Program Overview:
Intercept , developed by Youth Villages, is an evidence-based intensive, in-home program that focuses on keeping families together by preventing out-of-home placements or facilitating reunification after such placements, including foster care, residential treatment facilities, hospitalization, or juvenile detention centers. It is one of the first programs to receive the well-supported designation from Title IV-E Prevention Services Clearinghouse developed in accordance with the Family First Prevention Services Act (FFPSA) having been shown to reduce out-of-home placements and accelerate permanency for children.
This program serves youth from birth to age 18 who are facing serious emotional or behavioral challenges or have experienced trauma, such as abuse or neglect. Services are provided by highly trained Family Intervention Specialists directly in the family's home and community-Intercept is not an office-based program.
Intercept provides intensive, strength-based support tailored to each family's unique needs, ultimately helping them achieve lasting change through evidence-based mental health interventions, along with the development of new parenting and communication skills.
Essential Duties and Responsibilities:

* Assist students in the decision-making process, both educational and personal.
* Organizes and maintains all written/computer records for all students, grades 6-12, including special needs records.
* Responsible for student scheduling for classes each spring and for students new to the school.
* Assists with and interprets groups state testing programs.
* Actively seeks to enable students to pursue post-secondary education through information, 4-year planning sessions, newsletters, meetings, and individual conferences.
* Provides individual and group financial aid information through meetings, newsletters for parents, and individual conferences.
* Responsible for dissemination of career information related to entering the workforce.
* Conducts individual, personal counseling.
* Follows up with students and parents after grades are distributed.
* Organizes all scholarship information, publishes newsletters, and plans and conducts the honors program for seniors.
* Refers students to outside agencies when needed.
* Welfare referrals are made through the building principal.
* Assists the special education teacher in student scheduling and teacher contacts.
* Assists with homebound instructions, alternative school scheduling, and other special schedules for students who meet the criteria for these programs.
* Maintains strict confidentiality of students.
* Serves on liaison committees.
* Works closely with the student service personnel in the system.
* Other duties assigned by the principal.
* Attends professional conferences and seminars.

Job Skills / Requirements The Certified Nurse Assistant will work under the supervision of a Supported Living Program Manager at the agency's Charlestown location. He/she will work in a 24/7 residential setting providing health care, (including bathing, feeding, grooming, toileting, etc.) to residents in the Supported Living Program that serves clients with intellectual disabilities and/or developmental delays. The CNA will be responsible for completing shift tasks. These tasks include medication management, if needed. Must be dependable and adaptable/flexible.
