This position will further be required to support AOT programs in providing advanced-level consultative expertise towards addressing high-risk cases in the AOT program, including identifying crisis situations and determining appropriate clinical intervention policies; and identifying complex cases and determining appropriate clinical intervention policies. This position will utilize expert clinical and therapeutic knowledge to support AOT Care Coordination Teams in the development of individualized treatment plans for individuals with severe behavioral health conditions for the AOT program. This position is an advanced clinical leadership position responsible for providing expert support and oversight to multiple AOT programs across a designated region. This position will work in close cooperation with public and private organizations, including state agencies, local AOT teams, healthcare providers, and the judicial system, to enhance program efficiency and ensure individuals enrolled in AOT receive appropriate treatment to achieve optimum social and health outcomes. Additionally, this position plays a key role in applying expert clinical knowledge towards standardizing best clinical practices across jurisdictions, troubleshooting barriers to care, and driving system-wide improvements in service delivery.     2024-25 School Social Worker Department: Special Education and Student Services Reports to: Director of Special Education Salary: Unit I Salary Scale -Dependent upon degree and experience The School Social Worker will be responsible for addressing the mental, emotional, social, and physical well-being of students ranging from elementary to high school by providing case management when a student is working with outside providers and/or special programs. The School Social Worker must have the ability to work with students, parents, school staff, teams, and community partners to offer support to the family and maintain a strong school connection. Qualifications * Master's Degree in Social Work or a Master's Degree or Doctorate in professional counseling or a closely related field. * Experience in the areas of case management, wraparound services, and knowledge of community resources. Essential Job Functions * Provide interventive, consultative, and follow-up services in a variety of situations as needed; * Provide case management services to students/families that focus on mental health, physical wellness, resiliency, and coping skills; * Maintain and collect data on evidence-based interventions and their outcomes; * Maintain confidentiality and exercise sound judgment. Conducts on-site inspections of living areas, coordinates and participates in relief efforts.
