The VCU Health System's Department of Neurology is seeking a full time Nurse Practitioner or Physician Assistant (Advanced Practice Provider (APP)) for the Neuroscience Intensive Care Unit (NSICU). Advanced provider skills and judgment are necessary for this position. The APP will function as an essential member of a multidisciplinary team providing ongoing neurocritical care, consultative services and/or direct patient care to critically ill adult inpatients, including patients with complex neurological and neurosurgical disease, respiratory failure and/or critically illness. Hours will include days, weekends and holidays.
Responsibilities
Ability to make autonomous clinical judgments and independently implement management plans with minimal physician supervision. Responsibilities will include management of mechanical ventilation, tracheostomies, infections, and multiorgan critical care, ordering and interpretation of diagnostic tests and procedures, and managing other critically ill patients in an ICU setting (including but not limited to: intracranial hemorrhage, ischemic stroke, status epilepticus, perioperative neurosurgical care, septic shock, hemorrhagic shock, liver failure, renal failure, acute respiratory failure, and multiple organ failure). Job Requirements:
Minimum of 2 years' experience in Cardiothoracic Surgery as a board-certified, licensed Physician Assistant or Nurse Practitioner. This is a dayshift position, 4/10-hour shifts/week with potential for 3/12-hour shifts/week. Under the direction of a Cardiothoracic Surgeon and within established departmental practice guidelines, this Advanced Practitioner is responsible for performing a variety of duties.
Responsibilities will include but are not limited to pre-op evaluation of patients and preparation for surgery, first and second assisting in cardiac and thoracic surgery, endoscopic vein and radial artery harvesting for bypass, as well as bedside assisting for robotically assisted cases. Additionally, the candidate must be able to make rounds in the SICU and telemetry floor, efficiently progressing the patient toward discharge. The candidate will also assist with pre- and post-op patients in our outpatient clinic. In addition to the operating room duties, the candidate must be proficient at removing chest tubes, pacing wires, providing wound care, general procedures utilizing local anesthesia, new consults and discharging patients completing discharge summaries. Experience with managing mechanical assist devices including durable LVAD, Impella and ECMO is desirable but not required. Experience with Intensive Care Management of the postoperative Cardiothoracic Surgery patient including familiarity with ventilators, inotropes and vasoactive medications, and circulatory support devices and point of care ultrasound/echocardiogram. This candidate must also be willing to learn new skills such as the management of circulatory support devices (i.e. ECMO, Impella) in response to the needs of the program.
Licensure or eligible for licensure in the state of Virginia
Current BCLS or ACLS certification

Documentation in electronic health record - Epic
Theoretical background in anatomy, physiology, general medicine, pulmonary, emergency, and critical care medicine, management of common diseases, concepts of medicine and surgery, therapeutics including pharmacology, counseling techniques and interpersonal dynamics.
Scope of Practice:
Perform first and second assistance with cardiothoracic procedures including endoscopic vein and radial artery harvest.
Perform central line, arterial line and chest tube placement and removal.
Perform necessary bedside procedures as directed and supervised by attending physician including removal of pacing wires, intra-aortic balloon pumps and point of care ultrasound/echocardiogram.
Perform physical examinations and assessments of health with a knowledge of normal versus abnormal findings.
Develop disease specific patient care plans.
Order, interpret lab and radiologic diagnostic studies.
Prescribe appropriate medications/dosing ranges.
