Responsibilities
Responsible for the independent supervision of the delivery of care to a group of residents in a nursing unit. Assess resident needs, develop individual care plans, administer nursing care, evaluate nursing care, and supervise Certified Nursing Assistants (CNAs) and other personnel in the delivery of nursing care.
Qualifications
* A current, active license to practice as a Registered Nurse (RN) in State of practice.
* Demonstrate RN clinical assessment skills at the level necessary to meet the job requirements.
* Knowledge of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to long-term care.
* Effective interpersonal skills and the ability to work with an interdisciplinary care team.
* Ability to plan, organize, develop, implement, and interpret the programs, goals, objectives, policies and procedures that are necessary for providing quality care.
* Basic understanding of computer technology, including email use.
* Ability to read, write, speak, and understand English.
* Proven decision making and analytical skills.
* Able to relate positively, favorably and cooperatively with others, including employees, residents, family members, and personnel of outside agencies and organizations.
* Positive attitude toward the elderly.
* At least one year of experience in Long Term Care is preferred.

Duties and Responsibilities:
Develop a written plan of care for each resident and review all medical and nursing treatments to ensure they are in accordance with the resident's care plan and wishes.
Implement and maintain an ongoing quality assurance program for the nursing department, and monitor the facility's QI, QM and survey reports.
Assist in the planning, scheduling and revising of the MDS, including the implementation of CAAs and Triggers.
Review and revise care plans and assessments, and ensure that all personnel involved in providing care are aware of the resident's care plan.
Develop methods for coordination of nursing services with other resident services to ensure the continuity of the resident's total regimen of care.
Participate in and develop reviews of the discharge plans and prepare reports for the Care Plan Committee as directed.

Responsibilities
Complete required record keeping in regards to your role in admitting, transferring and discharging residents.
Coordinate with nursing staff and support personnel daily to plan the shifts' services, programs and activities to ensure the resident's total regimen of care is maintained.
Prepare and administer medications and review medication cards for completeness and accuracy.
Plan, organize, develop, implement and interpret programs, goals, objectives, policies and procedures that are necessary for providing quality care.
Provide leadership to nursing personnel and make daily rounds of your unit to ensure staff is providing direct care in accordance with resident's care plan and wishes.
Participate in facility surveys by authorized government agencies.
Qualifications
Must possess a nursing degree from an accredited college or university.
Must possess a current, unencumbered, active license to practice as an LPN in this state.
Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guidelines that pertain to nursing care facilities.
Experience with Electronic Medical Records and computer documentation systems.
Maintain confidentiality of all resident care information in accordance with HIPAA guidelines.
Must be able to read, write and speak English fluently.
Must have an active CPR/BLS Certification.

Admissions Process
* Receive leads from providers, specialty physicians, county offices on Aging and Veteran service organizations, facilities, or any customer source and convert appropriate inquiries to admissions.
* Coordinate inquiries from all hospitals in the market.
* In conjunction with facility requirements/abilities, clinically access patients to determine if the patient meets admission criteria.
* Make contact with potential inquiries as a representative of Avalon Health Care. Review charts, and assist with any questions from the case management team, referral source and/or family members.
* Obtain the necessary information (medical and financial) to qualify patients for admission.
* Facilitate patient placement based on clinical services needed, bed availability, patient/family preference, geographic location, etc.
* Troubleshoot areas of Admission, Public Relations, pre-certification and physician follow-up in conjunction with facility.
* Conducts needed assessments.
Sales & Marketing
* Manage referral sources and relationships in accordance with the facilities census development plan.
* Conduct daily visits to referring hospitals.
* Build and maintain relationships with Discharge Planners, Case Managers, Physicians and all other potential referral sources.
* Identify new and potential referral sources.
* Identify potential Physician relationships. Develop them through marketing, partnership, follow up.
* Maintains thorough knowledge of the market in general and the specific products, services and acuity trends within the market.
* Identify options for partnering with other providers in the market to increase facility census.
* Coordinates community education and events for hospitals, physicians and other referral sources.
* Coordinate tours at facilities for new referral sources families and follow through as appropriate.
* Follow up with referral sources with case studies and updates using the "Marketing the Avalon Way" reporting system.
* Develop quarterly marketing plan, updated monthly.
* Submit weekly reports/goals as required.
Qualifications
* Associate and/or Bachelor's Degree in health care field. Preferably Nursing.
* Preferably 1-3 years of clinical nursing experience with at least two years in long term care and two years of successful experience in administration, consultation or involvement in the survey process.
* Customer service, sales and marketing experience preferred.
Licenses/Certification
* Must have an RN in good standing
* Must meet all local health requirements for entering the hospital settings. As necessary, any drug screenings, tests, lab work or physical examination. If required.
