as needed to accommodate emergencies other unanticipated events.

Major Responsibilities:
Responsible for collaborating with directors, supervisors, charge nurses and others as needed to maintain appropriate staffing levels.
1) Maintains database and scheduling of all nursing associates
2) Receives daily report from the night supervisor, and gives report to the PM supervisor and staffing coordinator at the end of the shift, regarding staffing and acuity
3) Coordinates daily with nursing leaders and/or charge nurses in regards to staffing needs and discusses resource availability.
4) Collaborates with Clinical Managers and charge nurses regarding staffing according to acuity
5) Assigns float pool associates and Agency personnel to units as needed when available
6) Collaborates with unit leaders to determine LCD if necessary.
7) Completes all appropriate documentation on the staffing sheets, and documents in Advocateworks as to the placement of float pool staff.
8) Reviews staffing sheets and enters information that was missed being entered by the Administrative Nursing Supervisors.
9) Provides clerical support for Director/Manager of Clinical Operations in the form of agendas, minutes, flyers, misc. Types letters, memorandums, reports, and other types of correspondence, from rough drafts or provided copy.
10) Handles telephone calls promptly and courteously, screens, and directs calls; records messages accurately
Responsible for daily administration of Float Pool by:
1) Coordinating the orientation and on-boarding of new Float Pool associates.
2) Assures associate satisfaction standards are met.
3) Scheduling of Float Pool staff to match requests based on availability, need and competency; obtains needs schedule from corresponding units.
4) Provides ongoing Float Pool statistics of requirements according to PRN agreement.
5) Contacts agencies for supplemental staffing when determined necessary and completes appropriate documentation.
6) Collaborates with charge nurses to obtain coverage for high acuity, sick calls, overflow patients and sitters needs.
7) Assist the Clinical Operations manager with managing initial and annual compliance to regulatory standards of the float pool along with adherence to Advocate Condell Medical Center and Advocate Health Care policies, including float pool personnel licensure and compliance with standards.
8) Assures proper billing of agency charges.
Responsible for the cost effective use of agency resources for the house.
1) Negotiates hospital specific agency contracts if necessary, to meet staffing needs.
2) Communicates directly with agencies regarding resolution of quality issues, providing documentation as necessary.
3) Completes billing from agencies, verifying agency shifts worked, resolving any billing issues with the agency, and charging out agency costs to the unit in a timely manner.
4) Completes biweekly agency timecard, charging out agency use to the units.
5) Provides reports to nursing management regarding agency utilization as needed.
Responsible for administrative duties
1) Prepares and reconciles agency bills for payment when agency is utilized.
2) Responsible for maintaining office equipment and supplies for the float pool.
3) Submits float pool associate fund reimbursement to accounts payable after approval of the Manager of Clinical Operations.
Responsible for gathering and maintaining personnel information required for staffing program for the Float Pool.
1) Maintains Nursing Division data base and position control, ensuring confidentiality of these records. Reviews and keeps accurate records on all download information and updates all fields not included in this download.
2) Resource for Advocateworks staffing and scheduling software application. Assist with the maintenance of ongoing data entry for accuracy and timeliness.
3) Overseas ongoing schedule changes for the float pool and coordinates those schedule changes with appropriate departments as necessary.
4) Provides an appropriate handoff of staffing decisions at the end of the day that includes resource allocation that was provided, and discuss any possible or foreseeable concerns in regards to staffing over the next 24 hours to the Administrative Nursing Supervisor.
5) Assist in entering other information on the staffing sheets as needed.
6) Provides Clinical units with information regarding scheduled and available staff and assists in staffing decisions for the next 24 hours, Monday-Friday.
7) Backs up data as needed or required which includes archiving all of the med/surg schedules both when first published and after the pay period.
8) Responsible for monitoring staffing shortages for the upcoming weekend and balance and adjust staff accordingly.
Responsible for preparation of reports used in staffing and budgeting.
1) Prepares and prints statistical, acuity and personnel reports
2) Obtains appropriate information for the Hospital Report Card Act and submits preliminary information to the Director of Clinical Operations.
Responsible for overseeing the maintenance of all applicable computer equipment.
1) Maintains equipment according to established contracts and Shiftwise.
2) Receives updates from the Nursing Division, Nursing Education, and Advocateworks, and updates the computer equipment at necessary.
3) Keeps a copy of staffing contracts on file for reference.
4) Maintains appropriate levels of supplies for operation of the equipment and the department needs.
5) Super-user for the Advocateworks scheduling system - inputs new users and trouble shoots when issues arise. Responsible for granting access for the Advocateworks system for both editors and approvers of the Advocateworks system.
6) Facilitates and maintains upgrades in Advocateworks system by remaining current with available upgrades and interfaces.
Facilitates Cost Effective Allocation of Staff on all unit, and participates in CQI related projects.
1) Audits utilization of in-house Float Pool, agency, sitter, and other personnel.
2) Provides data unique to department, i.e. hours per patient's day, average daily census, etc. Assists in process to analyze data as requested by the Director of Nursing or Clinical Operations.
Education/Experience Required:
- High school graduate - 3 years previous office experience - experience as a Health Unit Secretary preferred
Knowledge, Skills & Abilities Required:

Schedule: Will join Surgical Transport and Control Desk cost center (1439) supporting the role of an OR Pavilion Scheduling Specialist - Feinberg OR. Will cross-train across various control desks. Orientation schedule will be from 8:00am-4:30pm. After orientation shift will remain Monday through Friday 8:00am-4:30pm. Coverage - responsible for filling shift gaps, call-offs, vacations, and absences for all OR Pavilion Scheduling Specialists and Feinberg Control Desk. Rotation - weekend, shift, and holiday coverage as needed by the department.
The OR Pavilion Scheduling Specialist, Operating Room reflects the mission, vision, and values of NM, adheres to the organization's Code of Ethics and Corporate Compliance Program, and complies with all relevant policies, procedures, guidelines and all other regulatory and accreditation standards.
The OR Pavilion Scheduling Specialist, Operating Room supports the Operating Room (OR) scheduling production process through planning and coordinating OR surgery schedule to maximize patient access and efficient use of operating rooms, equipment, and staff under supervision of the Medical Director/Anesthesia Coordinator/Director/Manager/Resource Coordinator. This position will act as an office scheduling liaison for the pavilion operating room staff, physicians, and management to relay patient throughput and scheduling information requiring interfacing with supporting departments.
Responsibilities:
Scheduling Functions:
Accesses protected health information (PHI) and ensures all job duties are in accordance with NM confidentiality policies and procedures and HIPPA guidelines.
Demonstrate in-depth scheduling working knowledge and expertise with clinical based computer systems by initiating, coordinating and maintaining the schedule over the operating room pavilion(s).
Monitors tentative operating room schedules and identifies scheduling barriers (e.g., surgical procedures/procedure name and discrepancies in type of operating room (OR)/pavilion assignment) and contacts appropriate scheduling office/service and implements solutions to improve scheduling workflow.
Collaborates with Medical Director/Anesthesia/surgeons/nursing/scheduling office to complete adjustments to the OR schedule prior to surgery date and communicates changes to scheduling office/Central Scheduling and others as appropriate.
Interacts effectively and acts as the pavilion liaison between multiple supporting departments to gather information needed to schedule surgeries consistent with department guidelines and reviewing physician scheduling requests and patient needs.
Establish and maintain effective working relationships with physicians, nursing, staff, and management.
Determines order in which surgical procedures are scheduled, by reviewing procedure type/category/case level, duration time, and OR availability and schedules cases in accordance with Surgical Services scheduling guidelines.
Assigns operating suites and equipment and block times to maximize efficient use of resources and communicates to the appropriate staff of the changes.
Collaborates and participates in the OR daily huddle with Pavilion Medical Director/Manager for final approval of the next day surgical schedule.
Ensures, maintains, and validates the surgery schedule is accurate and aligns with the scheduling guidelines in an efficient and equitable manner.
Cross trained to fill in and effectively perform all other job functions across surgical pavilions to staffing relief/coverage for the Information liaison(s)/OR Pavilion Scheduling Specialists(s)/Resource Coordinator, as needed.
Control Desk Functions:
Assists the Anesthesia Coordinator/OR Clinical Coordinator at the Control Desk to ensure optimal care and coordination of current day surgical patients.
Answer high volume phone line and aids the caller/employee to include accurately scheduling same day/next day add-on procedures within the electronic health record (e.g., EPIC).
Effectively communicates information with other departments, staff, management and physicians.
Recognizes and responds appropriately to semi-urgent/urgent/emergent scheduling situations per protocols.
Assists with rescheduling surgeries as needed to accommodate emergencies other unanticipated events.
Ability to collaborate across departments and build effective relationships with internal and external customers/staff to ensure operational processes are met.
Extends knowledge as required of new scheduling processes/service line expansions. Acquires and maintains knowledge of patient throughput/scheduling process changes/operational enhancements.
Contacts units/departments to coordinate surgical patients and accurately submits transport requests to the operating room/pre-operative holding area.

Required:

3-5 years of relevant surgery scheduling or related work experience.
