Responsibilities:
Work closely with AWARE's Clinical Professionals in providing services as assigned and developed through prescribed curriculum lesson plans.
Spend time with client and family as assigned and/or as identified on the treatment plan.
Assesses client, family and environments utilizing assessment tools.
Coordinates and conducts the team process to develop a comprehensive Individualized Treatment Plan.
Provide services to identified children and families in order to assist in gaining access to:
- Medical Services- Social Services- Educational Service- Vocational Services- Recreational Services
