The successful candidate will be responsible for coordinating the center's wound care and oversee the general application of the skin management program and treatment care.
Wage $50.00 - $56.99 per hour

Responsibilities
* Provide input for individualized plan of care in collaboration with the resident/responsible party and interdisciplinary care team by documenting problems, statements, goals, and approaches.
* Define realistic and obtainable long and short-term goals for the residents.
* Initiate appropriate nursing interventions with time frames for outcome evaluations and document appropriately in medical record.
* Revise plan of care with input from resident/responsible party and interdisciplinary care team.
* Implement teaching with resident/responsible party consistent with interdisciplinary plan of care.
* Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices.
* Transcribe physician and/or other health care professionals' orders appropriately and accurately.
* Ask for clarification of questionable orders from the ordering physician and/or other health care professionals.
* Notify physician and/or other health care professionals if orders are not carried out and document event appropriately.
* Perform only those activities which are within the scope of RN nursing practice.
* Follow company, facility and departmental policies and procedures, and appropriately utilize nursing judgment.
* Ensure accurate documentation of all medical records and reporting forms.
* Promptly respond to call lights and other resident needs.
Qualifications
* Must hold and maintain a current license to practice as a RegisteredNurse in practicing state.
* Recent clinical experience, education, or specialty skills specific to geriatrics.
* Demonstrate working knowledge of infection control principles and practices.
* Ongoing pursuit of Continuing Education Credits in clinical subjects, management, personal growth and development.
* Proven ability to communicate effectively and positively with staff, residents, and family members.
* Ability to adapt to a flexible work schedule.

Provide training and education of staff in all areas of health-related services, policies and procedures as assigned. The Regional RN Care Coordinator supports multiple communities, providing RN services to include on-call support and phone triage, nursing assessments, evaluating and documenting significant changes in condition, performing wound assessments, and conducting on-site routine clinical reviews.
ESSENTIAL DUTIES AND RESPONSIBILITIES
Completes or oversees nursing assessments required for significant changes of condition, in accordance with applicable regulations. Assesses health, functional and psychosocial status of residents. Participates in the development of individualized service plans.
Supports multiple communities as assigned, providing RN oversight and ensuring timely completion of required nursing evaluations and documentation. Responsibilities include phone triage, on-call support, wound assessments, significant change evaluations, and routine clinical reviews.
Provides training of staff in health-related services including but not limited to providing personal care and medication management.
Oversees the documentation and communication of resident care and services. Assists the management team to ensure resident records are completed in a timely manner and maintained according to company policies and state regulations.
Delegating nursing tasks may be required, as permitted under applicable state nursing laws and practice acts, with appropriate communication maintained with residents, their families, and health care practitioners.
Maintains and oversees medication services including staff training, staff oversight, pharmacy coordination, medication room and medication systems.
Maintains up-to-date knowledge of all rules, regulations and policies pertaining to resident care. Stays informed of industry and health care trends and standards, applying new knowledge to meet the changing needs of residents. Seeks out additional expertise and advice when needed to assure residents' needs are met and proper practices followed.
Performs other duties as assigned.
