A Brief Overview
Serves as a member of the provider's Care Team in coordinating the flow of patients through assisting provider with various responsibilities as requested, managing supplies and equipment, fielding and triaging phone call, and other front office tasks.
What you will do
* Rooms patients, takes vitals, prepares patient for physician, assists physician when needed. Accurately completes documentation for charts.
* Maintains and views patient records, charts, and other pertinent information to include, but not be limited to, reviews all lab, hospital and other reports, and posts in the electronic medical record as necessary. Prepares result letters for patients as directed by physician.
* Maintains exam rooms for necessary supplies and materials. Ensures cleanliness. Maintains and cares for all equipment in a responsible manner. Reports any repairs as needed.
* Assists the provider and nurse scheduling of tests and procedures and with other support duties as time allows, including but not limited to: making copies, opening mail, completion of disabilities forms, death certificates, completing DME forms, and fax items as requested.
* Requests charts as needed for telephone calls and as provider requests.
* Accurately and efficiently checks in patients following applicable policies and procedures. Answers billing questions.
* Answers multiple line telephone and routes calls promptly. Alerts appropriate personnel to urgent messages/issues that require immediate attention.
* Maintains computer appointment scheduling and follows office scheduling policies/parameters. Responsible for changing scheduling templates when needed.
* Assists non-ambulatory patients to lab and x-ray as needed.
* Appropriately obtains referral from insurance, documentation of the referral and communicates the information to the patient, physician, and/or patient's family as necessary.

Required Education, License/Certification, or Work Experience:

Job Summary
The restorative nursing aide serves as caregiver to the resident during the scheduled work period in long term care. Uses appropriate mobility, balance and strengthening exercises, incorporating into therapeutic activities. Includes using adaptive equipment, fall prevention, appropriate turning, positioning and repositioning techniques, basic range of motion exercises, and good body mechanics. Assists with training and implementation of special programs designed to improve resident's quality of life, ability to perform daily activities and maintain their highest functional level of independence. Obtains resident's report prior to providing care. Documents resident's progress of care and behaviors, recognizes and reports changes in resident's condition promptly.
Includes dressing/undressing programs, grooming, bathing, toileting, exercise/movement, and eating/swallowing programs. Demonstrates correct use and application of ice and paraffin baths, heat packets etc. according to state specific regulations. Uses the EMR accurately. Handles linens properly and disposes of soiled linens, clothing or supplies in adherence to regulations and guidelines: uses standard precautions by using personal protective equipment and observes transmission guidelines, washes hands between residentcares, report signs/symptoms of resident infections to appropriate staff in a timely manner.
Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use.
Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing. Completion of a C.NA training program, training if required by state and at least 40 hours of specific training related to restorative care procedures (may be provided by licensed therapists providing service within the organization).
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor
vehicle report and proof of valid driver's license. Also must be medically qualified to perform the essential driving functions of
this position as per our Sanford Driving Policy per the leaders request.

Completion of a C.NA training program, training if required by state and at least 40 hours of specific training related to restorative care procedures (may be provided by licensed therapists providing service within the organization). Qualifications
High school graduate or general equivalency diploma (GED) preferred, but not required.
Minimum age of 16. Prior clinical or nursing assistant experience in long-term care preferred.
Required Certified Nursing Assistant (CNA) certification with the State Board of Nursing, or state certified CNA within four months of date into the position by being enrolled in a training program or in the process of obtaining certification through Endorsement. CNA training and certification required and provided by facility.
Satisfactorily completes yearly department specific competency testing and maintains a record of continuing education credits for each year. A minimum of 12 hours continuing education are required annually. Maintains all department specific required certifications.
When applicable, for applicable states, certified and registered with the Unlicensed Assistive Person (UAP) Registry.
Dependent on facility and required responsibilities, this role may classify as a Sanford Category II Driver requiring a motor
vehicle report and proof of valid driver's license. Also must be medically qualified to perform the essential driving functions of
this position as per our Sanford Driving Policy per the leaders request.

Job Summary
The Nursing Assistant (NA) serves as caregiver to the resident during the scheduled work period in long term care. Provides resident-centered nursing care and daily living assistance to assigned resident under the supervision of a registered nurse (RN). Knowledge of and delivers age-appropriate care related to the physical and psychological needs of the resident as per care plan. Considered a member of the nursing team and is expected to know, and will be held accountable for, following infection prevention and control policies and Personal Protective Equipment use. Provides assistance with basic health care needs including daily living activities that may include, but are not limited to, bathing, toileting, grooming, dressing/undressing, obtaining and recording vital signs, and providing psychosocial support and other personal care to assigned resident. Assists the resident in transferring, repositioning, and walking using correct and appropriate transfer techniques and equipment and also provides range of motion and passive exercises. A Brief Overview
Accountable for preparing, administering and documenting medication administration in accordance with established policy and procedure under the supervision of a licensed nurse. Provides age appropriate personal care to residents and maintains a sanitary, orderly and attractive environment.
What you will do
* Provides assistance to residents with their activities of daily living.
* Performs routine duties and non-routine duties as directed by nursing staff. This may includes but not limited to assisting patients with bathing and dressing needs, shaving, oral hygiene, bed making, transfers into and out of bed, repositioning, toileting needs, serving and collecting food trays, encouraging and/or assisting with feeding, answering call lights, insuring a clean environment, transporting patients upon admission and dismissal, assisting with ambulation and orienting/reorienting of resident to the facility environment to insure resident safety.
* Performs blood pressure, pulse, temperature, respiration and weight. Documents concise and factual information on the appropriate forms.
* Performs or assistsresidents in performing specific medical care activities in accordance with established policy and procedure to contribute to the patient's plan of care. This includes assistingresidents in performing restorative exercises, collecting specimens as directed by nursing staff. Assessing and documenting completed cares. Provide feedback to nursing staff immediately on status or changes in patient conditions, pain management, and other important concerns.
* Assists with restocking supplies and maintaining appropriate work environment in unit including cleaning procedures.
* Participates in providing emotional and spiritual support for families.
* Complies with all aspects of resident rights including confidentiality and privacy. Report all allegations of resident abuse or misappropriation of resident property.
* Understanding and following established policy and procedures related to fire, tornado, evacuation, disaster plan, universal precautions, infection control procedures, hazardous and chemical communication program. Maintains an environment that is clean and safe for all residents, families and staff.
* Prepares, administers and documents medication administration in accordance with established policy and procedure under the supervision of a licensed nurse. Documents reasons prescibed medication are not administered and reports this to the charge nurse.
